ABO-Incompatible Renal Transplantation with High Antibody Titer: A Case Report.

BACKGROUND Even though renal transplantation across blood groups is not uncommonly practiced nowadays, there is still hesitation regarding ABO-incompatible transplantation with very high baseline antibody titer. In this case report, the outcome of an ABO-incompatible kidney transplant recipient with a high baseline isoagglutinin titer is reported. CASE REPORT The patient was a non-diabetic, 33-year-old man with end-stage renal disease secondary to chronic glomerulonephritis. The only kidney donor available was his mother, who was blood-group incompatible. The patient's blood group was O positi

Anti-HI can cause a severe delayed hemolytic transfusion reaction with hyperhemolysis in sickle cell disease patients.

BACKGROUND: Delayed hemolytic transfusion reaction (DHTR) is a life-threatening condition in sickle cell disease (SCD) patients that is frequently complicated by hyperhemolysis. Antibodies resulting from antigen disparity between donors of European ancestry and patients of African ancestry are common, but situations involving antibodies not classically of clinical significance are also encountered. Anti-HI is generally considered to be an innocuous naturally occurring antibody. STUDY DESIGN AND METHODS: We describe two cases of hyperhemolysis with anti-HI and provide details of the reported c

Pathogenesis in ABO incompatible liver transplantation: a clinicohistological evaluation of four patients.

The aim of this study was to clarify the pathogenesis of antibody-mediated rejection (AMR) of ABO-incompatible liver transplantation (ABO-I-LT). We investigated, within one month of surgery, the clinical courses of 10 patients who received ABO-I-LT. We encountered four cases of AMR, which were classified into two groups according to the stage of the AMR: early (within the first 14 postoperative days [PODs]) or late (after the 14th POD). There were three patients in the early stage, and one patient in the late stage. Three early-stage AMR patients had both hyperbilirubinemia and thrombocytopeni

A new concept of accommodation in ABO-incompatible kidney transplantation.

This work focuses on the mechanism of acute antibody-mediated rejection leading to graft loss and the mechanisms of accommodation permitting graft survival in ABO-incompatible kidney transplantation. As previously noted, accommodation occurs only with (i) post-transplant suppression of glycosyltransferase, a product of ABO histo-blood group genes in the graft and (ii) prevention of antigen-antibody reactions and delayed hyperacute rejection due to reduced antigenicity of enzyme-regulated histo-blood group antigens. This article discusses the mechanism of ABO histo-blood group glycosyltransfera

Tacrolimus (FK 506) induced thrombotic thrombocytopenic purpura after ABO mismatched second liver transplantation: salvage with plasmapheresis and prostacyclin.

We report the course of thrombotic thrombocytopenic purpura (TTP) in a patient receiving tacrolimus (FK506) immunosuppression for an ABO mismatched second liver graft. A Chinese woman with fulminant hepatitis-B reactivation failed a living-related orthotopic liver transplantation (OLT) due to portal vein thrombosis. An ABO mismatched cadaveric OLT (group A to O) was performed, with peri-operative plasmapheresis to reduce anti-A hemagglutinin titers. On day 30, she developed fever, hemolysis, thrombocytopenia and neurologic dulling. Prominent microangiopathic features in peripheral blood film,

Blood group change in a patient with blastic transformation of a myelodysplastic syndrome.

Loss of certain red blood cell antigens has been described in patients with acute myelocytic leukemia (AML). This paper describes the loss of blood group A antigen in a patient with AML. The acute leukemia in this patient was preceded by a myelodysplastic syndrome for several months. At the time of diagnosis the patient's red cells showed the 0 Rh(D)+ phenotype. After induction of complete remission with two courses of Daunorubicin, Cytosin-arabinosid, and Etoposid his blood group reverted to A2. Serological studies including saliva analysis revealed that the original blood group was very like

[Incidence, laboratory diagnosis and serologic prediction of hemolytic disease of newborn infants due to ABO incompatibility].

The frequency of the haemolytic disease of the newborn due to ABO-incompatibility (ABO-HDN), requiring treatment, is with approximately 5% of all hyperbilirubinemias in newborns lower than usually expected. Contrary to common conception the serologic diagnosis of ABO-HDN by means of the direct antiglobulin test (DAT) is possible with satisfactory accuracy. The enormous laboratory burden of IgG-anti-A, -B testing during pregnancy is in poor conformity with clinically demonstrable illness. This discrepancy is due to the rarity of true ABO incompatibilities and not to the ineffectiveness of labor

Serological complications of a major ABO incompatible bone marrow transplantation in a Polynesian with aplastic anemia.

A 24-year-old, blood-group O Polynesian woman with multiple red blood cell antibodies received a group A, HLA-compatible, sex-matched bone marrow transplantation as treatment for severe aplastic anemia. High retransplantation titers of ABO isohemagglutinins were successfully reduced by repeated plasmaphereses. Despite gross in vitro incompatibility in the routine crossmatch, technetium-labelled red blood cell survival studies performed immediately prior to transplantation predicted that a small proportion of the donor erythrocytes would neutralize residual postplasmapheresis isoantibody. Donor

Genetic markers in Welsh gypsies.

Data are presented on polymorphic genetic markers of 84 Welsh gypsies, members of an inbred population characterised by a high incidence of phenylketonuria and other recessively inherited diseases. Sixteen polymorphic loci were studied: the ABO blood group distribution was comparable to that of the surrounding population, the B gene frequency being 7-5%, a lower frequency than most earlier studies in gypsies. The haptoglobin 1 gene frequency (23-7%) was very different from that of the British population, resembling the frequency found in India and some previously reported gypsy groups. The dat

Frequency of ABO/Rhesus (D) blood groupings and ethnic distribution in the Greater-Accra region of Ghana, towards effective blood bank inventory.

Knowledge on blood group phenotypes is of key importance in clinical practice. It used in blood transfusion practice to determine the direction of recruitment of voluntary donors as required for each population within a country, and for disease association and population genetics studies. This study aimed at reporting the frequency of ABO and Rhesus (Rh) groups in the population of the Greater-Accra region of Ghana and amongst their various ethnic groups. A retrospective study in 11 main hospitals within the region was done. Data collected provided information on the blood group status of pers

ABO-incompatible heart transplants.

A month-old baby girl with blood type O positive received a donor heart organ from a donor with blood type B. This was the first institutional ABO-incompatible heart transplant. Infants listed for transplantation may be considered for an ABO-incompatible heart transplant based on their antibody levels and age. The United Network of Organ Sharing (UNOS) protocol is infants under 24 months with titers less than or equal to 1:4.(1) This recipient's anti-A and anti-B antibodies were monitored with titer assays to determine their levels; antibody levels less than 1:4 are acceptable pre-transplant i

Importance of Direct Antiglobulin Test (DAT) in Cord Blood: Causes of DAT (+) in a Cohort Study.

BACKGROUND: The direct antiglobulin test (DAT) is the cornerstone of the diagnosis of hemolytic disease of the newborn (HDN). The aim of this study was to review the incidence and causes of positive DAT in cord blood in relation to development of HDN. METHODS: We retrospectively reviewed all results of DAT, which is routinely performed in cord blood samples, along with the laboratory and infants' medical records. RESULTS: DAT was positive in 70/2695 (2.59%) cases. In 64/70 (91.43%) cases, DAT positivity was attributed to ABO incompatibility. There were 50/218 (22.93%) DAT (+) cases in the A/

Spectrum and outcome analysis of marked neonatal hyperbilirubinemia with blood group incompatibility.

BACKGROUND: Blood group mismatch between a mother and newborn carries a substantial risk for neonatal hyperbilirubinemia and kernicterus. In the current study, we investigate the spectrum and outcome of marked neonatal hyperbilirubinemia with blood group incompatibility. METHODS: We retrospectively assessed a cohort of 413 neonates with peak total serum bilirubin (TSB) values > or = 20 mg/dL between 1995 and 2007. Those with a gestational age< 34 weeks, birth weight < 2000 grams or G6PD deficiency were excluded. A total of 83 subjects with blood group incompatibility were enrolled. Neonates w

[Current validity of the exchange transfusion intervention by the classic indication. Our 15 years' experience].

In this paper the Authors report their personal experience of the use of exchange transfusion, secondary to classic indication, in the treatment of neonatal hyperbilirubinemia, in order to: 1) determine the trend over the past wears in the number of exchange transfused neonates, both from a global point of view and in relation to indications; 2) critically assess the risks, in terms of complications and mortality, correlated to exchange transfusion. Four hundred and eighty-eight neonates, who were subjected to 693 exchange transfusions in the Pediatric Clinic and Neonatal Division of the Polic

Significance of anti-A and anti-B isohemagglutinins in cord blood of ABO incompatible newborn infants: correlation with hyperbilirubinemia.

Transcutaneous bilirubin index measurements (TcBI) were determined in ABO-incompatible term white newborn infants who had an indirect Coombs' test for detection of ABO incompatibility (ABO test) performed on cord blood. Serial transcutaneous measurements in the first 72 hours of life in 63 ABO test positive (group I) and 31 ABO test negative (group II) infants were compared with a control group of 109 term ABO compatible infants (group III). During the first phase of this study, the accuracy of TcBI was confirmed in 33 infants who had serum bilirubin determinations demonstrating a correlation

Massive acute hemolysis secondary to Clostridium perfringens sepsis in a recently transfused oncology patient with multiple alloantibodies.

Sepsis with clostridia species is seen infrequently in oncology patients, and the massive acute intravascular hemolysis known to be associated with these organisms is rare. The urgency of making the diagnosis of clostridial sepsis is imperative since the course is rapidly fatal if untreated. We describe a patient with metastatic adenocarcinoma of the pancreas, known to have recent transfusions as well as multiple red cell alloantibodies, who developed massive nonimmunologic hemolysis associated with Clostridium perfringens sepsis. Because the hemolysis of C. perfringens sepsis can mimic that o

Effects of exchange transfusion on platelet counts.

With the purpose of knowing to what extent thrombocytopenia that occurs in exchange transfusion (ET) can be secondary to the procedure itself or to a generalized infection, blood counts were made in 42 neonates with hyperbilirubinemia due to maternal fetal isoimmunization of the ABO system. Platelets were quantified halfway and immediately after the exchange transfusion procedure as well as 8, 16, 24, 48 and 72 hours after. It was found that prior to ET platelet levels in donor and patient blood was similar and platelet counts were never under 100,000 mm3 (thrombocytopenia). Halfway and at the

A study of ABO incompatibility and neonatal jaundice in Black South African newborn infants.

In order to establish the role of fetomaternal ABO incompatibility as a cause of overall neonatal jaundice, its relationship to the degree of jaundice, and its etiologic contribution to severe jaundice necessitating exchange transfusion in South African blacks, a retrospective analysis of both laboratory and clinical data was undertaken. Two thousand forty-two blood samples were submitted for laboratory investigation from mothers and their jaundiced newborn infants and categorized according to ABO blood groups and levels of serum bilirubin. At levels of serum bilirubin below 301 mumol/l, ABO i
