Twenty-seven children and infants underwent surgery for bilateral infantile cataracts. In each infant, the lens in one eye was removed by discission and aspiration, and the other lens was removed by lensectomy and anterior vitrectomy. Ten of 27 eyes undergoing lensectomy and vitrectomy developed aphakic cystoid macular edema; in only one eye that underwent discission and aspiration did macular edema develop. Studies of high-speed motion pictures of the eyes of patients after extracapsular (EC) and intracapsular (IC) cataract extraction show that pseudophacodonesis and iridodonesis are the result of oscillations of the fluids in the anterior segment of the eye. These oscillations, initiated by movement of the eye, result in shearing forces on the corneal endothelium which may result in damage. Similar motion of the vitreous causes shearing forces which may damage the retina.

This report describes a series of 13 patients who underwent anterior vitrectomies for chronic aphakic cystoid macular edema (CME). All eyes had formed vitreous adherent to the intracapsular wound, vision of 20/50 or worse, and fluorescein angiographically documented CME for six months or more. Following the presentation of the results (success rate of 69%) and a discussion of this form of treatment, the beginning of a national, randomized, prospective study designed to evaluate the effectiveness of this treatment is described.

When an eye focuses on a distant target, it is often found that the long wavelength rays are focused on the receptors, but when the same eye looks at a target close to the eye, the green or blue rays are in focus in the retinal receptors. Of 78 patients eligible for prolonged wear contact lenses, 50 (65%) were successfully fitted and followed for four months to four years. Tesicon, one of the commercially available 'silicone rubber' lenses, was used in the correction of aphakic patients. In 74% of cases the lenses were considered successful for a daily wear regimen by the patient. Furthermore, a small number of patients could wear this lens without interruption for 3 to 6 days at a time. Despite this good acceptance by patients, corneal problems (mainly staining) and lens problems (dry surfaces) were frequently encountered.

A search of the literature yielded only one other paper concerning the use of ibuprofen for ophthalmic purposes. Marsili and Conte 8 used ibuprofen on a total of 138 patients with a variety of problems, both surgical and nonsurgical. They found an excellent to good response in about three fourths of their patients. This series of patients did as well or better. PURPOSE: To compare outcomes of unilateral cataract surgery in children aged 2-7 years with the outcomes reported in younger children.
METHODS: The medical records of patients who underwent unilateral cataract surgery between the ages of 2-7 years were reviewed retrospectively. Traumatic cataracts and ectopia lentis were excluded.
