Flow analysis of ventricular assist device inflow and outflow cannula positioning using a naturally shaped ventricle and aortic branch.

Tip geometry and placement of rotary blood pump inflow and outflow cannulae influence the dynamics of flow within the ventricle and aortic branch. Cannulation, therefore, directly influences the potential for thrombus formation and end-organ perfusion during ventricular assist device (VAD) support or cardiopulmonary bypass (CPB). The purpose of this study was to investigate the effect of various inflow/outflow cannula tip geometries and positions on ventricular and greater vessel flow patterns to evaluate ventricular washout and impact on cerebral perfusion. Transparent models of a dilated car…

Percutaneous introduction of left atrial cannula for left heart bypass: utility of biplane transesophageal echocardiographic guidance for transseptal puncture.

For introduction of a left atrial (LA) cannula by the transseptal puncture technique, we examined the feasibility of using biplane transesophageal echocardiography (B-TEE). A transseptal puncture was performed on 15 patients (3 male; 12 female; mean age, 48.9 +/- 11.2 years) by B-TEE guide during percutaneous transvenous mitral comissurotomy (PTMC). The entire Brocken-brough needle and the position of its tip were clearly observed in the right atrium by a longitudinal image of B-TEE in all patients (100%), and in 2 (13%) of them also by the transverse image of B-TEE. The puncture was about 1 c…

Utilizing Point-of-Care Ultrasound and Tourniquet Placement for Expedited Diagnosis and Management of Intravenous Cannula Breakage: A Case Report.

The breakage of an intravenous (IV) cannula represents a rare complication associated with peripheral IV cannulation, a prevalent procedure in the civilian and military prehospital setting. Intravenous cannula breakage can lead to further complications including infection, migration, and subsequent embolization. Although fast identification and extraction are essential management components, there are no consensual guidelines for the proper management of this complication. In this report, we present a case involving a 19-year-old soldier who presented to the emergency department following the …

High-Flow Nasal Cannula in Critically Ill Subjects With or at Risk for Respiratory Failure: A Systematic Review and Meta-Analysis.

High-flow nasal cannula (HFNC) oxygen delivery has been gaining attention as an alternative means of respiratory support for critically ill patients, with recent studies suggesting equivalent outcomes when compared with other forms of oxygen therapy delivery. The main objective of this review was to extract current data about the efficacy of HFNC in critically ill subjects with or at risk for respiratory failure. We performed a systematic review of publications (from database inception to October 2015) that evaluated HFNC in critically ill subjects with or at risk for acute respiratory failure…

Evaluation of different types of inferior vena cava cannulae placement by transesophageal echocardiography and its impact on hepatic dysfunction.

BACKGROUND: Postoperative hepatic dysfunction may occur in an otherwise uncomplicated open heart surgery. One of the reasons is malpositioning of the inferior vena cava (IVC) cannula in the hepatic vein (HV) or beyond. A straight cannula is considered more likely to be malpositioned compared to the angled cannula and a malpositioned cannula can lead to hepatic dysfunction. METHODS: In this prospective study, forty adult patients undergoing atrial septal defect repair were randomized into two groups as: straight cannula group (n=20) and angled cannula group (n=20). The cannula position was ass…

High-flow nasal cannula use in children with respiratory distress in the emergency department: predicting the need for subsequent intubation.

BACKGROUND: High-flow nasal cannula (HFNC) is a safe, well-tolerated, and noninvasive method of respiratory support that has seen increasing use in the care of children with respiratory distress. High-flow nasal cannula may be able to prevent intubations in infants and children with respiratory distress. OBJECTIVE: The objective of this study was to determine the clinical and patient characteristics that predict success or failure of HFNC therapy in children presenting to the pediatric emergency department (PED) with respiratory distress. DESIGN/METHODS: A retrospective cohort review was con…

Push-pull cannula for localized application of drugs and sampling of medium, combined with electrophysiological recordings in an interface slice chamber.

These experiments combined electrophysiological recordings from hippocampal slices with application of drugs to and sampling of extracellular fluid from a restricted region of the slice using a push-pull cannula placed under the slice in an interface chamber. Stable and apparently normal extracellular and intracellular recordings could be obtained directly over the tip of the cannula and solutions changed without disturbing the recording. Relatively rapid effects (1-5 min) were observed when TTX, CNQX, or medium containing 50 mM K+ were applied via the cannula and recovery from these effects w…

Disturbances of blood-flow velocity in the dorsal veins of the hand after vein cannulation and cannula fixation in the anaesthetised patient.

Modifications of the mean blood-flow velocity in the dorsal veins of the hand were assessed semi-quantitatively with continuous wave (CW) Doppler equipment in 32 anaesthetised patients (17 men and 15 women), 23-78 (median = 56) years of age, before and after venous catheterisation with cannula fixation to the skin. Cannulation of the vein caused a 48% reduction in the mean blood-flow velocity and made it impossible to detect any flow with the equipment used in 22% of the patients. A 10% further reduction in the mean blood-flow velocity and in the number of subjects with undetectable blood flow…

High-flow nasal cannula versus noninvasive ventilation in acute heart failure-related respiratory failure: a systematic review and meta-analysis.

BACKGROUND: Noninvasive ventilation (NIV) is the recommended first-line respiratory support for acute heart failure (AHF)-related respiratory failure. However, its tolerance is often limited. High-flow nasal cannula (HFNC) has emerged as a potential alternative, but comparative evidence remains inconsistent. This study aimed to evaluate the efficacy and safety of HFNC versus NIV in patients with AHF-related respiratory failure. METHODS: We conducted a systematic review and meta-analysis following PRISMA guidelines. We searched four major electronic databases from inception to April 2026. We i…
