OBJECTIVE: To describe patterns of atlantoaxial fractures in a population of consecutive elderly patients, including assessment of type, distribution and associated clinical and radiological findings, and to analyze any influence of the causative trauma mechanism on the individual fracture pattern.
MATERIALS AND METHODS: The distribution and type of 123 atlantoaxial fractures in 95 subjects older than 65 years (range: 65 - 102; mean age: 79 years) were retrospectively assessed. For each subject, trauma mechanism and clinical and neurological status were recorded at admission. Initial imaging

The authors report the case of a 27-year-old woman with an arthrogryposis multiplex congenita (AMC) associated with atlantoaxial subluxation. To the authors' knowledge, this is the first report of its kind. The authors review the literature with reference to dysraphic abnormalities associated with atlantoaxial subluxation and with AMC. The patient presented with severe tetraparesis following a minor traffic accident. She underwent a procedure in which transoral decompression and dorsal stabilization were performed and, postoperatively, made a good clinical outcome. The authors stress the need

Cervical myelopathy has become commonly recognized as a complication of rheumatoid atlantoaxial subluxation. A small group of patients with atlantoaxial subluxation may have intermittent symptoms associated with change of head position and which are due to vertebral artery compression. Two such cases are reported, one with necropsy findings of infarction in the area supplied by the vertebrobasilar system. The pathogenesis of the symptomatology and infarction is discussed.

The authors describe an unusual case of a complex traumatic fracture-dissociation injury of the craniovertebral junction, which the patient survived with no neurological damage. This case featured the rare combination of an avulsion of both the right occipital condyle and clivus and a fracture of the left lateral mass of the atlas. Because of the craniocervical ligament injury and the slight anterior occipitoatlantal dislocation, the lesion was considered to be unstable and was treated successfully with a cervical collar. The authors emphasize that thin-slice computerized tomography scanning w

Over 2 years, 16 patients with a total of 23 lesions of the craniovertebral junction and the upper cervical spine were treated. Clinically important aspects of diagnosis and classification are outlined. The conditions treated included fractures of the occipital condyle and atlas, dens-axis, and hanged-man fractures. Standard operative treatment, which has to be modified in cases of combined injuries, is presented. Surgery is frequently indicated, especially in elderly and polytraumatized patients. In addition, 2 cases of atlanto-occipital dislocation are described: 1 of these patients survived

Two new cases are reported of a rare syndrome of malformation consisting of occiptal encephalocele, aplasia of the vermis and deformity of the tectum. Less consistent components are aplasia of mammillary bodies, fusion of thalami, anomalies of cerebral gyral patterns, bifid atlas or bifid occipital squame, elevation of torcular, and cervical hydromyelia. One of the present cases had survived to the age of eight years with a ventricular shunt. This syndrome, of which five cases are known, is thought to be a tandem malformation in which either an occipital dysraphia or an encephalocele induces a

Tumor-related severe atlantoaxial dislocation is rare but may rapidly endanger the cervicomedullary junction when axis destruction, pathological fracture, and craniovertebral instability coexist. This single-case technical note describes a posterior-only occipital plate-assisted reduction technique for a selected patient with severe but reducible tumor-related atlantoaxial dislocation in whom the atlas and axis could not be used as reliable reduction anchors. Conventional reduction strategies for congenital, inflammatory, traumatic, or degenerative atlantoaxial dislocation commonly depend on t

Osteochondromas are the most common primary benign bone tumors which can be either solitary or multiple in the form of hereditary multiple exostosis (HME). Osteochondromas are located frequently in the long bones and rarely involve the spine. Cervical spine remains the most common site for spinal osteochondroma. However, majority of the cases are neurologically asymptomatic as most of them are slow growing with growth directed outside the spinal canal. In this case report, we describe a rare case of solitary osteochondroma arising from C1 vertebra (atlas) resulting in serious neurological comp

STUDY DESIGN: Retrospective radiological study. PURPOSE: To evaluate the anatomical feasibility of posterior occipital condyle-C1 lateral mass (OCLM) screw fixation in patients with basilar invagination (BI) and atlas assimilation (AA). METHODS: Thin-section cervical CTA of 73 patients (25 M / 48 F; 46.4 ± 12.4 year) was reconstructed. (1) Anatomical variables—hypoglossal canal height (HC), canal-to-insertion point height (HCH), upper (UL) and lower (LL) lines, internal (IH) and external (EH) heights—were measured. (2) Screw trajectories: cephalad angles (HCT/IT·CA), medial convergence angles

OBJECTIVE: Fractures of the atlas are typically considered stable or unstable based on the integrity of the transverse ligament. Whereas stable Jefferson burst fractures can be treated nonoperatively, unstable fractures with disruption of the transverse ligament often require surgical intervention. Atlas osteosynthesis has been proposed as a motion-preserving alternative to atlantoaxial fusion. Intraoperative navigation may facilitate safe placement of C1 instrumentation.
METHODS: Cases of patients with unstable atlas fractures treated with navigated osteosynthesis at a single level I trauma

INTRODUCTION: The palatal impaction of canine (PIC) can be predicted by some head and neck skeletal anomalies or variants. Since studies on this regard (especially vertebral anomalies) are scarce, this study was conducted.
METHODS: This case-control study was done on 46 PIC orthodontic patients (34 females, 12 males) and 46 control orthodontic patients (36 females, 10 males). The diagnosis of PIC was done on lateral cephalographs and panoramic radiographs. On cephalographs, sella turcica bridging (occurrence and severity) and ponticulus posticus (occurrence and severity) were assessed. Associ

BACKGROUND/OBJECTIVE: Atlantoaxial fusion procedures may be difficult in situations such as high-riding vertebral artery, a narrow C2 pedicle, and the presence of ponticulus posticus-arcuate foramen. With respect to vertebral artery (VA) anomalies, persistent first intersegmental artery, extracranial origin of the posterior inferior cerebellar artery at the C1-C2 level, and fenestration of the VA are all major risk factors for VA injury in C1-C2 fusion surgeries.
METHODS: To safeguard the VA, we designed the unilateral fusion procedure (all screws lie in the ipsilateral side, either the right

BACKGROUND: To date, no information about the cortical bone microstructural properties in atlas vertebrae with arcuate foramen has been reported. As a result, we aimed to test in an experimental model if there is a cortical bone thickening in an atlas vertebra which has an arcuate foramen that may play a protective role against bone fracture.
METHODS: We analyzed by means of micro-computed tomography the cortical bone thickness, the cortical volume, and the medullary volume (SkyScan 1172 Bruker micro-CT NV, Kontich, Belgium) in cadaveric dry atlas vertebrae with arcuate foramen and without ar

OBJECTIVE: To evaluate the relationship between temporomandibular disorders (TMD) and craniocervical posture in the sagittal plane measured from lateral radiographs of the head.
METHODS: The sample was comprised of 80 randomly selected students of dentistry at the Federal University of Rio Grande do Norte. Research Diagnostic Criteria for TMD (RDC/TMD) was used to evaluate the signs and symptoms of TMD. Lateral radiographs of each individual were used to measure the position of the hyoid bone, the craniocervical angle, and the occiput-atlas distance. A chi-square test was used to evaluate the

STUDY DESIGN: Retrospective study.
OBJECTIVE: The aim of this study is to show that a single traumatic fracture of the atlas is unusual but not rare and requires specific management strategies, which can be highlighted by the study of the junctional ligaments and membranes.
SUMMARY OF BACKGROUND DATA: A single traumatic fracture of the atlas arch is considered a rare event and has been analyzed in few case reports. Ligaments and membranes play a primary role in providing stability to the craniovertebral junction area.
METHODS: Here, we report 10 cases of a single traumatic fracture of the a

Shprintzen-Goldberg syndrome is a rare connective tissue disorder characterized by marfanoid habitus and additional dysmorphic stigmata. Craniocervical anomalies occur in fewer than 30% of cases. Serious vertebral instability can also occur, albeit rarely. The authors report on the first patient treated with surgical fusion at the craniocervical junction because of a C-1 dysplasia and severe instability. The skeletal and cardiovascular anomalies that can pose additional problems for surgical treatment and perioperative care are discussed in detail.
