Local anesthetic infusion through nerve sheath catheters for analgesia following upper extremity amputation. Clinical report.

BACKGROUND AND OBJECTIVES: Reports about the efficacy of local anesthetic perfusion of nerve stumps following lower extremity amputation are conflicting. We report our experience with this technique following amputation of the upper extremity. METHODS: Six consecutive patients undergoing proximal upper extremity amputations (four forequarter amputations and two shoulder disarticulations) for malignancy were prospectively observed. In all patients, catheters were placed within the amputated nerve sheaths at the conclusion of the procedure. Bupivacaine. 0.25%, was administered through each cath

The very long posterior tibial artery island flap.

Limb salvage procedures utilising tissue which would otherwise be discarded have become a well established practice in the management of trauma. These principles may also be utilised in the treatment of the burn injury. Two patients are presented with severe burns to the lower limbs involving muscle and bone where above knee amputation seemed inevitable. Salvage of the knee joint and conversion to a below knee amputation has been made possible by utilising the remaining skin of the sole of the foot based on a very long posterior tibial neurovascular pedicle ("Fillet of Sole" flap). This has pr

Knee disarticulation versus above-knee amputation.

If below-knee amputation is impossible, knee disarticulation should be considered before above-knee amputation, regardless of age and etiology. Knee disarticulation which leaves the femur and patella untoched offers many advantages. The surgical technique is simple and non-traumatic since no bone or muscle tissue is to be dissected. The thigh muscles are completely preserved and thus there is no muscular imbalance. The stump permits total end bearing and its bulbous shape permits easy and firm attachment of the prosthesis. A specially designed double-wall socket and various types of knee joint

New bone formation at amputation sites in electrically burn-injured patients.

Sixty-one amputation sites in 43 electrically injured men were examined for significant new bone formation. Twenty-eight amputations were classified as long bone (6 above knee, 4 below knee, 4 above elbow, 14 below elbow); 28 small bone (9 hand or finger and 19 foot or toe); and 5 disarticulations. Significant new bone formation occurred at the amputation site in 23 (82%) of the 28 long bone cases, but none occurred in the small bone or disarticulation cases. This difference was significant at the 0.0001 level (chi square = 40.53, 2 df). Fourteen (78%) of upper and nine (90%) of lower extremit

Through knee amputation with gastrocnemius musculocutaneous flap. 6 cases of tibial osteosarcoma followed for 3 (1-6) years.

6 cases of proximal tibial osteosarcoma were operated on by through knee amputation using a long dorsal gastrocnemius musculocutaneous flap; the usual anterior flap could not be used because of tumor size and the location of the biopsy tract. The only complication was one wound dehiscence which healed. The stump was of good quality in all patients. At follow-up, the patients walked without walking aids.

Polypoid traumatic neuroma of the gallbladder.

We report a case of polypoid traumatic (amputation) neuroma incidentally found in a gallbladder resected for cholelithiasis. This traumatic neuroma may have developed as the result of injury to the gallbladder during an unsuccessful cholecystectomy performed 20 years previously. Traumatic neuromas are known to arise at the stump of the cystic bile duct after cholecystectomy, but, to our knowledge, there seem to be no previous reports of traumatic neuromas arising in the gallbladder.

Effective local anesthesia for onabotulinumtoxin A injections to treat hyperhidrosis associated with traumatic amputation.

BACKGROUND: Botulinum toxin type A (BTX-A) injections are an effective treatment for controlling hyperhidrosis at sites of amputation. Hyperesthesia associated with amputated limbs is a major barrier to performing this procedure under local anesthesia. OBJECTIVE: To present a novel method for improving local anesthesia with BTX-A injections. Methods & RESULTS: A 29-year-old military veteran with a below-the-knee amputation of his right leg was suffering from amputation site hyperhidrosis, which was impeding his ability to comfortably wear a prosthesis. Prior to presenting to our clinic,

Verrucous hyperplasia of the great toe: a case and a review of the literature.

BACKGROUND: Verrucous hyperplasia is a hyperplastic disease that manifests as coalescent warty papules at stump amputation sites. Often overlooked, it can be confused with verrucous carcinoma. As such, it is of concern for dermatologic surgeons. OBJECTIVE: To present a patient with verrucous hyperplasia and review its literature. METHODS: We report a 62-year-old diabetic patient with a verrucous nodule that arose at the amputation site of her first three right toes. Despite repeated surgical removals and skin graftings, the verrucous nodule redeveloped. A biopsy revealed digitate epidermal h

[The value of the Syme amputation in selected cases of diabetic foot].

The aim of this paper was to assess the surgical and functional results of Syme's amputation in patients with infected necrosis of the fore- and midfoot caused by diabetes. The material consisted of 5 patients (4 men) age ranging from 44 to 78 years. No stump healing was achieved in a 44 year old man with arteriosclerosis who did not agree to a higher amputation. Prostheses were implemented in patients whose wounds healed 3-6 weeks after surgery. Follow-up period ranged from 2 to 6 years. Three patients died 4-5.5 years after surgery. One patient (equipped with a prosthesis) was observed for a

Increased excitability in the primary motor cortex and supplementary motor area in patients with phantom limb pain after upper limb amputation.

Using functional magnetic resonance imaging and single slice FLASH technique, we investigated reorganization of the hand representation of the primary sensorimotor cortex (SMC) in 16 patients with upper extremity amputation. Patients were asked to perform finger tapping with the intact hand, repetitive eye closing and anteflexion of the amputation stump or intact shoulder. Six normal volunteers served as control. In the normal volunteers activations during shoulder anteflexion, finger tapping and eye closure were located within the central sulcus in a medio-lateral fashion. Patients demonstrat

Continuous postoperative regional analgesia by nerve sheath block for amputation surgery--a pilot study.

A pilot study of continuous postoperative regional analgesia by nerve sheath block for lower limb amputation is presented. At the time of exposure of sciatic or posterior tibial nerve trunks during above- or below-knee amputations in 11 patients with ASA physical status III or IV, a catheter was introduced directly into the transected nerve sheath for continuous infusion of 0.25% bupivacaine at a rate of 10 mL/h for 72 h. Effective amputation stump analgesia was obtained, significantly reducing the need for on-demand narcotic analgesics during this time to a mean dose equivalent of 1.4 mg of m

Bilateral lower leg replantation versus prosthetic replacement: long-term outcome of amputation after an occupational railroad accident.

BACKGROUND: This retrospective study investigated three very similar cases of bilateral lower leg amputation. The aim was to determine which of two therapeutical procedures is associated with better long-term outcome: replantation or primary treatment of the stumps and subsequent prosthetic replacement. METHODS: Evaluation included clinical examination, gait analysis, and a workup of the psychosocial background. Health problems were documented using the Nottingham Health Profile. Follow-up assessments were performed 6, 7, and 18 years after the trauma. RESULTS: One patient underwent successf

Rehabilitation outcome 5 years after 100 lower-limb amputations.

Received wisdom commends a policy of maximizing the ratio of below-knee to above-knee amputations in patients with end-stage arterial disease. After adoption of this policy, the long-term outcome of 100 consecutive lower-limb amputations in 96 patients was monitored by annual review for 5 years. The ratio of primary below-knee to above-knee amputations was 2:1, with 9 per cent of below-knee amputations undergoing revision to a higher level. At 2 years after amputation only 26 per cent of patients were successfully walking out of doors, while 40 per cent had died. By 5 years 67 per cent were de

Amputation of finger by horse bite with complete avulsion of both flexor tendons.

Amputation of fingers with tendon avulsion occurs through a traction injury, and most occur through a ring avulsion mechanism. Usually the flexor digitorum profundus is torn out with the amputated finger. Replantation usually is recommended only when the amputation is distal to the flexor digitorum superficialis insertion. Animal bites are relatively common, with a decreasing order of frequency of dogs, cats, and humans. Horse bites are relatively infrequent but are associated with crush injuries and tissue loss when they occur. This article describes a 23-year-old man with amputation of his m

Evaluation of arterial obstructive leg and foot disease by three-phase bone scintigraphy.

BACKGROUND: The level selected for amputation should generally be the lowest compatible with tissue viability, with a reasonable expectation of wound healing in patients with arterial obstructive leg and foot disease, but determining the amputation level of an ischemic lower limb remains controversial. The general consensus is that a decisive and final decision about the amputation level should be made intraoperatively based on the extent of hemorrhage from the incised skin and soft tissue, and the degree of viability of the stump. OBJECTIVE: To estimate the extent of such hemorrhage, and thu

Pre-prosthetic Physiotherapy Rehabilitation in Post-operative Transtibial Amputation in a Patient With Congenital Talipes Equinovarus.

Amputation is more common in men than women, a lot of studies suggest this. It is the complete or partial removal of an extremity through a surgical process and is said to be a life-saving procedure performed in various critical conditions. The main objective of amputating a limb or any part at a level is that it should be carried out in a way that will give a stump of optimum length to facilitate the prosthetic fitting at later stages. After amputation, the patients are usually trained with prostheses so that they can carry on with functional activities without any restrictions. One of the di

Modular endoprosthetic implant for maximizing residual limb length: A case report.

BACKGROUND:: For transfemoral amputations, the residual femoral length is critically important to prosthetic function. The aim of this report is to describe a novel method of extending femoral length and to assess its stability over time. CASE DESCRIPTION AND METHODS:: A 57-year-old woman with recurrent parosteal osteosarcoma was treated with above-knee amputation. A portion of the distal femoral endoprosthesis, which included uncemented fixation with a Compress® stem, was retained in an effort to extend the short femoral remnant and maximize stump length. FINDINGS:: At 3 years follow-up, th

The periosteal requirement and temporal dynamics of BMP2-induced middle phalanx regeneration in the adult mouse.

Regeneration of mammalian limbs is restricted to amputation of the distal digit tip, the terminal phalanx (P3). The adjacent skeletal element, the middle phalanx (P2), has emerged as a model system to investigate regenerative failure and as a site to test approaches aimed at enhancing regeneration. We report that exogenous application of bone morphogenetic protein 2 (BMP2) stimulates the formation of a transient cartilaginous callus distal to the amputation plane that mediates the regeneration of the amputated P2 bone. BMP2 initiates a significant regeneration response during the periosteal-de

Imaging of the painful lower limb stump.

Several postoperative complications associated with pain may develop in the stump of an amputated lower limb. Clinical findings are often nonspecific; however, radiologic evaluation, especially with magnetic resonance (MR) imaging, is useful in the early diagnosis of these complications, thereby helping minimize physical disability with its psychologic and socioeconomic implications. Conventional radiography can demonstrate evidence of osseous origins of pain (eg, aggressive bone edge, heterotopic ossification, osteomyelitis) and should be the first imaging study performed after clinical exami

A multidisciplinary approach and surgical tips in major amputations of diabetic patients.

We aim to describe the results obtained in major amputations (transtibial and transfemoral) of diabetic patients followed with a close combined approach (medical and surgical). We evaluated 37 cases with an average age of 73.55 years. All were admitted in our Diabetology Department where they were monitored and treated in order to reach a new balance concerning cardiac failure, anemia, and other pathologies. Then, the orthopedic surgeons operate choosing an adequate level for amputation and pointing about adequate stump covering, accurate vessel, perineural and periosteal hemostasis, and nerve
