Out-of-pocket payment for surgery in Uganda: The rate of impoverishing and catastrophic expenditure at a government hospital.

BACKGROUND AND OBJECTIVES: It is Ugandan governmental policy that all surgical care delivered at government hospitals in Uganda is to be provided to patients free of charge. In practice, however, frequent stock-outs and broken equipment require patients to pay for large portions of their care out of their own pocket. The purpose of this study was to determine the financial impact on patients who undergo surgery at a government hospital in Uganda. METHODS: Every surgical patient discharged from a surgical ward at a large regional referral hospital in rural southwestern Uganda over a 3-week per

[Complement System Abnormalities in Patients with Atypical Hemolytic Uremic Syndrome and Catastrophic Antiphospholipid Syndrome].

BACKGROUND: The role of the alternative complement pathway (AP) abnormalities in the pathogenesis of aHUS is well studied. Clinical and morphological manifestations of atypical HUS and catastrophic APS are often similar. However, studies on the state of AP in patients with CAPS are virtually absent. AIMS: The aim of our study was to assess the state of AP in patients with CAPS and aHUS. Patients and methods: The study enrolled 67 patients (pts) with a diagnosis of CAPS (28 pts) and aHUS (39 pts). Studies of the complement system are made of 10 pts with CAPS and 20 aHUS. Factor H, I, B, D cont

Resistance Exercise Training on Disease Impact, Pain Catastrophizing and Autonomic Modulation in Women with Fibromyalgia.

Women with fibromyalgia (FM) often complain of whole-body pain, and muscle fatigue, which may be related to autonomic dysfunction. Therefore, the purpose of the present study was to investigate the effects of resistance exercise training (RET) on disease impact, pain catastrophizing, and autonomic modulation in women with FM. Women with FM (n=26) and healthy control women (HC: n=9), aged 19-65 yrs, were compared at rest. Women with FM were randomly assigned to a resistance-training group (FM-RT: n=14) or a non-exercising control group (FM-CON: n=12). Women in the FM-RT group underwent 8-weeks

Preoperative Predictors of Pain Catastrophizing, Anxiety, and Depression in Patients Undergoing Total Joint Arthroplasty.

BACKGROUND: The relationship between pain catastrophizing and emotional disorders including anxiety and depression in osteoarthritic patients undergoing total joint arthroplasty (TJA) is an emerging area of study. The purpose of this study was to examine the association of these factors with preoperative patient characteristics. METHODS: A prospective cohort study of preoperative TJA patients using the Pain Catastrophizing Scale (PCS) and Hospital Anxiety and Depression Scale (HADS-A/HADS-D) was conducted. Preoperative measures included visual analog pain scale (VAS), Harris Hip and Knee Soci

Relationships Between Refraining From Catastrophic Thinking, Repetitive Negative Thinking, and Psychological Distress.

Skills to refrain from catastrophic thinking were negatively related to worry and a wide range of psychological distress. Repetitive negative thinking (including worry) is proposed as a common etiological factor for a wide range of psychological distress. Therefore, reduced repetitive negative thinking would mediate the negative relation between refraining from catastrophic thinking and psychological distress (depression, social anxiety, phobia, generalized anxiety, and obsessions and compulsions). As an overlap between five indices of psychological distress was expected, we first computed lat

Vesicovaginal fistula following insertion of a foreign body in the vagina for sexual gratification: could it be catastrophic?

In developing countries such as India, the vesicovaginal fistula (VVF) is a commonly found entity in urogynaecology. VVFs secondary to obstructive labour still remain the most common cause in these countries. We report a very unusual case of a VVF in a young woman that developed due to self-insertion of a plastic box for sexual gratification.

A case of catastrophic antiphospholipid syndrome: first report with advanced cardiac imaging using MRI.

This present case pertains to a 48-year-old woman with a history of antiphospholipid syndrome, who presented with progressive fatigue, generalized weakness, and orthopnea acutely. She had a prior diagnosis of antiphospholipid syndrome with recurrent deep vein thromboses (DVTs) and repeated demonstration of lupus anticoagulants. She presented in cardiogenic shock with markedly elevated troponin and global myocardial dysfunction on echocardiography, and cardiac catheterization revealed minimal disease. Cardiac magnetic resonance imaging was performed, which revealed findings of perfusion defects

Risk factors, complication and measures to prevent or reverse catastrophic sodium overcorrection in chronic hyponatremia.

Hyponatremia is the most common electrolyte disorder encountered in clinical practice. Patients who develop this condition for more than 48 hours are at risk for severe neurological sequelae if correction of the serum sodium occurs too rapidly. Certain medical disorders are known to place patients at an increased risk for rapid correction of serum sodium concentration. Large-volume polyuria in this setting is an ominous sign. For these patients, early identification of risk factors, close monitoring of serum sodium correction and the use of 5% dextrose with or without desmopressin to prevent o

Use of prescription analgesic medication and pain catastrophizing after total joint replacement surgery.

OBJECTIVE: To survey the use of analgesic medication 4.8 years after total joint replacement (TJR) surgery and assess the determinants of medication usage. PATIENTS AND METHODS: Of 852 patients who had undergone TJR for osteoarthritis were recruited from secondary care. Participants (mean age, 73.7 years) responded to a questionnaire on medication use, physical function and pain (WOMAC, VAS and body pain), pain catastrophizing and illness behaviour (somatization). RESULTS: Only 37% of study participants were not on any pain relief medication, 25.1% were taking opioids, 6.9% were taking presc

A Rasch analysis of the pain catastrophizing scale supports its use as an interval-level measure.

OBJECTIVES: To evaluate the properties of the Pain Catastrophizing Scale (PCS) from a Rasch paradigm. METHODS: A secondary analysis of 235 patients with work-related pain conditions was performed using the Rasch methodology. Unidimensionality, item fit, location independence, differential item functioning, response option structure, and linearity were evaluated for the 13-item PCS score. RESULTS: Two items (8 and 12) required rescoring to address disordered response thresholds. Significant misfit to the Rasch model was corrected through the use of testlets based on the original 3 factors of

A potentially catastrophic anatomical variation: aberrant internal carotid artery in the middle ear cavity.

Aberrant internal carotid artery (ICA) is a rare but a very important vascular anomaly of temporal bone. Misdiagnosis of the anomaly may lead to massive hemorrhage and severe complications during otologic procedures. It is essential to keep this anomaly in mind for any otologic surgeon to prevent catastrophic complications. We present a case of aberrant ICA appeared as a nonpulsatile middle ear mass. The patient had a complaint of hearing loss, and the otoscopic examination of the patient revealed a tympanic membrane perforation and a blue-reddish retrotympanic mass. Multidetector computed tom

NEK9 depletion induces catastrophic mitosis by impairment of mitotic checkpoint control and spindle dynamics.

NEK9 is known to play a role in spindle assembly and in the control of centrosome separation, but the consequences of NEK9 targeting in cancer cells remain to be elucidated. In this study, we used siRNA to investigate the consequences of targeting NEK9 in glioblastoma and kidney cancer cells as a first step in assessing its potential as an anti-cancer therapeutic target. Live cell imaging revealed that NEK9 depletion of U1242 glioblastoma and Caki2 kidney carcinoma cells resulted in failure of cytokinesis. Interestingly, NEK9-depleted Caki2 cells overrode mitosis under incorrect chromosome ali

Catastrophic paradoxical embolus after hemodialysis access thrombectomy in a patient with a patent foramen ovale.

We present the first case of documented clot in-transit through a patent foramen ovale (PFO), caused by mechanical thrombectomy of a clotted AV fistula. Embolus after access thrombectomy is a rare, but known complication of the procedure. Paradoxical embolus is likewise a rare consequence of a right-sided clot, embolising to the left-sided circulation in a patient with a PFO. Suspected embolus through a PFO after dialysis access thrombectomy has been reported, but no case has ever documented the clot in-transit through the PFO.

Catastrophic haemoptysis in the Fontan circulation--a novel surgical approach to denude the culprit bleeding vessels.

We describe the case of a young man aged 19 years with a double inlet left ventricle associated with transposition of the great arteries, and a mechanical aortic valve requiring anticoagulation, who presented with massive haemoptysis. At cardiac catheterisation, there were multiple feeder vessels to a bleeding leash surrounding and communicating with his left main bronchus. Despite occlusion of the larger feeder vessels, he continued to have massive haemoptysis. We describe a novel surgical strategy of denuding the peribronchial vessels through a left lateral thoracotomy. This successfully sto

Catastrophic polyethylene failure diagnosed with magnetic resonance imaging in a painful total knee arthroplasty.

Determining the etiology of a painful knee after arthroplasty can be extremely challenging. Traditionally, orthopedists relied mainly on physical examination, laboratory results, serial radiographs, and 3-phase bone or indium-labeled white blood cell scans; however, recent advances in magnetic resonance imaging (MRI) software have given orthopedists another powerful tool in their diagnostic armamentarium. We provide the MRI software modification technique for metallic artifact reduction as well as present a novel case in which MRI was used to diagnose catastrophic polyethylene postfailure in a

Mitotic catastrophe as a prestage to necrosis in mouse liver cells treated with Helicobacter pullorum sonicates.

Helicobacter pullorum infections have been associated with several enterohepatic diseases, but the mechanism of action is currently undefined. The present study was therefore set up to investigate possible cytotoxic effects of this pathogen on liver cells. A mouse hepatic cell line was exposed to H. pullorum sonicate and cytotoxicity was observed for all isolates after incubation for 72 h. Features characteristic for mitotic catastrophe characterized by chromatin condensation, formation of multinuclear distended cells and micronucleation, were recorded. In addition, intranuclear pseudoinclusio
