Complications of hydroxyapatite bone cement reconstruction of retrosigmoid craniotomy: two cases.

Hydroxyapatite bone cement is a versatile material used to reconstruct many types of bony surgical defects, and its applications have been widely reported in the literature. Still, complications of its use do occur. We describe 2 cases of hydroxyapatite resorption and subsequent seroma formation in patients who had undergone retrosigmoid craniotomy. The presentation in both cases mimicked a CSF leak. In both cases, the fragmented cement was removed, and the patient experienced no further complications. While hydroxyapatite cement is a highly useful product for the reconstruction of some cranio

Optimal cytocompatibility of a bioceramic nanoparticulate cement in primary human mesenchymal cells.

INTRODUCTION: The aim of this study was to evaluate and compare the cytotoxic effects of BioAggregate (Innovative BioCaramix Inc, Vancouver, BC, Canada), a novel bioceramic nanoparticulate cement, on human mesenchymal cells. White Pro-Root MTA (Dentsply, Tulsa Dental, Tulsa, OK) was used as a reference for comparison. METHODS: Fifty-six human maxillary incisor teeth were submitted to a step-back flaring technique and prepared for cytotoxicity assay in an in situ root-end filling experimental model. After retro filling, each root containing MTA, BioAggregate, or empty root canals (control) was

Cemented total hip replacement cable debris and acetabular construct durability.

BACKGROUND: Third-body wear can adversely affect the outcome of total hip arthroplasty by causing increased polyethylene wear, osteolysis, and component loosening. We hypothesized that there would be greater generation and migration of metal debris to the bearing surfaces in hips in which cobalt-chromium cables were used to reattach the osteotomized greater trochanter when compared with hips in which stainless steel wires were used. METHODS: Between June 1981 and December 1983, 196 consecutive total hip arthroplasties were performed with use of an Iowa stem and a titanium-backed cemented acet

Cemented total hip arthroplasty in a patient with alpha-mannosidosis: a case report.

Mannosidosis is an extremely rare genetic disease occurring due to deficiency of the lysosomal enzyme, alpha-mannosidase. Patients with this disorder often suffer from musculoskeletal abnormalities and muscular weakness leading to joint destruction and severe morbidity along with other major systems involvement. We present here such a case of a 27-year-old male that highlights the challenges in management of hip joint destruction secondary to Mannosidosis.

Systemic trafficking of macrophages induced by bone cement particles in nude mice.

Macrophages play an important role in the biological response to wear particles, which can result in periprosthetic osteolysis and implant loosening. In this study, we demonstrate that polymer particles induce systemic trafficking of macrophages by non-invasive in vivo imaging and immunohistochemistry. The distal femora of nude mice were injected with 10% (w/v) Simplex bone cement (BC) suspensions or saline (PBS). Reporter RAW264.7 macrophages which stably expressed the bioluminescent reporter gene fluc, and the fluorescence reporter gene gfp, were injected intravenously. Bioluminescence imagi

Osteoporosis primer for the vertebroplasty practitioner: expanding the focus beyond needles and cement.

Osteoporosis is a common cause of vertebral compression fractures. Although vertebroplasty is used to treat the pain, the risk of additional compression fractures is very high in these patients. Adequate evaluation and management of the underlying osteoporosis is critical to reducing the risk of subsequent fractures. Such an evaluation involves understanding the underlying physiology of osteoporosis and the role of calcium, vitamin D, prescription medication, and lifestyle changes. This brief review is intended to familiarize neuroradiologists with these aspects so they can advise patients abo

Injectable bone cement augmentation for the treatment of distal radius fractures: a review.

Fractures of the distal radius often occur as a result of low-energy trauma and are frequently seen in osteoporotic patients. Many biomechanical studies and clinical case series have been carried out to investigate the effects on the fractured bones of different fixation methods such as cast immobilization, percutaneous pins, external fixators, or open reduction with internal fixation. In addition, the use of different bone cements as stand-alone solutions or as an adjunct to the aforementioned fixation methods is described as one possible way of increasing the performance of the fixation by r

Percutaneous osteoplasty with use of a cement-filled catheter for a pathologic fracture of the humerus.

Percutaneous osteoplasty has been used for pathologic fractures of long bones as an adjunct to orthopedic surgery. The present report describes a case of percutaneous osteoplasty with a cement-filled catheter and cement augmentation as the principal therapy to unite a painful pathologic fracture of the humerus. After the procedure, the patient experienced immediate pain relief and improved limb mobility. Percutaneous osteoplasty with this method holds promise as a treatment for pathologic fractures of the humerus that are not amenable to surgical repair.

The influence of endplate-to-endplate cement augmentation on vertebral strength and stiffness in vertebroplasty.

STUDY DESIGN: Controlled in vitro trial. OBJECTIVE: To study vertebral strength in relation to cement augmentation technique after vertebroplasty and to assess the influence of the biomechanical compression model on postoperative results. SUMMARY OF BACKGROUND DATA: In the treatment of osteoporotic vertebral fractures, the role of vertebroplasty has been well established. Biomechanical compression models thus far used, compressing vertebrae by only 25% of their initial height, did not show a correlation between cement augmentation volumes and postoperative compression strength. In these stud

Intraspinal leakage of bone cement after vertebroplasty: a report of 3 cases.

We report 3 cases of vertebroplasty-induced intraspinal leakage of bone cement that were referred to us for management. Two patients received decompressive surgery, and one received rehabilitation. The gross surgical finding of yellowish dura mater and intradural fibrosis, adhesion, and microscopic finding of arachnoid membrane fibrosis are suggestive of late effect of thermal injury. These patients had residual lower extremity weakness and urinary and stool problems 13 months, 3 years, and 4.75 years post-vertebroplasty, respectively.

Clinical use of a newly developed calcium phosphate cement (XSB-671D).

BACKGROUND: Several kinds of thermally processed hydroxyapatite have been commonly used for bone defects due to bone diseases or fractures instead of autologous bone grafts. However, these materials do not change in form during surgery and are not absorbed over a long period. To overcome this disadvantage, calcium phosphate bone cements (CPCs) have been developed and are already in use in clinical cases. These materials have merit in that to be formed in situ they are injectable and absorbable, but there are still problems with their handling and their hardening properties. A newly developed C

Leaching of metals on stabilization of metal sludge using cement based materials.

Toxicity characteristic leaching procedure(TCLP) of zinc plating sludge was carried out to assess the leaching potential of the sludge and the leachates were analyzed for heavy metals. The concentration of zinc, chromium, and lead in the leachate were 371.5 mg/L, 1.95 mg/L and 1.99 mg/L respectively. Solidification of zinc sludge was carried out using four different binder systems consisting of cement mortar, fly ash, clay and lime and cured for 28 d. The ratio of sludge added varied from 60% to 80% by volume. The solidified products were tested for metal fixing efficiency and physical strengt

Subsidence of titanium straight stems in combination with highly viscous bone cement.

Varying results and a high rate of subsidence have been reported for the straight femoral stem (M.E. Muller) made of titanium alloy. We examined subsidence in 135 titanium-alloy straight stems implanted with high viscosity cement after 68.8+/-11.5 months using a digital high-precision method (EBRA-FCA). One revised implant showed a subsidence of 14.6 mm and another 2.5 mm over 5 years. A third implant without migration was found to be loose. The 122 implants without loosening showed a mean subsidence of 0.1+/-0.1 mm, and focal osteolysis was seen in two. Altogether, we found subsidence of the

A rough surface finish adversely affects the survivorship of a cemented femoral stem.

UNLABELLED: To assess the role of surface finish in the survivorship of a cemented femoral stem, we evaluated the midterm clinical and radiographic performances of a cohort of patients who had total hip arthroplasties with two cemented femoral stems that differed only in surface finish. One hundred seventy-five patients [64 total hip arthroplasties with rough, textured stems (radius, 1.75-2.5 microm) and 138 total hip arthroplasties with satin finish, textured stems (radius, 0.5 microm)] were followed up clinically and radiographically for 4-8 years. All surgeries were done by one surgeon usin

Ten-year survival of the MS-30 matt-surfaced cemented stem.

The Morscher-Spotorno (MS-30) femoral stem is a stainless-steel, straight, three-dimensionally tapered, collarless implant for cemented fixation in total hip replacement. We report the results at ten years of a consecutive series of 124 total hip replacements in 121 patients with the matt-surfaced MS-30 stem and an alumina ceramic head of 28-mm diameter. All the stems were fixed with Palacos bone cement with gentamicin using a modern cementing technique. They were combined with an uncemented, press-fit cup. The mean period of observation was 10.2 years (8.3 to 12.1) and no patient was lost to

[Good results with a cemented titanium stem after 7-9 years].

AIM: Because of partially ver y controversial results in cemented hip stem replacements made of titanium alloy we investigated in our own patients the mid-term survival of the "Euroform"-stem. This stem is anatomically designed for cemented fixation, made of Ti (6)Al (4)V. METHOD: Within a period between 1990 and 2000 we implanted 2,141 Euroform-prostheses. The course of 103 patients which were operated between April 1990 and April 1992 was followed up. 9 patients died in the meantime and 8 could not be reached. Of the remaining 86 patients we could examine 73 patients with 79 prostheses (87

Radiographic analysis of polyethylene glenoid components using modern cementing techniques.

The purpose of this study was to examine the initial radiographic appearance and changes occurring over time in patients who have undergone total shoulder arthroplasty by modern methods of bone preparation and current cement techniques. Sixty-five patients underwent seventy total shoulder arthroplasties by use of a cemented all-polyethylene, keeled glenoid component. The surface radius was equal to the radius of a one-piece humeral component. The mean clinical follow-up was 4.18 years (range, 2-8.6 years); radiographic follow-up averaged 3.9 years (range, 2-8.6 years). Three observers evaluate

[Antibiotic-loaded bone cements: from laboratory studies clinical evaluation].

Antibiotic-loaded bone cements (ALBC) were initially used empirically, both for the treatment of infected prostheses and for antibioprophylaxis. We conducted in vitro and animal studies as a prerequisite for proper clinical evaluation. We measured gentamicin diffusion from methacrylate bone cement, and found that the concentration in surrounding fluid was significantly above the minimal inhibitory concentration (MIC). We then implanted ALBC in the proximal femora of 100 ewes, in conditions close to those of total hip replacement (THR). Antibiotic concentrations in bone were above 4 x MIC for m
