Stereotactic radiosurgery (SRS) is a well-established technique that is replacing whole-brain irradiation in the treatment of intracranial lesions, which leads to better preservation of brain functions, and therefore a better quality of life for the patient. There are several available forms of linear accelerator (LINAC)-based SRS, and the goal of the present study is to identify which of these techniques is best (as evaluated by dosimetric outcomes statistically) when the target is located adjacent to brainstem. We collected the records of 17 patients with lesions close to the brainstem who h

INTRODUCTION: The nuclear syndrome of the oculomotor nerve, first described in 1981, is characterized by ipsilateral third nerve palsy and elevation paresis of the contralateral eye.
CASE REPORT: A 59-year-old man suddenly developed diplopia and headache. Neurological examination showed nuclear ophthalmoplegia of the left third nerve without any other neurological abnormality. MRI findings demonstrated a typical, popcorn-like, smoothly circumscribed left posterior lesion. Surgical resection was not performed because of the deep localization. Spontaneous bleeding developed.
CONCLUSION: This n

OBJECTIVE: We discuss the clinical aspects, pathology, and molecular genetics of 7 patients with primitive neuroectodermal tumors (PNETs) arising in the brainstem that were treated at our institution from 1986 through 1995. Most neuro-oncologists avoid performing biopsies in children with pontine tumors. This article raises the question as to whether biopsies should be performed, because treatment recommendations might differ if a PNET was diagnosed rather than a pontine glioma.
PATIENTS AND METHODS: We reviewed the clinical neuro-oncology database and the files of the Division of Neuropathol

A 5-year-old female spayed Spitz dog had a 5-week history of right head tilt, seizures, and progressive quadriplegia. Analysis of cerebrospinal fluid revealed 27,600 white blood cells per microliter with 63% mononuclear phagocytes, 27% lymphocytes, 6% neutrophils, 3% plasmacytoid cells, and 1% eosinophils, and over 2000 mg/dl protein. On contrast-enhanced magnetic resonance images, a focal 1-cm oval lesion was identified in the right ventral brainstem. There was also marked contrast enhancement of the meninges in the following areas: surrounding the brainstem, outlining cerebellar folia, along

We report two clinico-pathological cases with an initially hidden brainstem tumour presenting as chronic hydrocephalus of "idiopathic type". Diagnosis was established respectively one and two years after a successful shunting procedure, as repeated CT scan was performed because of gait deterioration. The first case was a bifocal glioblastoma invading the leptomeninges of the posterior fossa and spinal cord, and resulting in a communicating hydrocephalus. The second case was an ependymoma of the fourth ventricle leading to e non-communicating hydrocephalus. Rarity of such cases is emphasized.

Six children had recurrent sixth nerve palsy the average duration of which was six weeks. One youngster, after several episodes of sixth nerve palsy, had residual esotropia and underwent strabismus surgery. The average interval between recurrences was 1.3 years. Ipsilateral recurrences were a prominent feature. Thus far the recurrences of palsy in this group of patients has not been associated with any identifiable intracranial process. Multiple recurrences in the absence of any recognizable febrile illness clearly suggests that not all "benign" sixth nerve palsies in children are due to posti

A patient had the rare combination of central neurogenic hyperventilation (PaCO2 of 9 torr) and a normal level of consciousness for eight days. Morphine attenuated but never corrected the hyperventilation. Experimental effects of hypocapnia, which decreases both cerebral blood flow and metabolism in humans, are at odds with the normal mentation initially seen in this patient despite her marked and persistent hypocapnia. Death occurred after progressive brainstem dysfunction. Pathological study showed a well-differentiated astrocytoma involving primarily the medulla and pons, with scattered tum

BACKGROUND: Ependymoma is a slowly growing benign neoplasm that constitutes 3-9% of all neuroepithelial spinal cord tumors.[3,4] They rarely involve the cervicomedullary junction where they both compress the distal brainstem and upper cervical cord. Due to the critical contiguous structures, gross total resection of these lesions may result in significant morbidity/mortality.[1,2] Utilizing intraoperative neuromonitoring can help limit the risks of removing these lesions. Not when considering the risk/complications of partial versus total resection, the surgeon should keep in mind that they ar

Cyclic vomiting syndrome (CVS) remains a mysterious disorder despite our increasing knowledge since its classic description by Gee in 1882. Its hallmark feature of recurrent, explosive bouts of vomiting punctuating periods of normal health causes substantial medical morbidity (50% of patients require intravenous therapy), as well as significant time lost from school (20 school absences per year) and work. Limited epidemiologic data indicate that CVS may occur more commonly than previously thought, affecting as many as 1.9% of school-aged children. Besides the relentless vomiting, the child usu

Glioblastoma (GBM) represents the most aggressive primary malignant tumor of the adult central nervous system, with an extremely poor clinical prognosis. As the first-line chemotherapeutic agent for GBM, temozolomide (TMZ) has encountered a critical therapeutic bottleneck due to the development of drug resistance in tumor cells. GBM pathogenesis is jointly driven by multiple factors, including genetics, epigenetics, and the tumor microenvironment. O6-methylguanine-DNA methyltransferase (MGMT) -driven repair of DNA alkylation lesions is the central molecular mechanism underlying TMZ resistance

BACKGROUND: Glioblastomas (GBM) are brain tumors with the worst prognosis despite treatment with surgery and radio/chemotherapy, emphasizing the need for new therapies and improved treatment efficacy. Previously, we showed that clinically relevant ionizing radiation (IR) doses enhance GBM cell dedifferentiation into a stem-like phenotype, increasing stemness markers, self-renewal, and tumorigenic abilities. This work focuses on identifying early mechanisms driving this plasticity, particularly metabolic adaptations, as tumor metabolism may support therapy resistance and recurrence. METHODS: In

BACKGROUND: Central pain, characterized by neuropathic pain, can manifest due to injury to the superior spinothalamic tract. The brainstem includes sensory and motor pathways as well as nuclei of the cranial nerves, and therefore cancer metastasis in the region requires early intervention. Although stereotactic radiosurgery (SRS) is commonly employed for the treatment of brain metastasis, it poses risks of late complications like radiation necrosis (RN). RN exacerbates the progression of brain lesions within the irradiated area, and in the brainstem, it can damage multiple nerves, including th

BACKGROUND: Intracranial germ cell tumors (GCTs) comprise 3%-5% of pediatric primary central nervous system (CNS) tumors in Western countries. Though they are related in embryonic origin to gonadal GCTs, which are considered highly treatable with cisplatin-based chemotherapy regimens, intracranial GCTs vary in malignant potential and sensitivity to radiation and chemotherapy, generally carrying a worse prognosis. Metastases of intracranial GCTs outside of the CNS are rare, indicate a poor prognosis, and their salvage treatment is not well established.

OBJECTIVE: Patients affected with von Hippel-Lindau disease often develop multiple hemangioblastomas in the cerebellum and spinal cord. Timing of surgical intervention is difficult and depends largely on the anticipated surgical morbidity. However, data regarding surgical outcome after multiple cerebellar and medullary surgeries are scarce. Our objective was to evaluate cumulative surgical morbidity in patients operated on multiple cerebellar and medullary hemangioblastomas and to deduce recommendations for treatment.
METHODS: We performed a retrospective analysis for a consecutive cohort of

An adult bald eagle ( Haliaeetus leucocephalus) presented for nystagmus and an inability to fly. On physical examination, the eagle was open-mouth breathing and tachycardic at 200 beats per minute, had a wrinkled cere and sunken eyes, and was an estimated 10% dehydrated. Additionally, the eagle was extremely weak, with neurologic abnormalities including bilateral proprioceptive deficits, nystagmus, and no pupillary light reflex in the left eye. Despite aggressive treatment, the eagle continued to decline rapidly and subsequently died. On histologic examination, diffuse and widespread infiltrat

Here we report a boy with epidermal nevus syndrome associated with brainstem and cerebellar malformations and neonatal medulloblastoma. The patient had epidermal nevi and complicated brain malformations including macrocephaly with polymicrogyria, dysmorphic and enlarged midbrain tectum, enlarged cerebellar hemispheres with small and maloriented folia. The patient died after surgical resection of medulloblastoma which was newly recognized on MRI at 51 days of age. Postmortem pathological examinations showed very unique and bizarre malformation of the midbrain and hindbrain. The cerebellar corte

Glioblastoma multiforme (GBM) is the most lethal primary brain tumor in adults, with median survival rarely exceeding 15 months under current therapy with surgery, radiotherapy, and temozolomide (TMZ). Resistance mechanisms, tumor heterogeneity, and glioma stem cells (GSCs) remain major obstacles. Curcumin has shown pleiotropic anticancer effects, but its poor bioavailability and limited blood–brain barrier (BBB) penetration restrict clinical utility. Demethoxycurcumin (DMC), a natural curcuminoid analogue, provides greater chemical stability and potent multi-targeted activity. In vitro studie
