Historically, the ipsilateral acoustic reflex (IAR) has played a prominent role in middle ear screening protocols. However, in 1990, the American Speech-Language-Hearing Association's revised guidelines, "Screening for Hearing Impairment and Middle Ear Disorders," deleted the IAR as a screening parameter. This decision was based on studies that demonstrated that the inclusion of the IAR resulted in a high false-positive rate without adding to the hit rate and thus was a major contributor to over-referral. However, other investigations demonstrated the converse, that the IAR is a sensitive and

BACKGROUND: The human tympanic membrane has reasonably good sound transmission properties and withstands high static pressure loads. Destruction of the tympanic membrane resulting from middle ear diseases or trauma may be repaired by different types of grafts. Middle ear surgery mostly uses autologous temporal fascia, cartilage, or cartilage perichondrium transplants which differ in their acoustical characteristics and mechanical strength.
METHODS: We have investigated the acoustical and mechanical properties of these materials and compared them with human tympanic membranes by constructing a

Adenoidal hypertrophy is the commonest cause of nasal obstruction in the pediatric population. It may cause marked morbidity as regards respiratory physiology, facial growth and middle ear function. Determination of adenoidal presence and size is not easy. Nasal endoscopy and radiology are the most accepted modes of diagnosis and each has its disadvantages. We have used acoustic rhinometry to determine the size of adenoids. Changes in nasal volume and resistance were recorded and an easy formula was devised to determined adenoid size. This technique is easy, non-invasive and reproducible with

OBJECTIVE: In this article, we review the surgical outcomes of 179 patients with acoustic neuromas.
METHODS: Most of the tumors (84%) were operated on using a retrosigmoid, transmeatal approach. A transpetrosal, retrosigmoid approach was used in 10% of the patients, most of whom had large tumors. The translabyrinthine (4%) and transmastoid, transpetrosal, partial labyrinthectomy approaches (2%) were used selectively. The operative approaches are discussed. Tumors were categorized according to their cerebellopontine angle dimensions as small (< 2 cm), medium (2.0-3.9 cm), and large (> or = 4 c

The purpose of the present study was to describe the effects of acute laryngitis on some aerodynamic, acoustic, and perceptual measures. Eleven subjects with diagnosed acute laryngitis due to upper respiratory infection were recorded during a laryngitic episode and 1 week to 10 days after amelioration of the laryngitic condition. Fundamental frequency, values, collapsed across the five vowels, were significantly reduced in the laryngitic compared with the normal speaking condition. The decrease in fundamental frequency associated with acute laryngitis suggests an increase in the mass of the vo

Distortion-product otoacoustic emissions (DPOAEs) are otoacoustic emissions evoked by two pure tones. The greatest advantage of DPOAEs is their frequency specificity with respect to the eliciting bitonal stimuli. The purpose of this study was to compare DPOAEs in two populations. This paper reports input/output DPOAEs functions and DPOAE-audiograms for audiometric frequencies of f2 between 696 and 6006 Hz in a normal neonate population and in an adult control group. Fifteen healthy fullterm newborns (29 ears) and 8 normal-hearing adults (16 ears) participated as subjects. Results at medium fre

A questionnaire was sent to 134 patients who had undergone surgery for a unilateral acoustic neuroma between 1980 and 1993, to obtain data on the consequences on their quality of life, physical condition, social life, employment, and use of medical facilities. Distinctions were made between the translabyrinthine-transotic approach, the suboccipital approach, the tumor size, and the number of operations per patient. We found that the patients' reported state of health after surgery was poorer than that in a group of comparable nonoperated patients. Recuperation after an operation took many mont

Intraoperative auditory monitoring is a useful adjunct that is currently evolving. Near-field monitoring techniques such as electrocochleography and direct eighth nerve compound action potential are being used more frequently. The use of these two techniques is compared in 26 patients undergoing hearing preservation acoustic neuroma resection. Overall, 9 (35%) of the 26 patients had their hearing preserved. Three (23%) of the 13 with electrocochleography monitoring and 6 (46%) of the 13 with direct compound action potential monitoring had hearing preserved after surgery. Although there was a s

The authors describe the case of a 71-year-old man without signs of phacomatosis with a history of progressive psychomotory lowering and two simultaneous tumors of the olfactory groove and of the cerebellopontine angle, as demonstrated by CT scans. Histologic examinations showed the lesions to be a meningioma and a neurinoma. The rarity and the possible pathogenesis of this association are discussed.

The acoustic reflection method provides a noninvasive way to determine the airway geometry. Based on a discrete upper airway model, an inverse scheme is developed to infer the upper airway area as a function of distance. We incorporate this scheme into a system that can generate multiple acoustic pulses to sample the upper airway geometry at a maximum frequency of 30 Hz, making possible determination of the airway area-distance relation as a function of time. Therefore, we can monitor the dynamic behavior of the upper airway during breathing. To validate the approach, we visualized vocal cord

OBJECTIVE: To compare morphologies of crossed and uncrossed acoustic reflex waveforms.
DESIGN: Subjects were 12 young adults with normal hearing. A signal-averaging technique was used to compare onset and offset latencies of crossed and uncrossed acoustic reflex waveforms when maximum amplitudes were matched.
RESULTS: Onset latency was similar for the two modes, but offset latency was significantly longer for the crossed reflex. Further, in individual subjects the difference between crossed and uncrossed offset latencies was inversely proportional to maximum reflex amplitude.
CONCLUSION: Re

Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the cranial nerves at the brainstem in the majority of cases. Trigeminal neuralgia occurring in 3.3% of acoustic neurinomas is usually assumed to be a sign of large tumour size; if associated with small tumour size, an additional pathology, such as typical vascular compression must be suspected and has to be explored at surgery. While facial paresis will usually lead to immediate radiological diagnosis of a possible cerebellopontine angle (CPA) neoplasm, facial spasm is usually not expected to be associated with a

In 95 percent of the cases, patients with acoustic neuromas will have some magnitude of hearing loss in the affected ear. This paper reports on four patients who had acoustic neuromas and normal hearing. Results from the case history, audiometric evaluation, auditory brainstem response (ABR), electroneurography (ENOG), and vestibular evaluation are reported for each patient. For all patients, the presence of unilateral tinnitus was the most common complaint. Audiologically, elevated or absent acoustic reflex thresholds and abnormal ABR findings were the most powerful diagnostic tools.

Patients with acoustic neuroma may have sudden sensorineural hearing loss. Most patients with sudden hearing loss seek medical attention promptly, but the diagnosis of an acoustic neuroma may be delayed for months or years because sudden hearing loss is an unusual initial symptom of an acoustic neuroma. In a retrospective review of 836 cases of sudden hearing loss, we found 13 patients with acoustic neuromas. The prevalence of acoustic neuromas for those screened with auditory brain stem response or magnetic resonance imaging was 2.5%. In addition to these 13 patients, 79 acoustic neuroma pati

Experience gained in the treatment of acoustic neurinomas over a period of 43 years is presented. This is divided into three eras based on the prevailing knowledge and techniques. Advances in diagnosis, anaesthesia and microsurgical techniques have reduced the mortality to 4% in large tumours and to less than 1% in small tumours. With large tumours, facial nerve preservation has been possible only in 20% of patients. Large tumours continue to predominate, thus providing a formidable challenge. Saving life and the functions of the lower cranial nerves still form the main goal in the majority of
