Electrophysiologic findings in long-term asymptomatic chagasic individuals.

Under electrophysiologic studies, 44 asymptomatic chagasic individuals were studied. The mean age was 39.9 years, with 26 being male. The surface ECG showed normal tracings in 12 patients, left anterior fascicular block in three, incomplete right bundle branch block in two, and right bundle branch block in 27 (isolated in six, associated with left anterior fascicular block in 19 and with left posterior fascicular block in two). Sinus nodal dysfunction was found in eight (18.1%), and 29 (66%) presented with some form of abnormal AV nodal behavior during atrial stimulation. Repetitive ventricula

Functional properties of mahaim fibers.

Electrophysiological studies were performed in a patient with a short P-R interval and a small delta wave. The findings suggest that ventricular pre-excitation resulted from an infranodal bypass (Mahaim type). As the functional properties of the pathway were evaluated, impaired conductivity (suggested by rather long refractory periods) became apparent at frequency stress. The pathway could easily be blocked by Ajmaline. This demonstrated an unexpected early diastolic improvement in conductivity; i.e. a supernormal phase of conduction. Due to this supernormal phase, Mahaim-fiber conduction was

Direct and reflex cardiac bradydysrhythmias from small vagal nerve stiumaltions.

Alterations in cardiac pacemaker location, its rate of discharge, and A-V conduction patterns were induced in anesthetized adult dogs by electrical stimulation of the thoracic vagi and their small cardiac branches before and after cervical vagotomy. Electrical activity from small, contiguous bipolar silver electrodes was amplified and recorded by an optical oscillograph. The electrodes were located over the SA node, the three internodal pathways, the left atrium, and ventricular epicardium. A hoffman-type plaque electrode was placed over the A-V node to record a His bundle electrogram simultan

[Cryosurgical ablation of the atrioventricular conduction system--experimental study for the determination of the appropriate ablation site and the long term follow-up results with clinical application].

In a series of experimental studies in dogs, we determined the exact site for the disruption of the atrioventricular conduction system by cryosurgical ablation which does not disturb the intraventricular conduction. The long-term symptomatic effects were evaluated in 9 patients with supraventricular tachycardia. In a total of 15 adult mongrel dogs, a thoracotomy was performed under pentobarbital anesthesia that was maintained during the procedure with artificial ventilation and cardiopulmonary bypass. The body temperature was maintained at 37 degrees C and throughout each experiment, the ECG w

Atrioventricular block in the atypical form of junctional reciprocating tachycardia: evidence supporting the atrioventricular node as the site of reentry.

Serial electrophysiologic studies were performed in 19 patients with the atypical form of supraventricular tachycardia having a long RP and short PR interval. In all 19 patients, supraventricular tachycardia was found to have a 1:1 P-QRS relation during initial control electrophysiologic studies, and in all 19 patients, electrophysiologic studies suggested that junctional reentry was the mechanism of supraventricular tachycardia. Seven of the 19 patients developed atrioventricular (AV) block during initiation of supraventricular tachycardia or after induction of supraventricular tachycardia fo

Discussion of LBBP synchronization effects in HF patients with LBBB and comparison with BiV-CRT.

Left bundle branch block (LBBB) is common in heart failure patients, and could induce dyssynchrony of ventricular contraction, deterioration of cardiac function, and increased mortality. Cardiac resynchronization therapy (CRT) with biventricular pacing reduces ventricular dyssynchrony, heart failure hospitalization, and all-cause mortality in heart failure patients with LBBB. However, there are approximately 30% nonresponders and 10% of patients remain untreated owing to an unsuitable coronary sinus vein. His bundle pacing (HBP) is a more physiological pacing modality which has showed inspirin

[Studies on the effect of Melixetin (Kö 1173) on AV-conduction time and sinus pulse automation in persons with healthy hearts and in patients with disorders of the impulse-conduction system].

The influence of Mexiletine on the av-conduction time and sinus node recovery time of 24 patients with and without disturbance of impulse formation and conduction was examined. There was no change of conduction time proximal to the His-bundle. Four patients developed block distal to the His-bundle, 3 of which had a conduction disturbance before application of the drug consisting in 2:1 av-block or bifascicular block. Two patients with WPW-syndrome showed no change of impulse conduction. Five out of 12 patients showed an increase of the effective recovery period. Three out of 15 patients showed

Effect of lead body and helix design variables on implantation success, insertion depth, and muscle torque in the left bundle branch area: Insights from an ex vivo porcine model.

BACKGROUND: Lumenless and stylet-driven leads used for left bundle branch area pacing differ in design and have a significant implantation learning curve. Although previous studies examined longer helices for deep-septal pacing, the influence of other design variables remains unclear. OBJECTIVE: This study aimed to evaluate how helix design and axial force affect interventricular septum insertion efficacy. METHODS: Rigid leads were developed using helical coils with variable outer diameter, number of turns, and pitch. Porcine septa (n = 16) were clamped perpendicularly for insertion using an

Antidromic AVRT with changing His activation. What is the mechanism?

We present a case of antidromic AVRT involving an atriofascicular pathway in a patient with an atretic coronary sinus os. This patient had left ventricular dysfunction with rate-related left bundle branch block aberrancy in sinus rhythm. In the electrophysiology lab, there was VA dissociation for a few beats during antidromic tachycardia. The possible mechanisms that could explain this intracardiac finding are discussed.

Effect of Direct Slow Pathway Capture Mapping-Guided Ablation on Typical Atrioventricular Nodal Re-Entrant Tachycardia.

BACKGROUND: Direct slow pathway capture (DSPC) mapping is a novel electrophysiological technique for detecting antegrade slow pathway input sites. However, the effect of DSPC mapping-guided ablation on atrioventricular nodal re-entrant tachycardia (AVNRT) is unknown. OBJECTIVES: This study aimed to evaluate the efficacy and safety of DSPC mapping-guided ablation in typical AVNRT patients. METHODS: A multicenter retrospective study was conducted in 301 consecutive typical AVNRT patients. The outcomes in patients who underwent DSPC mapping-guided ablation (DSPC group) and those who underwent c

Interesting phenomena during threshold testing of conduction system pacing: What is the mechanism?

BACKGROUND: Although the interest in conduction system pacing is increasing, the data on longitudinal dissociation in the literature is still limited. METHODS AND RESULTS: We performed left bundle branch (LBB) pacing on a patient with sick sinus syndrome and atrioventricular block. The transition of QRS morphology can be observed during the threshold testing. The main findings were different left ventricular activation times when selective LBB capture was performed at different outputs. CONCLUSIONS: This phenomenon may be due to the anatomical basis of longitudinal dissociation of the His-Pur

Safety and feasibility of conduction system pacing in patients with congenital heart disease.

INTRODUCTION: Conduction system pacing (CSP) has emerged as an ideal physiologic pacing strategy for patients with permanent pacing indications. We sought to evaluate the safety and feasibility of CSP in a consecutive series of unselected patients with congenital heart disease (CHD). METHODS: Consecutive patients with CHD in which CSP was attempted were included. Safety and feasibility, implant tools and electrical parameters at implant and at follow-up were evaluated. RESULTS: A total of 20 patients were included (10 with a previous device). A total of 10 patients had complex forms of CHD,

Comparison of cardiac function between left bundle branch pacing and right ventricular outflow tract septal pacing in the short-term: A registered controlled clinical trial.

BACKGROUND: The novel method of left bundle branch pacing (LBBP) has been reported to produce a narrower QRS duration and lower pacing threshold than right ventricular outflow tract septal pacing (RVOP). However, whether LBBP is superior to traditional RVOP in improving cardiac function still lacks sufficient evidence. OBJECTIVE: The purpose of this study was to compare the changes in cardiac function (especially in brain natriuretic peptide (BNP)levels, left atrial function, and left ventricular diastolic function) within 7 days between LBBP and RVOP. METHODS AND RESULTS: A single-centre pr

Alternating trifascicular block and cardiac memory.

We describe a case of a 70year-old woman with effort dyspnea and alternating trifascicular conduction pattern, worsened with exercise to atrioventricular block (AV Block). The alternating conduction at rest resulted in marked T wave changes. We discuss the mechanism of T wave memory changes and the management of exercise-induced AV Block.
