Extrahepatic bile duct (EHBD) tumors often become symptomatic in an advanced stage when curative resection is seldom possible. An endocrine cell carcinoma of the extrahepatic bile duct in a 79-year-old man is described. The patient had complaints of jaundice and epigastric pain due to a small tumor located at the confluence of the common hepatic duct with the cystic duct. The indication for biliary drainage was malignant obstructive jaundice in 10 and benign obstructive jaundice in 2 cases. The effectiveness of drainage is indicated by the disappearance of jaundice. A 58-year-old man manifested obstructive jaundice secondary to adenocarcinoma of the common hepatic duct. Several nonsurgical methods of therapy are available for treatment of retained common bile duct calculi. These include percutaneous extraction, endoscopic extraction, dissolution, and endoscopic sphincterotomy. The method chosen depends on location and size of calculi, size of sinus tract, patient age, surgical risks, and other factors. In most cases, procedures can be carried out safely and successfully with few or no complications.

Common bile duct stones were demonstrated as high-density objects on CT in 22 of 45 cases. About 96% of the remaining cases (22 of 23) exhibited other abnormal CT findings which included common bile duct dilatation, concomitant gallbladder stones, and/or presence of intraductal air. CT is a useful diagnostic modality in the elevation of patients with suspected choledocholithiasis.
