Reaction of a human head/neck/torso system to shock.

The purpose of this study is to predict human response to, and potential damage from, impact loading by using numerical and physical models to monitor the head and thoracic reactions, intervertebral disk pressures, muscle elongations, and some internal organ pressures. The numerical model consists of a three-dimensional lumped-parameter system of ten rigid bodies connected by nine intervertebral joints and 28 muscle pairs. The masses represent the head; cervical vertebrae C1-C2, C3-C4, C5-C6, C7-T1 (the first thoracic vertebra); the entire thorax; lumbar vertebrae L1-L2, L3, L4-L5; and the pel

Analysing the correlation between preoperative clinical and radiological shoulder balance parameters among adolescent idiopathic scoliosis (AIS) patients with Lenke type 1 and 2 curves.

BackgroundPrevious studies reported that medial shoulder balance (MSB) with neck tilt correlated radiologically with T1 tilt, first rib angle (FRA), and cervical axis (CA), whereas lateral shoulder balance (LSB) correlated radiologically with clavicle angle (Cla-A), coracoid height difference (CHD), clavicle-rib intersection distance (CRID), and radiographic shoulder height (RSH). Nevertheless, there is a scarcity of literature correlating both the clinical and radiological shoulder balance parameters in adolescent idiopathic scoliosis (AIS) patients. This study investigates the correlations b

Sternal fractures occur most often in old cars to seat-belted drivers without any airbag often with concomitant spinal injuries: clinical findings and technical collision variables among 42,055 crash victims.

BACKGROUND: The incidence and treatment of sternal fractures caused by traffic accidents is of increasing importance to ensure best possible outcomes. METHODS: Analysis of technical indicators of the collision, preclinical, and clinical data of patients with sternal fractures from 1985 to 2004 among 42,055 injured patients was assessed by an Accident Research Unit. Two time groups were categorized: 1985 to 1994 (group A) versus 1995 to 2004 (group B). RESULTS: Of 42,055 patients, 267 (0.64%) suffered a sternal fracture. Regarding the vehicle type, the majority occurred after car accidents in

Neglected Sprengel's deformity in an 80-year-old female cadaver: a case report.

BACKGROUND: Sprengel's deformity is a congenital abnormality of the shoulder girdle. Because scapular retraction, such as the Green procedur … rome, presence of an omovertebral bone, and absence of the trapezius muscle. In addition, bilateral cervical ribs were in contact with the brachial plexus. These anomalies may lead to numbness, pain, and limited range of motion of the neck and upper girdle with aging. CONCLUSIONS: Because most adult patients with Sprengel's deformity experience neck pain and …

Surgical management of Gorham-Stout syndrome involving the cervical spine with bilateral pleural effusion: A case report and literature review.

Gorham-Stout syndrome (GSS) is a rare disease characterized by spontaneous and progressive osteolysis caused by benign proliferation of lymphatic vessels or capillaries. It most commonly occurs in children or young individuals without any inherited predisposition. GSS most commonly affects the shoulder girdle, pelvis, ribs and skull. Its diagnosis is mainly based on radiological and pathological findings. The present study reports on the case of a 22-year-old male patient diagnosed with GSS involving the C1-T1 vertebrae accompanied by bilateral pleural effusion. Resection of the occipital and

Familial vertebral segmentation defects, Sprengel anomaly, and omovertebral bone with variable expressivity.

A 10-year-old boy was found to have an unusual presentation of the Sprengel anomaly, omovertebral bones, and segmentation defects of the vertebral column at the cervical, thoracic, and sacral level. In addition, he showed hypertelorism, downslanting palpebral fissures, ptosis, webbing, and hypoplasia of the thenar and hypothenar areas. He had moderate mental delay. In addition to the segmentation defects and omovertebral bones, radiological studies showed a small pelvis and 11 pairs of ribs. Some of the features were present in the mother, and minimal symptoms were present in the father. The p

Longitudinal brainstem laceration associated with complex basilar skull fractures due to a fall: an autopsy case.

This report describes an autopsy case of a rare longitudinal brainstem laceration associated with complex basilar skull fractures. The victim was a 40-year-old male who died immediately after falling from a roof (9.2m in height) of a factory onto a concrete floor. The postmortem examination revealed an incomplete ring fracture of the base of the skull with longitudinal fractures of the sphenoid (clivus of the dorsum sellae turcicae) and occipital bones, cerebral contusions in the frontal and temporal poles, a longitudinal brainstem laceration at the posterior median sulcus of the pons accompan

Unusual anomalies of the arteries at the base of the heart in a dog.

Anomalous blood vessels at the base of the heart in a 6-month-old German Shepherd Dog are described. On the right side, the thoracic vertebral artery was absent and the dorsal intercostal arteries to the first 2 ribs arose directly from the deep cervical artery. The right bronchoesophageal artery was exceptionally large and arose from the right costocervical trunk. An anastomosis was present between the fifth dorsal intercostal artery and the left bronchoesophageal artery. On the left side the left bronchoesophageal artery arose from the left pulmonary artery close to the bifurcation of the pu

Diagnosis of thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is a nonspecific label. When employing it, one should define the type of TOS as arterial TOS, venous TOS, or … stenosis or aneurysms. Symptoms are those of arterial ischemia and x-rays almost always disclose a cervical rib or anomalous first rib. Venous TOS presents with arm swelling, cyanosis, and pain due to subclavian vein obstruction, with or without thrombosis. Neurogenic TOS is due to brachial plexus compression usually from scarred scalene muscles secondary to …

Spinal Involvement in Charge Syndrome: Implications for Management.

CHARGE syndrome (OMIM #214800) is an autosomal dominant disorder caused by mutations in the CHD7 gene in most cases. Although originally defined by the CHARGE acronym (coloboma, heart defects, choanal atresia, growth restriction, genital hypoplasia, and ear anomalies), the recognized phenotype has expanded considerably to include highly prevalent manifestations such as cranial nerve dysfunction, vestibular anomalies, feeding and gastrointestinal complications, endocrine abnormalities, and musculoskeletal involvement. Skeletal abnormalities have been described previously; however, the frequency

A case of cerivco-thoracic anomaly simulating traumatic fracture-dislocation.

A patient who sustained severe head trauma had a simulated C7, T1 fracture dislocation. Anteroposterior tomography of the cervico-thoracic junction disclosed anomalous vertebral-costal articulations. The rotation of the spine and the abnormal posterior articulations were secondary to anterior costal bridging. Tomography of the costo-vertebral facets in patients with unusual rib configurations may help delineate cases of thoracic spine rotation.

A Novel Titanium End-Disc-Assisted Allograft Preparation System for Anterior Cervical Corpectomy and Fusion: Development and Preliminary Biomechanical Evaluation.

Background: Allograft bone remains an attractive option for anterior cervical corpectomy and fusion (ACCF), but its use can be limited by variable graft geometry, insufficient endplate contact, and operator-dependent trimming. This study aimed to develop a titanium end-disc-assisted allograft construct with a standardized cutting and assembly system and to preliminarily evaluate its static mechanical integrity under axial compression, compression-shear, and torsional loading. Methods: The construct consisted of a controlled-length bone graft, two Ti6Al4V end-discs, and four countersunk fixatio
