Immunoglobulin A deficiency following treatment with lamotrigine.

Lamotrigine (LTG) is an anti-epileptic drug and mood-stabilizing agent, whose adverse effects include skin rash and dizziness. Interactions with the immune system are rare, and only a few cases linking hypogammaglobulinemia to LTG treatment have been previously described. In this report, we describe a case in which a patient developed hypogammaglobulinemia, and a subsequent immunoglobulin A (IgA) deficiency, following LTG treatment. As a result of her immunodeficiency, the patient presented with a severe urinary tract infection and required intravenous immunoglobulin. Serum levels of immunoglo

Kidney function and the use of nitrofurantoin to treat urinary tract infections in older women.

BACKGROUND: The antibiotic nitrofurantoin is commonly used to treat uncomplicated urinary tract infections. However, when this drug is used by patients with reduced kidney function, its urine concentration may be subtherapeutic. METHODS: We conducted a population-based study of older women (mean age 79 years) in Ontario, Canada, whose estimated glomerular filtration rate was relatively low (median 38 mL/min per 1.73 m(2)) and for whom 1 of 4 antibiotics had been prescribed for urinary tract infection: nitrofurantoin, ciprofloxacin, norfloxacin or trimethoprim-sulfamethoxazole. We assessed 2 m

Prostatitis: Man's hidden infection.

Prostatitis exists when inflammation of prostatic glands and tissues results from infection or allergy. Gram-positive and negative bacteria cause most prostatic infections, but infections may also be caused by fungi, mycoplasma, viruses, and other nonbacterial infecting agents. Precise diagnostic localization of infection to prostatic glands is accomplished by obtaining divided urinary specimens and prostatic fluid and observing numbers of bacteria (or other infecting agents) present in each specimen. Treatment of prostatitis remains difficult because of several factors: patients may lack the

Predictors of patient response to antibiotic therapy for the chronic prostatitis/chronic pelvic pain syndrome: a prospective multicenter clinical trial.

PURPOSE: To our knowledge antibiotics are the most popular choice of therapy for all categories of the chronic prostatitis/chronic pelvic pain syndrome. We determine if culture, leukocyte and/or antibody status of prostate specific specimens predicts patient response to antibiotic therapy. MATERIALS AND METHODS: Patients clinically diagnosed with the chronic prostatitis/chronic pelvic pain syndrome according to the National Institutes of Health (NIH) definition had a lower urinary tract evaluation that included standard microscopy and culture of prostate specific specimens, and determination

Current concepts in antimicrobial therapy of prostatitis.

Acute prostatitis usually is caused by aerobic gram-negative organisms or, to a lesser extent, the enterococci. The treatment of acute prostatitis requires the use of an antimicrobial with the appropriate spectrum for ten to fourteen days. However, treatment of chronic prostatitis is a more difficult therapeutic problem because of the relative impermeability of the noninflamed prostate to the majority of antimicrobial agents. The organisms most commonly responsible for chronic prostatitis include the aerobic gram-negative organisms, as well as chlamydia. Chlamydia may be the sole pathogens, or

Inhibition of Proteus mirabilis growth through gallium-based disruption of iron and heme metabolism.

The increased incidence of multi-drug resistant (MDR) Proteus mirabilis highlights the need for novel antibiotics targeting P. mirabilis. In this study, we examined the potential antibacterial effects of gallium-based antimicrobials on P. mirabilis. We performed in vitro susceptibility testing of gallium compounds against P. mirabilis UTI isolates. Ga(NO3)3 and gallium protoporphyrin IX (GaPP) effectively inhibited P. mirabilis growth, with minimum inhibitory concentrations (MIC) of 64 μg/ml and 2.5 μg/ml, respectively. To further assess the effects of gallium compounds on P. mirabilis growth

Prostatic Abscess Secondary to Postoperative Wound Infection Caused by Mycobacterium mageritense.

Mycobacterium mageritense is a species of rapidly growing mycobacteria (RGM) found in environmental sources, possibly causing skin, soft tissue, and intravascular device infections. We encountered a case of prostatic abscess caused by M. mageritense that developed three months after transurethral resection of a bladder tumor. The patient was treated with antibiotics and underwent radical cystectomy. Prostatic abscesses due to RGM are rare, but may sometimes be overlooked. Mycobacterial cultures may be helpful in the diagnosis of culture-negative prostatic abscesses following these procedures.

Economic Burden of Community-Acquired Antibiotic-Resistant Urinary Tract Infections: Systematic Review and Meta-Analysis.

BACKGROUND: Antibiotic resistance (ABR) poses a major burden to global health and economic systems. ABR in community-acquired urinary tract infections (CA-UTIs) has become increasingly prevalent. Accurate estimates of ABR's clinical and economic burden are needed to support medical resource prioritization and cost-effectiveness evaluations of urinary tract infection (UTI) interventions. OBJECTIVE: This study aims to systematically synthesize the evidence on the economic costs associated with ABR in CA-UTIs, using published studies comparing the costs of antibiotic-susceptible and antibiotic-r

Challenges of infectious diseases in older adults: From immunosenescence and inflammaging through antibiotic resistance to management strategies.

Infectious diseases in older adults present a significant challenge to the healthcare system, marked by increased morbidity, mortality, and rising costs of care. Age-related changes (ARCs) in the immune system, including immunosenescence and inflammaging, contribute to heightened susceptibility to severe infections and reduced vaccine responsiveness. Additionally, alterations in the normal microbial flora due to aging and factors such as antibiotic therapy predispose older individuals to infections and age-related diseases. Changes in body composition also affect the pharmacokinetics and pharm

Skipped wells and scientific error during fosfomycin agar dilution and broth microdilution lead to inconsistent minimal inhibitory concentrations and may be cause for reevaluating testing methods for Klebsiella pneumoniae.

Despite the first-line recommendation of fosfomycin for uncomplicated urinary tract infections (UTIs), there are pressing barriers for optimizing its use for the treatment of non-Escherichia coli Enterobacterales UTI. There are no approved breakpoints for oral use against other Enterobacterales, and the recommended agar dilution (AD) reference method for minimal inhibitory concentration (MIC) determination is largely impractical. Using 160 clinical Klebsiella pneumoniae isolates, we sought to understand rates of skipped wells and MIC imprecision in broth microdilution (BMD) and how that compar

Relationship between biofilm formation of Pseudomonas aeruginosa and susceptibility to rifaximin and ofloxacin.

Pseudomonas aeruginosa is responsible for many infectious diseases related to antibiotic resistance. Biofilm formation may help bacteria to pass the treatment with antibiotics and overcome the immune system. Here, the relationship between antibiotics (rifaximin and ofloxacin) susceptibility and biofilm formation was evaluated. Ten isolates of P. aeruginosa were isolated from 110 urine specimens obtained from urinary tract infections (UTIs) patients. The spectrophotometric method was used to measure biofilm formation on polystyrene microtiter plates. Inhibitory zone onto agar plates was used to

Endogenous Endophthalmitis from Urinary Tract Infection Caused by Group B Streptococcus: A Case Report.

We present a case of endogenous endophthalmitis with urinary tract infection (UTI) caused by group B Streptococcus (GBS). An 86-year-old female initially presented with ocular pain and sudden visual disturbance of the left eye. The patient did not complain of other symptoms and had no history of recent ocular surgery or trauma. Endogenous endophthalmitis was clinically diagnosed based on ophthalmic examination, history, and lab results showing systemic infection. A few days later, GBS was identified in her aqueous humor, blood, and urine cultures. Intravitreal ceftazidime and vancomycin inject

Uropathogenic Escherichia coli endeavors: an insight into the characteristic features, resistance mechanism, and treatment choice.

Uropathogenic Escherichia coli (UPEC) are the strains diverted from the intestinal status and account mainly for uropathogenicity. This pathotype has gained specifications in structure and virulence to turn into a competent uropathogenic organism. Biofilm formation and antibiotic resistance play an important role in the organism's persistence in the urinary tract. Increased consumption of carbapenem prescribed for multidrug-resistant (MDR) and Extended-spectrum-beta lactamase (ESBL)-producing UPECs, has added to the expansion of resistance. Characterization of Escherichia coli isolated from urinary tract infection and association between virulence expression and antimicrobial susceptibility.

BACKGROUND: The capacity of antibiotics to modulate bacterial virulence has raised concerns over the appropriateness of antibiotic therapies, including when dosing strategies fall below sub-therapeutic levels. In this work, we investigated the ability of antibiotics to influence virulence in Escherichia coli isolated from urinary tract infection (UTI). RESULTS: Out of 120 isolates, 32.5% carried pap, 21.7% carried hlyA, and 17.5% carried cnf. The predominant B2 phylogroup was significantly associated with the quinolone-resistant isolates. A significant association was seen between the presenc
