[Differential diagnosis of the saddle nose and perichondritis of the outer ear - the von Meyenburg-Altherr-Uehlinger syndrome (panchrondritis)].

Basing on the description of a case treated by the authors, the von Meyenburg-Altherr-Uehlinger syndrome is illustrated with its manifold symptoms and the resulting difficulties in diagnosis. Besides the signs of involvement of the eyes, joints, heart and vascular system, the symptoms caused by affection of the cartilage of the ear, nose, trachea and bronchi are important for the ENT surgeon. Aspects of differential diagnosis are discussed. The aetiology and pathogenesis of the syndrome are still unknown. Therapy is symptomatical with steroids and, in special cases, with immunosuppressive drug…

Agenesis of the trachea: report of a case and review of the literature.

Immediately after birth, a full-term infant was noted to be in severe respiratory distress and died approximately five hours later, despite resuscitative efforts. Postmortem examination revealed agenesis of the trachea with a 1-mm fistula joining the esophagus and the main stem bronchi at the carina. The right kidney, ureter, Fallopian tube, and cornu of the uterus were also absent. The 25 previously reported cases of agenesis of the trachea are summarized, with special emphasis on associated abnormalities.

Bronchial carcinoma. Lobar distribution of lesions in 250 cases.

In a study of the lobar distribution of tumors in 250 consecutive cases of primary bronchial cancer, it was noted that 130 of the tumors originated in the upper lobes, 11 in the right middle lobe, and 49 in the lower lobes. Some 40 arose in the main bronchi, and most of the remainder were either "hilar" or unspecified in anatomic location. There was no apparent correlation of the lobar site of these tumors with the lobar location of childhood pneumonic lesions as observed in another group of patients in the same hospital.

Imaging of the trachea.

Numerous benign and malignant tracheal diseases may affect the trachea primarily and secondarily. While the posterior anterior (PA) and lateral chest radiograph is the conventional study for initial evaluation of the trachea and central airways, findings may not always be apparent on conventional radiographs, and further evaluation with cross sectional imaging is usually necessary. Computed tomography (CT) is the imaging modality of choice for imaging the trachea and bronchi. Familiarity with the imaging appearances of the normal and diseased trachea will enhance diagnostic evaluation.

[Broncholytic effect of prednisolon in the rats inhaled with nitrogen dioxide].

In the rat model of chronic obstructive pulmonary disease, the dilatation effect of prednisolon was most obvious after 15 days of nitrogen dioxide inhalation. In a longer inhalation, the dilatation effect decreases, and on the 90th day of the inhalation prednisolon enhances contraction of the trachea and bronchi. Prednisolon seems to act upon the respiratory tract via interaction with endings of afferent C-fibres. In prolonged inhalation of nitrogen dioxide, resistance against corticosteroids develops as the result of the C-fibre ending inactivation.

[Expression of water channel protein 1 in the lung of the drown rat].

OBJECTIVE: To determine the expression of water channel 1 protein in the lung of drown rat and that of after death thrown into the water. METHODS: Immunohistochemical method was used and the computer image analysis was conducted to detect the distribution of AQP1. RESULTS: The positive expression of AQP1 was seen in the capillary endotheliocyte of the interstitial and around bronchi and in the alveolar endothelial cells. The value of intergrated optical density was statistical significant. CONCLUSION: It suggests that AQP1 is one of sensitive signs to distinguish ante-mortem and postmortem …

Primary endobronchial actinomycosis associated with broncholithiasis.

Primary endobronchial actinomycosis is an extremely rare disease that presents with endobronchial mass. We report 2 cases of primary endobronchial actinomycosis associated with broncholithiasis. There was no foreign body material, suggesting that these broncholiths were formed endogenously. Even though it is very rare, endobronchial actinomycosis should be included in the differential diagnosis of calcified endobronchial mass, especially when there is no clinical or radiological evidence of a granulomatous infection, such as tuberculosis, and the yellowish materials obstructing the bronchi are…

Xanthoma disseminatum with respiratory tract involvement and fatal outcome.

Xanthoma disseminatum (XD) is a rare mucocutaneous xanthomatosis classified as a benign form of non-Langerhans' cell histiocytosis. The case history is presented of a 61 year old woman with XD who developed dyspnoea and spirometric features of airflow obstruction. Bronchoscopy and computed tomography confirmed involvement of the large and medium sized bronchi and she subsequently died from acute respiratory failure. The post-mortem findings and the importance of respiratory tract disease in this unusual condition are discussed.

[The value of the CT scanner in intensive care].

Chest x-rays are often the first examination carried out at the bed side on patients in intensive care and their interpretation is often difficult. When CT scans are carried out they enable the mediastinum to be examined as well as the bronchi and pulmonary parenchyma. Using this technique to confirm a diagnosis or to explain those aspects on a standard x-ray which were difficult to analyse increasingly justifies the transportation of a patient the scanner. Spiral CT scans are particularly useful in the diagnosis of pulmonary emboli.

Acquired tracheobronchomegaly resulting in severe respiratory failure.

Tracheobronchomegaly is a distinct clinical and radiologic entity characterized by marked dilatation of the trachea and bronchi. In this report we describe a Marfanoid patient who developed TBM in his adulthood with rapid deterioration, leading to hypercapnic respiratory failure. This case represents an acquired form of TBM in a patient with Marfanoid features which has not been previously described. Tracheobronchomegaly should be considered as a cause of chronic respiratory failure in a young adult and can be diagnosed easily with chest roentgenogram and CAT scan.

Approaches to asthma management.

Allergic asthma is caused by antigen reaction with IgE fixed to mast cells of the bronchi. The reaction causes release of slow-reacting substance of anaphylaxis and histamine, which cause bronchial constriction. Prophylactic treatment includes avoidance of antigens, desensitization and use of cromolyn sodium. The latter appears to impede release of chemical mediators by the mast cell. Although of no value during an acute attack, this agent reduces attacks and reduces the need for other drugs in patients with severe perennial asthma.

Type 2 Congenital Pulmonary Airway Malformation in Tetralogy of Fallot with Absent Pulmonary Valve Syndrome.

Tetralogy of Fallot with absent pulmonary valve syndrome is a unique congenital condition often accompanied with dilated pulmonary arteries causing varied degree of airway compression. Frequently, abnormal pulmonary arterial branching at the segmental level and compression at the level of intrapulmonary bronchi can be seen. The occurrence of isolated bronchopulmonary malinosculation in the form of congenital pulmonary airway malformation is exceedingly rare. This report highlights a case of such rarity, involving a young child with recurrent lower respiratory tract infections.

A rare case of tracheobronchopathia osteochondroplastica.

Tracheobronchopathia osteochondroplastica (TO) is a rare benign disease of the lung that causes bony and/or cartilaginous nodules to grow in the trachea and main bronchi. It has an unknown cause and remains largely unknown due to its rarity. Here, we report a case of TO in a 63-year old woman at our hospital. Bronchoscopy revealed multiple cartilaginous and ossifying nodules that are characteristic of TO, and through histopathology of the biopsies, TO was confirmed. Given the rarity of TO, we aim to contribute to a more comprehensive understanding of TO's diagnosis and treatment.

Fatal Bronchopneumonia and Tracheitis in a Green Turtle (Chelonia mydas) Caused by Serratia proteamaculans.

A free-ranging subadult, male green turtle (Chelonia mydas) presented with radiographic evidence of pneumonia and died acutely. On necropsy, the trachea and bronchi were plugged by diphtheritic membranes, comprised of fibrin, necrotic debris, and colonies of bacilli, identified as Serratia proteamaculans. S. proteamaculans, typically considered an opportunistic plant pathogen, has rarely been described as causing disease in animals. This is the first report of S. proteamaculans causing severe necrotizing tracheitis and bronchopneumonia in a reptile.

Lung pulse visualized through pleural effusion as a diagnostic sign of complete obstructive atelectasis of the left lung in a critical patient with respiratory failure.

Complete obstructive atelectasis occurs when mucous or a foreign body obstruct one of the main bronchi. Several lung ultrasound signs have been associated with this entity. We describe the case of a patient admitted to the surgical critical care unit in whom lung ultrasound led to a diagnosis of complete obstructive atelectasis, and the presence of pleural effusion provided direct visualization of lung pulse, a sign that has only previously been described by interpreting ultrasound artifacts.

[PECULIARITES OF SIMULTANT INTERVENTIONS FOR SOME TYPES OF CHRONIC PHTHISIC PLEURITIS, COEXISTENT WITH PULMONARY TUBERCULOSIS].

In coincidence of chronic phthisic pleuritis in a rigid stage with pulmonary tuberculosis operative intervention is indicated of a pleuropulmonectomy type, what is a complex situation for performance and preservation of the patient's functional state. Pleuropulmonectomy in some patients is complicated by empyema and pathological processes in bronchi. Possibilities of operative interventions application, alternative to pleuropulmonectomy, were studied. Of 48 patients, to whom pleuropulmonectomy is indicated in accordance to data of clinic-roentgenological investigations, in 7--simultant operati…

Case report. Metastatic pulmonary calcification in renal failure: a new HRCT pattern.

A 56-year-old male with chronic renal failure presented with a 6 month history of progressive dyspnoea. High resolution CT of the chest showed multiple, peripheral, centrilobular nodules in the upper and mid zones, consistent with metastatic pulmonary calcification. Some of these pulmonary nodules showed ring calcification, a pattern that to our knowledge has not been described before. Calcification was also seen in the segmental pulmonary arteries, bronchi, trachea and subcutaneous vessels of the chest wall.

[Lung tissue changes in experimentally induced inflammation and its treatment].

As a result of the investigation of experimental models of pneumonia, three stages of its development and peculiarities of epitheliostromal interrelations in walls of bronchi and in lung respiratory region characteristic for every stage were established. Administration of benzonal as an inductor of the surfactant system along with polyunsaturated fatty acids exerted stimulating effect on the adaptive processes at acute and subacute stages in pneumonia. The number of macrophages increased, their function enhanced, which, along with type II alveolocytes proliferation indicated the activation of …

Thoracoscopic Segmentectomy for Right Upper Lobe With Unique Anatomic Variation.

Combined variation of bronchus and pulmonary vein is rare. We report the case of a 32-year-old man with lung adenocarcinoma in whom three-dimensional computed tomography bronchography and angiography displayed a unique anatomic variation in the right upper lobe. The posterior subsegmental bronchi originated from the apical segmental bronchus and the anterior segmental bronchus, respectively; the apical subsegmental vein shared a trunk with the posterior subsegmental vein. Based on this discovery, combined pulmonary segmentectomy was performed accurately.
