Charcot neuroarthropathy is often a devastating diabetic foot complication that poses a great risk for limb loss and can have a significant impact on a patient's quality of life in the presence of multiple existing comorbidities. It is a progressive and debilitating condition characterized by joint dislocation, pathologic fracture(s), and extensive destruction of the foot or ankle architecture secondary to dense peripheral neuropathy. This pathologic process can be idiopathic, secondary to acute trauma or previous surgery, or attributable to repetitive "microinjury." Once the Charcot process h…

UNLABELLED: Dislocation of the ankle joint without a concomitant malleolar fracture is rare. With all reported cases to date being unilateral injuries, we present the case of a 28-year-old male who sustained bilateral ankle dislocations without associated ankle fractures following a road traffic accident. Ten months after the trauma, the patient had returned to work with minimal sequelae related to the injury. To our knowledge, this is the first case of its kind to be reported.
LEVEL OF CLINICAL EVIDENCE: 4.

BACKGROUND: Malunited ankle fractures are uncommon. They produce symptoms of persistent pain, joint effusion, limitation of dorsiflexion and eventually lead to ankle arthritis. We feel that correction of the ankle alignment can improve the final outcome and present our results.
MATERIALS AND METHOD: From May 2004 to April 2006, seven patients with a malunited fibular fracture aged 25 to 62 years (male:female ratio, 5:2) were treated in our institute. All patients were referred for persistent pain. The delay between injury and the operative intervention was 3 to 16 months. All patients were as…

INTRODUCTION: Peripheral arterial disease is a coronary risk equivalent; a low ankle-brachial index (ABI) is indicative of systemic vascular disease, and should place a patient in the high-risk category. Few physicians measure ABI because it is technically challenging and time consuming. Oscillometric blood pressure monitors are readily available and easy to use. The use of a simple method of documenting ABI was assessed and compared with the conventional method.
METHODS: The oscillometric ABI (OABI) was measured for normal volunteers, patients attending a cardiovascular risk clinic (Cardiova…

BACKGROUND: The surgical treatment of posterior ankle impingement is associated with a high rate of complications and a substantial time to recover. An endoscopic approach to the posterior ankle (hindfoot endoscopy) may lack these disadvantages. We hypothesized that hindfoot endoscopy causes less morbidity and facilitates a quick recovery compared with open surgery.
METHODS: Fifty-five consecutive patients with posterior ankle impingement were treated with an endoscopic removal of bone fragments and/or scar tissue. The symptoms were caused by trauma (65%) or overuse (35%). All patients were e…

We present a series of 16 patients treated between 1993 and 2006 who had a failed total ankle replacement converted to an arthrodesis using bone grafting with internal fixation. We used tricortical autograft from the iliac crest to preserve the height of the ankle, the malleoli and the subtalar joint. A successful arthrodesis was achieved at a mean of three months (1.5 to 4.5) in all patients except one, with rheumatoid arthritis and severe bone loss, who developed a nonunion and required further fixation with an intramedullary nail at one year after surgery, before obtaining satisfactory fusi…

Syndesmotic ruptures associated with ankle fractures are most commonly caused by external rotation of the foot, eversion of the talus within the ankle mortise, and excessive dorsiflexion. The distal tibiofibular syndesmosis consists of the anterior inferior tibiofibular ligament, posterior inferior tibiofibular ligament, and interosseous ligament, and it is essential for stability of the ankle mortise. Despite the numerous biomechanical and clinical studies pertaining to ankle fractures, there are no uniform recommendations regarding the use of the syndesmotic screw for specific injury pattern…

We describe a new posteromedial portal through the bed of the posterior tibial tendon. It can be made easily with a 0.5-inch posteromedial skin incision along the course of the posterior tibial tendon just behind the posterior colliculus of the medial malleolus. After the flexor retinaculum is incised and the posterior tibial tendon is retracted anteriorly, a small bulging area of capsule can be seen as a result of saline inflation. The new posteromedial portal can then be made easily through this inflated capsule. This portal allows good access to the posterior joint surface and has a minimal

Each of the seven systems described has advantages and drawbacks that consist of ease or difficulty of application, accuracy, validity, and availability. This article should contribute to make these systems better known and easier to apply, and thus, will encourage their use in clinical practice. [Fig.: see text].

BACKGROUND: Chronic lateral ankle instability occurs in 10% to 20% of people after an acute ankle sprain. The initial form of treatment is conservative but if this fails and ligament laxity is present, surgical intervention is considered.
OBJECTIVES: To compare different treatments, both conservative and surgical, for chronic lateral ankle instability.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialized Register (to July 2005), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2005, Issue 3), and MEDLINE (1966 to April 2006), EMBAS…

Chronic ankle pain can be due to multiple causes. A thorough review of the patient's history with a physical examination concentrating on anatomic structures surrounding the ankle is imperative. The most common of causes have been presented. The addition of provocative testing and radiographic examinations can aid in elucidating the pathology. After treatment of the injury, attention to training technique, shoe and insert usage as well as individual gait abnormalities are integrated into global patient education to decrease the incidence of injury recurrence.

Ankle arthroplasty is emerging as an alternative to arthrodesis. Restoring and maintaining proper bony alignment and muscle balance in the limb is essential if there is to be any chance for long-term survival. Deformity can arise above the ankle, in the joint itself, or in the foot. Secondary procedures in the foot and leg, including osteotomies, fusions and muscle balancing, are performed either before or simultaneously with ankle replacement.

BACKGROUND: Ankle arthroscopy has generally been performed with use of anterior portals with the patient in the supine position. Little has been published on ankle arthroscopy performed with use of posterior portals, particularly with the patient in the prone position. The purpose of the present study was to evaluate the relative safety and efficacy of ankle arthroscopy with use of posterior portals with the limb in the prone position.
METHODS: Thirteen fresh-frozen cadaver specimens were used. Posterolateral and posteromedial portals were established. Arthroscopy was performed, and the exten…

Thirty-eight patients (mean age 49 years; range 19-91 years; nine of them over 60 years; 28 women, 10 men) suffering from an isolated Weber B fracture with a dislocation of less than 1 mm underwent functional therapy using a pneumatic ankle brace and were included in a prospective study. The clinical outcome was measured according to the Olerud-Molander ankle score. Functional therapy was finished in 34 cases successfully. Twenty-one patients were scored after 17 months on average (range 8-27 months) with the Olerud-Molander ankle score. A very good result was seen in 18 patients, including 12…

Pediatric ankle fractures account for approximately 5% of pediatric fractures and 15% of physeal injuries. The biomechanical differences between mature and immature bones, as well as the differing forces applied to those bones, help explain the differences between adult and pediatric fractures. The potential complications associated with pediatric ankle fractures include those seen with adult fractures (such as posttraumatic arthritis, stiffness, and reflex sympathetic dystrophy) as well as those that result from physeal damage (including leg-length discrepancy, angular deformity, or a combina…
