Discontinuation of Pneumocystis jirovecii pneumonia prophylaxis with CD4 count <200 cells/µL and virologic suppression: a systematic review.

BACKGROUND: HIV viral load (VL) is currently not part of the criteria for Pneumocystis jirovecii pneumonia (PCP) prophylaxis discontinuation, but suppression of plasma viremia with antiretroviral therapy may allow for discontinuation of PCP prophylaxis even with CD4 count <200 cells/µL. METHODS: A systematic review was performed to determine the incidence of PCP in HIV-infected individuals with CD4 count <200 cells/µL and fully suppressed VL on antiretroviral therapy but not receiving PCP prophylaxis. RESULTS: Four articles examined individuals who discontinued PCP prophylaxis with CD4 count

The impact of HIV infection and CD4 cell count on the performance of an interferon gamma release assay in patients with pulmonary tuberculosis.

BACKGROUND: The performance of the tuberculosis specific Interferon Gamma Release Assays (IGRAs) has not been sufficiently documented in tuberculosis- and HIV-endemic settings. This study evaluated the sensitivity of the QuantiFERON TB-Gold In-Tube (QFT-IT) in patients with culture confirmed pulmonary tuberculosis (PTB) in a TB- and HIV-endemic population and the effect of HIV-infection and CD4 cell count on test performance. METHODOLOGY/PRINCIPAL FINDINGS: 161 patients with sputum culture confirmed PTB were subjected to HIV- and QFT-IT testing and measurement of CD4 cell count. The QFT-IT wa…

A trial of the effect of micronutrient supplementation on treatment outcome, T cell counts, morbidity, and mortality in adults with pulmonary tuberculosis.

BACKGROUND: Tuberculosis (TB) often coincides with nutritional deficiencies. The effects of micronutrient supplementation on TB treatment ou … hs in HIV-negative subjects (95% CI, -14% to 88%; P = .08). Supplementation increased CD3+ and CD4+ cell counts and decreased the incidence of extrapulmonary TB and genital ulcers in HIV-negative patients. Micronutrients reduced the incidence of peripheral neuropathy by 57% (95% CI, 41% to 69%; P < .001), irrespective of HIV status. There were no significant …

Association of heart rate with coronary risk factors and increased white blood cell counts in healthy Japanese people.

Elevated resting heart rate (HR) is associated with hypertension in addition to or similar to increased incidence of cardiovascular morbidity and mortality. Subjects with high HR exhibit the characteristic features of insulin resistance syndrome. Sympathetic overactivity underlies this clinical condition. However, the true mechanism of high HR is unclear. We examined the relation between resting HR and age, gender, traditional coronary risk factors, and white blood cell (WBC) count in urban Japanese. The study population comprised 2,736 people aged 40, 50, and 60 years old. Multiple regression

[Nuclear DNA content and chromosomal counts of cultured cells derived from human cervix with normal and abnormal epithelia].

In order to examine the cellular properties of each cultured cell derived from normal and abnormal epitheliums of the cervix, we measured the nuclear DNA content and chromosomal counts of these cultured cells. The following results were obtained. The nuclear DNA content of cultured cells derived from normal, mild, moderate, severe dysplasia, carcinoma in situ (cis) and invasive carcinoma was distributed among low 2c-high 4c, low 2c-6c, low 2c-7c, low 2c-10c, low 2c-11c and low 2c-13c respectively. A number of the cells exceeding 4c apparently increased from lesions of moderate dysplasia. In th

Tumor Budding in Gastric Carcinoma: Beyond Counting Cells at the Invasive Front-A Review of Current Evidence and Biological Perspectives.

Tumor budding is increasingly recognized as a histopathologic feature associated with invasive behavior in gastrointestinal malignancies. While its prognostic value is well established in colorectal carcinoma, its significance in gastric adenocarcinoma remains less clearly defined because of marked morphologic heterogeneity, variable growth patterns, and the absence of gastric-specific assessment criteria. Multiple studies have associated high budding density with adverse clinicopathologic features, including lymph node metastasis, lymphovascular invasion, advanced tumor stage, and poorer surv

Immune profiling of the buffalo mammary gland across graded low somatic cell counts under tropical conditions.

Milk somatic cell count (SCC) indicates udder health and milk quality, but levels much lower than the normal range may signal mammary gland immunocompromise. This study evaluated the immunological status of the buffalo mammary gland across graded low SCC levels under non-infectious conditions (<2 ×10⁵ cells/mL). Forty-eight clinically healthy Murrah buffaloes in early lactation were categorized into four SCC groups: Group 1 (0.20-0.60 ×10⁵ cells/mL), Group 2 (0.61-0.90 ×10⁵ cells/mL), Group 3 (0.91-1.40 ×10⁵ cells/mL), and Group 4 (1.41-1.90 ×10⁵ cells/mL). Buffaloes were screened using the Ca…

Increasing daily step counts improves physical fitness, and reduces pain and arterial stiffness in sickle cell patients.

Patients with sickle cell anemia (SCA) have long been discouraged from physical activity (PA). The aim of the present study was to assess the impact of increasing daily step counts on physical fitness, pain and vascular function in patients with SCA. Thirty-eight patients with SCA were recruited and equipped with a Fitbit wrist-worn accelerometer-based PA tracker for five weeks to objectively quantify their baseline daily step counts. Patients were then randomly assigned to one of three groups: 1) control group - no specific information regarding PA was given for eight weeks (N=12); 2) PA1 gro

Increased Mortality with Intermediate Ascitic Polymorphonuclear Cell Counts Amongst Patients with Cirrhosis: Time to Redefine the Care Approach.

BACKGROUND: Spontaneous bacterial peritonitis (SBP) is a serious complication in patients with decompensated cirrhosis and ascites. Diagnosis typically relies on an ascitic polymorphonuclear (A-PMN) cell count ≥ 250 cells/high-power field (HPF). METHODS: In this retrospective cohort study, 117 hospitalized patients with acute decompensation of chronic liver disease and a diagnostic paracentesis were evaluated. Clinical, laboratory, and imaging data were collected. Patients were stratified by A-PMN counts of ≤50, 51-249, or ≥250 cells/HPF. Additional analysis was performed with patients strati…

Pre-treatment blood cell counts and inflammatory markers predict the radiologic severity of post-tuberculosis lung disease.

BACKGROUND: Patients with pulmonary tuberculosis (TB) achieve microbiological cures following anti-TB treatment; however, post-tuberculosis lung disease (PTLD) persists in some. This study aimed to evaluate whether pre-treatment blood cell count indicators and inflammatory markers could predict the radiologic severity of PTLD. METHODS: Overall, 169 patients with pulmonary TB diagnosed at Beijing Chest Hospital were prospectively enrolled. Baseline data were recorded and chest computed tomography (CT) images after the completion of anti-TB treatment were scored from 0 to 24 using a validated sy…

An algorithm-based technique for counting mitochondria in cells using immunohistochemical staining of formalin-fixed and paraffin-embedded sections.

PURPOSE: Visualizing mitochondria in cancer cells from human pathological specimens may improve our understanding of cancer biology. However, using immunohistochemistry to evaluate mitochondria remains difficult because almost all cells contain mitochondria and the number of mitochondria per cell may have important effects on mitochondrial function. Herein, we established an objective system (Mito-score) for evaluating mitochondria using machine-based processing of hue, saturation, and value color spaces. METHODS: The Mito-score was defined as the number of COX4 (mitochondrial inner membrane)

High Platelet Count is a Potential Prognostic Factor of the Early Recurrence of Hepatocellular Carcinoma in the Presence of Circulating Tumor Cells.

PURPOSE: Recent studies indicated the vital role of platelet in enhancing the survival of circulating tumor cells (CTCs) in the blood, thereby stimulating the metastasis of tumors. CTCs have been considered an indicator of early tumor recurrence. Therefore, this study evaluated the prognostic potential of platelet count in predicting the early recurrence of hepatocellular carcinoma (HCC) in the presence of CTCs. PATIENTS AND METHODS: 127 patients, whose preoperative CTCs were detected, were enrolled in this study. Univariate analysis was performed to identify the significant association of fa

Mast cell evaluation in gastrointestinal biopsies: should we be counting? A critical review and practical guide for the surgical pathologist.

Mast cells are residents of the tubular gastrointestinal (GI) tract, where they play an important role in host defence and other vital functions. Dysregulation of mast cells has been implicated in the pathogenesis of several neoplastic, inflammatory, and functional disorders, some of which may manifest with GI symptoms. Surgical pathologists must therefore confront when and how to evaluate GI biopsies for mast cells, and whether such decisions should be based on morphologic criteria, clinical context, or direct request from clinical colleagues. The pathologist's role in evaluation of mast cell

High molecular risk variants, severe thrombocytopenia and large unstained cells count affect the outcome in primary myelofibrosis.

Apart from the driver mutations, high molecular risk (HMR) variants and other factors have been reported to influence the prognosis of primary myelofibrosis (PMF). The aim of our study was to investigate the impact of laboratory and molecular characteristics at the time of diagnosis (TOD) on the PMF outcome. The study group consisted of 82 patients recruited from three Polish university centers. Among the driver mutations, only CALR type 1 positively influenced the overall survival (OS). The risk of progression to accelerated or blastic disease phase (AP/BP) did not depend on the driver mutati

Natural killer cells 56bright16- have higher counts in the umbilical cord blood than in the adult peripheral blood.

INTRODUCTION AND HYPOTHESIS: Umbilical cord blood (UCB) is an alternative source of hematopoietic stem cells for allogeneic hematopoietic stem cell transplantation in the absence of a compatible donor. The UCB transplantation has a lower incidence of chronic graft versus host disease (GvHD), but is associated with slower engraftment and slower immune reconstitution, compared to other sources. Dendritic cells (DCs) and Natural Killer cells (NKs) play a central role in the development of GvHD and the graft versus leukemia (GvL) effect, as well as in the control of infectious complications.
