Invasive electrophysiological evaluation of patients with sleep apnoea-associated ventricular asystole-methods and preliminary results.

Twelve patients (aged 48 +/- 12 y) with ventricular asystole of >3 s due to complete atrioventricular (AV) block (n = 8), sinoatrial (SA) block or sinus node arrest (n = 3) or both (n = 1) associated with obstructive sleep apnoea underwent invasive electrophysiological evaluation of sinus node function and AV conduction properties before and after administration of atropine (0.02 mg kg-1). Ventricular asystole lasted for 5.9 +/- 2.8 s (range 3.1-13 s). Sinus node function was assessed by measurement of sinus node recovery time, sinoatrial conduction time, and the response of sinus rate to atro

Primary cardiac tumors causing sudden death: a review of the literature.

Sudden unexpected death accounts for 200,000-400,000 deaths each year in the United States. Although the vast majority of these fatalities are related to atherosclerotic heart disease, a small percentage (approximately 0.0025%) stem from primary cardiac neoplasms. There have been 120 cases of sudden death attributed to primary cardiac tumors in the (published) literature. Although 103 of these lesions were histologically benign (86%), their intracardiac locations precipitated conductive and hemodynamic abnormalities that resulted in sudden death. These tumors are usually easily recognized at n

[Use of ajmaline in the diagnosis of sick sinus syndrome with a-v associated conduction defects].

The electrophysiological effects of ajmaline (1 mg/Kg i.v.) on sinus node were evaluated in 63 control subjects and in 12 pts with sick sinus syndrome (S.S.S.). In the control group the mean spontaneous cycle length (S.C.L.) was found significatively (less less than 0.001) reduced (8,?%), and corrected sinus node recovery time (C.S.N.R.T.) significatively (p less than 0.01) prolonged (30,2%) by the drug. In the patients with S.S.S. the S.C.L. was prolonged by 16% but not significatively and the C.S.N.R.T. by 60,7% (p less than 0.05). In 8/12 pts, with S.S.S. spontaneous sinoatrial blocks appea

Dysrhythmias in patients with a complete atrioventricular septal defect: From surgery to early adulthood.

OBJECTIVE: Outcomes after surgical repair of complete atrioventricular septal defect (cAVSD) have improved. With advancing age, the risk of development of dysrhythmias may increase. The aims of this study were to (1) examine development of sinus node dysfunction (SND), atrial and ventricular tachyarrhythmias, and (2) study progression of atrioventricular conduction abnormalities in young adult patients with repaired cAVSD. STUDY DESIGN: In this retrospective multicenter study, 74 patients (68% female) with a cAVSD repaired in childhood were included. Patients' medical files were evaluated for

Senile systemic amyloidosis: a clinico-pathological study of twelve patients with massive amyloid infiltration.

Hearts from 12 patients, 82-92 years old, with immunohistochemically verified pronounced senile systemic amyloidosis, were studied postmortem. Sections from the left ventricle, atrioventricular and sinus nodes and atrioventricular bundle were studied using the light microscope. The weights of the hearts varied between 310 and 870 grams. In the studied parts of the conduction system, the amounts of amyloid were considerably smaller than in the surrounding myocardium. Clinical data were taken from available case records. Eight of the patients had a history of uncomplicated congestive heart failu

Late reversible complete atrioventricular block and PR interval normalization after antegrade slow pathway ablation for atrioventricular nodal re-entrant tachycardia with pre-existing PR prolongation.

UNLABELLED: Late complete atrioventricular block (CAVB) is a rare periprocedural complication in the treatment of atrioventricular (AV) nodal re-entrant tachycardia. However, it can necessitate permanent pacemaker implantation. We present a case of late CAVB that developed during the periprocedural period. Its pathogenesis was attributed to the indirect or functional effects on the fast pathway of the AV node due to the presence of paroxysmal supraventricular tachycardia with PR prolongation. Additionally, PR prolongation regressed to within the normal range after curing the late CAVB, and the

A rat model of complete atrioventricular block recapitulates clinical indices of bradycardia and provides a platform to test disease-modifying therapies.

Complete atrioventricular block (CAVB) is a life-threatening arrhythmia. A small animal model of chronic CAVB that properly reflects clinical indices of bradycardia would accelerate the understanding of disease progression and pathophysiology, and the development of therapeutic strategies. We sought to develop a surgical model of CAVB in adult rats, which could recapitulate structural remodeling and arrhythmogenicity expected in chronic CAVB. Upon right thoracotomy, we delivered electrosurgical energy subepicardially via a thin needle into the atrioventricular node (AVN) region of adult rats t…

Case reports. 1. An autopsy case of fatal arrhythmia induced by injuries of the atrioventricular conduction system: a case report.

A 65-year-old woman died three days after being involved in a traffic accident, following an episode of ventricular fibrillation. She was di … lative necrosis with inflammatory reaction in the ordinary myocardium and adipose tissue around the atrioventricular node, which had spread to the proximal portion of the His' bundle. It is considered that these findings caused ventricular fibrillation to occur, and that the cause of death in this case was myocardial contusion due to blunt thoracic injury. This case wo …

Atrioventricular nodal reentrant tachycardia: a review.

BACKGROUND: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common form of regular narrow complex tachycardia. It is due to … be reproducibly initiated or terminated by cardiac pacing. The reentrant circuit is limited to the atrioventricular node and a small amount of perinodal atrial tissue. Acute termination of tachycardia can be achieved by vagal manoeuvres or drugs. Adenosine compounds are excellent drugs, as are calcium channel blockers, for acute termination of the arrhythmia. If chron …

Cardiac sarcoidosis--an occult cause of sudden death: a case report and literature review.

Sarcoidosis is a multi-systemic granulomatous disease of unknown cause. It commonly involves lymph nodes, lungs, eyes, and skin. Cardiac sarcoid may be isolated, or associated with systemic involvement. Cardiac involvement is found in 20-50% of autopsied patients with sarcoidosis. However, it only gives rise to clinical manifestations in about 5% of patients. Cardiac involvement by sarcoid has been reported to manifest as complete heart block, papillary muscle dysfunction, congestive heart failure, pericarditis and/or effusion, conduction abnormality or arrhythmia, chest pain, and sudden death

Paradoxical dynamic interaction of heart period and vagal activity on atrioventricular conduction in the dog.

The dynamic interaction of simultaneously changing heart period and single vagal stimuli on atrioventricular conduction (AV interval) was quantified by subtracting the vagally induced responses of the paced heart preparation from those of the unpaced heart preparation. This difference was significantly greater (P less than 0.05) than the AV responses to changes in heart period (delta A) alone without vagal stimulation (using the identical delta AA recorded from the unpaced heart in the same preparation, but with a crushed sinoatrial node). That is, for a given increase in AA interval, the AV c

Symptomatic sick sinus syndrome due to guanethidine. Case report.

A patient is described who developed symptomatic sinus bradycardia as low as 20 beats per minute and sinus arrest of up to 4.4 seconds while receiving guanethidine, 75 mg daily. The bradycardia resolved following discontinuation of the drug and reappeared upon challenge with it. Intrinsic disease of the sinoatrial and atrioventricular nodes was evidenced 3 weeks following discontinuation of the guanethidine by a borderline abnormally prolonged sinus node recovery time of 1500 msec and a PR interval of 0.28 seconds. Although sinus bradycardia is a known and not infrequent side effect of guaneth…

Dual challenge inside the womb: a case report of concomitant fetal atrio-ventricular block associated with maternal anti-SSA antibodies and fetal tachyarrhythmia diagnosed as Wolff-Parkinson-White syndrome after birth.

Fetal autoimmune atrioventricular block (AVB) is a rare but potentially life-threatening condition. It results from the passage of maternal anti-SSA/Ro or Anti SSB/La auto-antibodies into the fetal circulation, leading to inflammation and fibrosis of the AV node and often to irreversible damage. Besides AVB, these antibodies can also cause cardiomyopathies, but there is no evidence linking them to tachyarrhythmias. We present the case of a patient with significant risk factors for fetal AVB: a prior history of hydrops fetalis, high anti-SSA/Ro antibody levels and hypothyroidism. In this case,

Virus myocarditis in a 1-month-old boy presenting as two types of paroxysmal supraventricular tachycardia.

Herein we describe the case of a 1-month-old boy with acute viral myocarditis, who presented with two kinds of paroxysmal supraventricular tachycardia, and who was cured after medical treatment. He was brought to the emergency room with poor feeding due to fever. On the third day of hospitalization, a narrow QRS tachycardia (180-200 beats/min) was detected. Echocardiography showed a high echoic area at the atrial septum around the atrioventricular node. The patient was clinically diagnosed with acute myocarditis. The narrow QRS tachycardia was diagnosed as incessant junctional ectopic tachycar…

Atrioventricular nodal dysfunction secondary to hyperparathyroidism.

The relationship of hyperparathyroid-associated hypercalcemia with clinical significant bradyarrythmias still remains controversial. We present a 66-year-old patient with dizziness, headache and paroxysmal 2:1 atrioventricular block. A 24-hour Holter report revealed symptomatic intermittent 2(nd) degree (2:1) atrioventricular block with a mean heart rate of 46 bpm. A 2D echocardiogramm showed normal ejection fraction and there was no valve dysfunction or calcification. The biochemistry results showed elevated serum calcium level, low phosphate level, elevated serum parathyroid hormone level an
