Intermittent downbeat nystagmus secondary to vermian arachnoid cyst with associated obstructive hydrocephalus.

A 27-year-old man presented with a history of dizziness and intermittent vertical oscillopsia after oblique extension of the head and neck, following a neck injury 2 years previously. He had no other symptoms of cerebellar or brainstem dysfunction. On examination, an intermittent downbeat nystagmus lasting about fifty seconds was elicited by head extension and rotation. Radiologic examination showed slight lateral instability of the odontoid on lateral rotation, and computed tomography brain scan with Iopamidol 300 (Niopam) contrast revealed a 5-cm vermian arachnoid cyst in the posterior fossa

[Post-operative analgesia with sub-arachnoid fentanyl: ventilatory effects in elderly patients].

Twenty eight elderly patients scheduled for urologic surgery were randomly assigned to receive in a double blind fashion subarachnoid hyperbaric bupivacaine 15 mg with the addition of 50 micrograms (group A, n = 7), 25 micrograms (group B, n = 7), 12.5 micrograms (group C, n = 7), of fentanyl or 1 ml of saline (group D, n = 7) for a total volume of 4 ml. The pattern of breathing and the ventilatory response to CO2 were studied before 90, 150, 480 minutes after the subarachnoid injection. In group A mild pruritus and sedation occurred in 5 patients, nausea, vomiting and periodic breathing occur

Pineal corpora arenacea produced by arachnoid cells in the bat Myotis blythi oxygnathus.

There are corpora arenacea among the cell layers of the arachnoid on the dorsal surface of the pineal organ of the bat (Myotis blythi oxygnathus). The pineal arachnoid consists of electron lucent cells connected by cell injunctions to flat sheets and sandwiched on both sides by electron-dense cell rows. Among the superficial cell layers, collagen fibrils form loose bundles. In the electron-lucent cells, pinocytotic vesicles, rough surfaced endoplasmic reticulum, active Golgi areas and granular vesicles of various sizes can be found. Electron dense cells display fewer cytoplasmic organelles tha

[Facial paralysis secondary to arachnoid cysts. Description of a case in a girl with precocious puberty].

Arachnoid cysts are unusual in childhood. We describe a case of arachnoid cyst in a girl with precocious puberty. The cyst caused facial paralysis from compression. A link between arachnoid cyst and precocious puberty was excluded by cerebral TAC. We briefly discuss the aetiology and topography of arachnoid cysts.

Arachnoid cysts of the middle fossa predispose to subdural haematoma formation fact or fiction?

An association between arachnoid cysts of the middle fossa (ACMF) and complicating subdural haematoma (SDH) has been previously noted. More recently it has been hypothesized that ACMF may predispose to SDH formation. The Plymouth Neurosurgical Unit has treated twenty patients with ACMF between 1976-1985, seven of these being complicated by SDH. There was an age range of 11-56 years in those with SDH. Six of the seven patients with ACMF and SDH gave no significant trauma history. Four of these were males aged 11 to 20 years. The presenting histories, clinical findings and subsequent management

To shunt or to fenestrate: which is the best surgical treatment for arachnoid cysts in pediatric patients?

The treatment options for intracranial arachnoid cysts are either craniotomy and fenestration of the cyst into the cerebrospinal fluid spaces or shunting of the cyst contents extracranially. Fenestration may eliminate the need to shunt, but it is a major operative procedure and is not always successful. To determine which treatment provides the greatest benefit with the fewest complications, the records of 31 patients with 34 arachnoid cysts treated at the Children's Hospital of Los Angeles between 1976 and 1986 were reviewed. The mean age of the patients was 4.4 years, with a range of 0 to 15

A convexity intracerebral arachnoid cyst: a CT entity to be differentiated from a cystic glioma.

A 65-year-old female with an intracerebral cyst was examined in our clinic. It was suspected to be a cystic glioma by the preoperative neuroradiological studies, even with the use of sophisticated CT techniques. By operative and histopathological examination, it was confirmed as an arachnoid cyst mainly embedded in the parenchyma.

Diagnosis of perineurial arachnoid cysts using computed tomography: technical and clinical considerations.

Perineurial arachnoid cysts are often multiple lesions that are usually found in the lumbosacral area. These cysts may cause compression of surrounding structures. Examination of the first, second and third sacral nerves should be performed on patients with prolonged symptoms of a herniated disc or cauda equina syndrome. Three cases are presented for which computed tomography provided accurate, noninvasive diagnosis. Perineurial cysts are often associated with asymptomatic herniated disc; thus, surgery on the disc with an underlying symptomatic cyst may result in a failed back syndrome.

[Congenital arachnoid cysts of the posterior fossa midline. On their anatomoradiological variability (author's transl)].

An anatomical and radiological study of seven cases with arachnoid cyst of the midline of the posterior fossa in which there was either necropsic or surgical anatomical diagnosis is reported. A great variation was observed regarding their location, size and extension, which covers all the zone existing between the foramen of Magendie and the quadrigeminal lamina, presenting two cases which rose to the medial fossa, one of them entering in a left lateral ventricle and implanting itself in its' walls. Associated to arachnoid cysts other malformations have been found, such as type I Chiari, parti

Meningeal inflammation and arachnoid barrier breakdown in a mouse model of neonatal bacterial meningitis.

Newborns are especially susceptible to bacterial meningitis, primarily caused by group B Streptococcus (GBS), due to incomplete maturation of immune and barrier systems. While meningitis is well known to break down the blood-brain barrier, its effect on the meningeal arachnoid barrier remains poorly understood. Using a neonatal bacterial meningitis model, we demonstrate that GBS infection significantly increases arachnoid barrier permeability, disrupts tight junctions, and elevates meningeal production of proinflammatory cytokines. CD206+/LYVE-1+ border-associated macrophages undergo morpholog

Dynamic MRI of Fluid and Solute Transportation across the Arachnoid Barrier in the Human Meninges.

Background The dura mater has a permeable vasculature similar to that of peripheral organs, and is separated from the brain, which is enclosed within the arachnoid barrier. Fluid and solute transport across the arachnoid barrier between the subarachnoid space (SAS) and dura is crucial for brain waste clearance and immune surveillance, but few neuroimaging approaches can capture this dynamic process. Purpose To establish a clinically feasible intravenous gadolinium-based contrast agent (GBCA)-based MRI approach for assessing fluid and solute transport across the arachnoid barrier. Materials and

Spinal extradural arachnoid cyst in the dorsolumbar region: A case report and literature review.

BACKGROUND: Spinal extradural arachnoid cysts (SEACs) represent <1% of all spinal epidural lesions. They mostly occur in the thoracic region and occasionally are thoracolumbar in location. Typically, patients may be asymptomatic or exhibit pain with/without a neurological deficit. CASE DESCRIPTION: A 44-year-old female presented with a 5-year history of chronic low back pain and intermittent dysesthesia but had no neurological deficits. The magnetic resonance revealed an extradural cyst at D12-L1, isointense to cerebrospinal fluid (CSF) and displacing the dural sac ventrally. The patient unde

While meningitis is well known to break down the blood-brain barrier (BBB), how the meningeal arachnoid barrier, a critical component of the blood-cerebrospinal fluid barrier (B-CSFB), responds to infection is poorly understood. Using a neonatal mouse model of bacterial meningitis, we demonstrate that GBS infection significantly increases arachnoid barrier permeability, coinciding with alterations in Claudin-11 tight junction

Arachnoid Granulation Morphologies Associate with β-Amyloid and Tau Pathology in Older Adults.

INTRODUCTION: Arachnoid granulations (AG) enlarge with age, yet limited studies evaluate AG in neurodegeneration. Here, we investigate associations of AG morphology with Alzheimer's disease (AD) pathology. METHODS: Macroscopic AG properties were systematically evaluated along dorsal postmortem brain specimens from older adults (n=882). Regression models were used to analyze their associations with AD neuropathologic indices, controlling for demographic factors. RESULTS: Participants died at mean age of 90.62 (SD=6.98) years. β-amyloid (Odds ratio/OR, 0.79 [95% CI, 0.66-0.95]) and neurofibril

Parietal intradiploic arachnoid diverticulum: A rare delayed complication of endoscopic third ventriculostomy.

A postoperative intradiploic arachnoid diverticulum secondary to endoscopic third ventriculostomy is an extremely rare occurrence. It has a varied clinical presentation and is caused by the insinuation of the arachnoid through the dural rent into the intradiploic compartment following the procedure. This case report describes an incidentally diagnosed right parietal intradiploic arachnoid diverticulum as a complication of endoscopic third ventriculostomy. Skull radiography revealed an expansion of the right parietal bone with an intact outer table. Non-contrast computed tomography showed a wel

From Hospital Bed to the Playground: A Child's Story of Overcoming a Spinal Arachnoid Cyst.

Spinal arachnoid cyst is an uncommon medical condition. Recurrence of arachnoid cysts is also an unfamiliar phenomenon. The recurrence and the postoperative complications had lengthened the craving for the patient's ambulation. However, appropriate surgical decisions and adequate surgical management with modern equipment have made the management unparalleled. Here, we explain a boy's journey from a bedridden condition due to a dorsal arachnoid cyst to the playground. Not only the disease entity but also the management and journey of the patient made this case uncommon. Finally prompt response

Challenges with labour epidural placement in a parturient with a lumbar arachnoid cyst: a case report.

This case report describes the management of a patient with a lumbar arachnoid cyst requesting epidural labour analgesia. The patient was otherwise healthy but had suffered from multiple dural punctures during attempted epidural placement in a previous pregnancy. Magnetic resonance imaging following the dural punctures revealed the cyst, which was at the level of the L3-4 interspace, effacing the posterior epidural space at that level. Multidisciplinary management in this subsequent pregnancy, involving radiology and anaesthesia specialists, allowed ultrasound-guided identification of a higher
