[Three dimensional finite element model of a modified posterior cervical single open-door laminoplasty].

Objective: To build a three-dimensional finite element models of a modified posterior cervical single open-door laminoplasty with short-segmental lateral mass screws fusion. Methods: The C(2)-C(7) segmental data were obtained from computed tomography (CT) scans of a male patient with cervical spondylotic myelopathy and spinal stenosis.Three-dimensional finite element models of a modified cervical single open-door laminoplasty (before and after surgery) were constructed by the combination of software package MIMICS, Geomagic and ABAQUS.The models were composed of bony vertebrae, articulating fa

Cervical spondylolysis.

The term cervical spondylolysis describes a long-standing, perhaps congenital defect of the pars interarticularis of a cervical vertebra. We report 10 new cases of cervical spondylolysis and review the literature. All patients in this report were treated nonoperatively with subsequent symptomatic improvement. Cervical spondylolysis must be differentiated from its traumatic counterparts radiographically. Characteristic radiographic findings include well-corticated margins at the defect, a characteristic "bow tie" deformity, and ipsilateral dysplastic facets. Compensatory hypertrophic changes of

Atlas fractures: treatment and long-term results.

Fractures of the first cervical vertebra (C1) represent approximately 7% of all acute cervical spine fractures. Frequently, the atlas will be fractured in combination with the axis (44% of cases). Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute traumatic atlas fractures are reviewed. The long-term outcome from an atlas fracture is good (median follow-up, 40 months).

Congenital absence of a vertebral pedicle in the cervical spine. A case report.

Congenital absence of a vertebral pedicle in the cervical spine, a rare disorder, was observed in a 41-year-old woman. The disorder may be confused with a "dumbbell" tumor and erosion by a tortuous vertebral artery. The diagnosis is established by characteristic radiologic changes, especially in oblique roentgenograms, myelograms, and CT scans. Nerve root decompression was performed by hemilaminectomy at the level of the fifth cervical vertebra. There was little relief of pain, the chief preoperative complaint. Surgery may not be indicated for patients with congenital absence of a vertebral pe

Distal junctional failure in posterior cervical and cervicothoracic fusion: A systematic review and meta-analysis of associated factors.

PURPOSE: Distal junctional kyphosis (DJK) is a significant complication following posterior cervical or cervicothoracic fusion. Despite the increasing number of these procedures, the factors associated with DJK remain underexplored, particularly compared to proximal junctional kyphosis. This systematic review and meta-analysis aimed to identify key factors associated with DJK and provide actionable recommendations for its prevention. METHODS: A comprehensive search of PubMed, Scopus, Embase, and Web of Science was conducted to identify case-control and cohort studies reporting on factors asso

Overloaded vertebral body: a unique radiographic phenomenon following multilevel anterior cervical discectomy and fusion.

PURPOSE: Because previous studies have not focused on postoperative cervical collapse, the purpose of the present study was to introduce the overloaded vertebral body (OVB) phenomenon following multilevel zero-profile anterior cervical discectomy and fusion (ACDF) as well as to investigate its effects on radiographic outcomes. METHODS: We conducted a retrospective study involving patients who underwent ACDF. A total of 55 patients were included in the analysis, including 110 OVB and 110 non-OVB. The evaluated vertebral parameters included the vertebral cross-sectional area (CSA), wedge angle

Cervical spine injury after virtual reality gaming: a case report.

BACKGROUND: We report a patient who fractured the seventh cervical vertebra while playing a virtual reality (VR) game, without any other trauma. This case report aims to describe the spinal trauma incurred during the use of a VR headset in a video game. CASE PRESENTATION: The Caucasian patient presented with pain and swelling in the lower cervical spine at our clinic after playing a video game involving a combination of shoulder, arm and head movements while wearing a VR headset. Preexisting comorbidities were not present in the 31-year-old male. No history of regular medication use or drug a

Development of the Synarcual in the Elephant Sharks (Holocephali; Chondrichthyes): Implications for Vertebral Formation and Fusion.

The synarcual is a structure incorporating multiple elements of two or more anterior vertebrae of the axial skeleton, forming immediately posterior to the cranium. It has been convergently acquired in the fossil group 'Placodermi', in Chondrichthyes (Holocephali, Batoidea), within the teleost group Syngnathiformes, and to varying degrees in a range of mammalian taxa. In addition, cervical vertebral fusion presents as an abnormal pathology in a variety of human disorders. Vertebrae develop from axially arranged somites, so that fusion could result from a failure of somite segmentation early in

Anterior retropharyngeal approach (ARPA) for high cervical spine.

BACKGROUND: One of the major challenges in operating on the spine lies in taking an anterior approach for the high cervical spine. In patients with a short neck, Klippel-Fiel syndrome or when the C3 vertebra is high in relation to the hyoid bone, it will be difficult to access the C3 body. The transoral route is a highly contaminated zone, and therefore, no instrumentation or grafts can be placed through it. METHOD: The anterior retropharyngeal approach (ARPA) for the high cervical spine. CONCLUSION: The anterior retropharyngeal approach is an excellent approach for the high cervical spine w

Chalkstick Fracture: A Catastrophic Injury.

Cervical spine region is known as very prone for injury because it is great mobility. Hyperflexion or hyperextension may create life-threatening injury to ankylosed cervical spine vertebra and spinal cord. The posttrauma result can range from neurological damage to the death. We present a case of a 52-year-old male patient, came to the emergency department with a history of road traffic accident induced quadriplegia and shock. The final diagnosis was chalkstick fracture of spine with complete cord transaction.

[Various technics of occipito-cervical fusion].

Indications for occipital-cervical fusion are destruction by tumor, pathological instability and certain fracture dislocations of this region. For palliative surgery we prefer a combined fusion with tension band wire and bone cement; in curative procedures a well fitting cortical bone graft fixed by a circumferential tension band wiring is used. For occipital anchoring we suggest an epidural course of wires through 2 bore holes, in order to cervically fix a circumferential wire around the posterior arch of the vertebra.

A comparative study of breed differences in the anatomical configuration of the equine vertebral column.

The importance of the equine thoracolumbar vertebral column in orthopaedic disorders is well recognized and diagnostic imaging becomes more feasible, but little is known about variations in the anatomical configuration within breeds. In this descriptive post-mortem study, anatomical variations in three widely differing breeds: Warmblood horses, Shetland ponies and semi-feral Konik horses are described. The caudal cervical (C), thoracic (T), lumbar (L) and sacral (S) regions of the vertebral column of 30 Warmblood horses, 29 Shetland ponies and 18 Konik horses were examined using computed tomog

Surgical Management of Unstable Atlas Fractures: A Single Institutional Retrospective Review with Comparison of Occipitocervical and Atlantoaxial Fusion Outcomes.

BACKGROUND: Atlas fractures represent 25% of craniocervical injuries, most common in elders. Unstable first cervical vertebra fractures with lateral mass displacement require surgical fixation. Options include atlantoaxial fusion (AAF) or occiput-cervical fusion (OCF), each have their respective merits and detractors. Here, we compare the clinical and neurological outcomes of patients with unstable atlas fractures treated with AAF and OCF. METHODS: A retrospective review of patients with unstable atlas fractures that underwent either AAF or OCF between 2005 and 2023 was performed. Demographic

Ultrasonographic imaging of loggerhead sea turtles (Caretta caretta).

Twenty live and five dead juvenile and subadult loggerhead sea turtles were examined ultrasonographically. Ten soft tissue areas of the integument were used as acoustic windows: cervical-dorsal and cervical-ventral, left and right cervicobrachial, left and right axillary, left and right prefemoral and left and right postfemoral windows. Anatomical cross-sections were performed on the dead turtles to provide reference data. The fourth and fifth cervical vertebrae, the spinal cord, and the venous sinuses of the external jugular vein were clearly visible through the cervical-dorsal acoustic windo…

Cervical spine radiographs in the trauma patient.

Significant cervical spine injury is very unlikely in a case of trauma if the patient has normal mental status (including no drug or alcohol use) and no neck pain, no tenderness on neck palpation, no neurologic signs or symptoms referable to the neck (such as numbness or weakness in the extremities), no other distracting injury and no history of loss of consciousness. Views required to radiographically exclude a cervical spine fracture include a posteroanterior view, a lateral view and an odontoid view. The lateral view must include all seven cervical vertebrae as well as the C7-T1 interspace,…

The epidemiology of fractures and fracture-dislocations of the cervical spine.

A total of 717 fractures and fracture-dislocations of the cervical spine occurred in 657 patients in a 13-year period. Injury was commonest in the third decade of life. The levels most frequently injured were the second, fifth, and sixth. Injuries of the first and second cervical vertebrae commonly occurred together, and injuries involving the upper and lower cervical spine in the same individual occurred in 9 per cent of patients with fractures of C1 and C2. Odontoid fractures were the most frequent fractures in patients over 70 years of age, and formed the majority of fractures in patients o…

Comprehensive Physiotherapeutic Management of Atlas Occipitalization: A Case Report.

The atlas (C1) and occipital bone at the base of the skull fuse together in atlas occipitalization, an uncommon congenital abnormality. Because it can result in cervical spine instability, nerve impingement, and related symptoms including stiffness, pain, and neurological impairments, it poses a challenging therapeutic problem. We describe the case of a female patient, 27 years old, who had gradually deteriorating neck discomfort, stiffness, and limited cervical mobility for six years prior to presentation. Her symptoms worsened over time despite conservative treatment, so more testing was nec
