Prevention of Early Childhood Caries from the First Level of Care: An Interdisciplinary Approach and Motivational Communication.

Early Childhood Caries (ECC) is considered one of the most prevalent chronic diseases globally, particularly affecting children in socially vulnerable situations. Its significance goes beyond oral health, undermining overall development and quality of life and placing a considerable economic burden on health systems. Given the inadequacy of conventional curative approaches, this manuscript proposes a preventive model based on primary care, structured around an interdisciplinary approach and family-based communication strategies. It analyzes, based on scientific evidence, the fundamental pillar

Multidisciplinary approach to lymphedema diagnosis and management.

Lymphedema is a chronic, progressive condition characterized by the accumulation of protein-rich interstitial fluid due to impaired lymphatic transport. It significantly impairs quality of life and presents complex diagnostic and therapeutic challenges. Despite its prevalence, lymphedema remains underdiagnosed and undertreated, in part due to limited provider education and a lack of access to coordinated care. For this reason, a multidisciplinary approach to lymphedema management is crucial. Early diagnosis is critical and requires collaboration across primary care, oncology, vascular medicine

Caring for the technology dependent child--a case study.

Children who are dependent on medical technology present unique challenges for families and health professionals who attempt to care for them at home. The medical and nursing needs of these children challenge and stretch the financial, social and emotional capacities of parents in ways that service providers do not fully understand. Case management is a collaborative parent/nurse model of community nursing practice that has been implemented to meet the needs of children and families with complex special needs where the care goes across primary and secondary care settings. The aim of case manag

Integrating psychology and medicine in CPAP adherence--new concepts?

To date, continuous positive airway pressure (CPAP) is the most effective intervention in the treatment of obstructive sleep apnoea, but adherence to this treatment is often less than optimal. A variety of factors and interventions that influence and improve CPAP use have been examined. There is increasing recognition of the multifaceted nature of CPAP adherence: the patient's psychological profile and social environment have been recognised, in addition to the more extensively researched patient's treatment and physiological profile. Understanding how these multiple factors impact on CPAP use

Support for deprescribing in European clinical practice guidelines: a systematic review.

BACKGROUND: Polypharmacy is increasingly common among older adults with chronic diseases, yet clinical practice guidelines mainly emphasise initiating therapies rather than discontinuing them. This imbalance contributes to overprescription, therapeutic inertia and preventable harm. OBJECTIVE: To assess the prevalence and quality of recommendations for treatment discontinuation in European guidelines for chronic diseases published between 2019 and 2024. METHODS: A systematic review was conducted following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 standar

Design and implementation of online acceptance and commitment therapy with enhanced therapist support for chronic low back pain (ACT for PAIN).

BACKGROUND: Chronic low back pain (cLBP) typically involves behavior patterns in the development and maintenance of symptoms. Acceptance and Commitment Therapy (ACT) is an evidence-based treatment for chronic pain shown to improve pain-related depression, anxiety, and sleep, as well as pain catastrophizing, pain interference, and levels of reported pain. It has been proven effective when delivered in person or online, but an online therapist-enhanced ACT has not been tested to treat cLBP. This approach may have care delivery advantages while maintaining patient engagement with treatment. OBJE

Fear of movement interacts with trunk mobility, and pain intensity to predict disability in patients with chronic low back pain: a classification and regression tree (CART) analysis.

BACKGROUND: Low back pain significantly impacts global health, particularly in low-income areas where primary care challenges are prevalent. Assessing both biophysical and psychological factors associated with low back pain-related disability is crucial for developing effective screening and management strategies. OBJECTIVES: This study aimed to examine the association between fear of movement, trunk mobility, pain intensity, physical activity, and low back pain-related disability, and to identify key factors contributing to higher disability levels in patients with chronic low back pain. The Costs and Health Benefits of Expanded Access to MDMA-assisted Therapy for Chronic and Severe PTSD in the USA: A Modeling Study.

BACKGROUND AND OBJECTIVE: Intensive psychotherapy assisted with 3,4-methylenedioxymethamphetamine (MDMA-AT) was shown in Phase 3 clinical trials to substantially reduce post-traumatic stress disorder (PTSD) symptoms compared to psychotherapy with placebo. This study estimates potential costs, health benefits, and net savings of expanding access to MDMA-AT to eligible US patients with chronic and severe PTSD. METHODS: Using a decision-analytic model, we compared the costs, deaths averted, and quality-adjusted life years (QALYs) gained of three, 10-year MDMA-AT coverage targets (25%, 50%, and 7

A pilot evaluation of Arthritis Self-Management Program by lay leaders in patients with chronic inflammatory arthritis in Hong Kong.

The objectives of this paper are to evaluate the efficacy of a community-based lay-led Arthritis Self-Management Program (ASMP) among patients with chronic inflammatory arthritis and evaluate the effectiveness of "shared care collaboration" between hospital and community. We trained 17 lay leaders and recruited patients with chronic inflammatory arthritis via a new shared-care model between hospital rheumatology centers and community organizations. Participants were allocated to interventional group or a wait list control group. Evaluations were completed before, after (6 weeks), and 3 months

The medical care abuser: differential diagnosis and management.

The differential diagnosis of patients who are abusive of or seem addicted to medical care is discussed using the approach outlined in the new Diagnostic and Statistical Manual III of the American Psychiatric Association. These are generally chronic disorders and their managements, using the substance abuse/addiction model, are aimed at protecting both patients and physicians from unnecessary, expensive, and potentially lethal medical interventions. The disorders considered include somatization disorder (Briquet's syndrome), psychogenic pain disorder (psychalgia), hypochondriasis, factitious d

Effectiveness of Digital Health Interventions to Improve Self-Care in Patients With Chronic Diseases: Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Chronic diseases account for most global morbidity and mortality, increasing the need for effective long-term self-care support. Digital health interventions, such as mobile apps, telemonitoring, and connected devices, are increasingly used to promote self-care; yet, their overall effectiveness across chronic conditions remains unclear. OBJECTIVE: This systematic review and meta-analysis evaluated whether digital health interventions improve self-care in adults with chronic diseases. METHODS: We searched PubMed, CINAHL, Scopus, and PsycINFO for randomized controlled trials (RCTs;

Use of Mental Health Interventions by Physiotherapists to Treat Individuals with Chronic Conditions: A Systematic Scoping Review.

Purpose: Physiotherapists work with people with chronic conditions and can act as catalysts for behavioural change. Physiotherapy has also seen a shift to a bio-psychosocial model of health management and interdisciplinary care, which is important in the context of chronic conditions. This scoping review addressed the research question "How do physiotherapists use mental health-based interventions in their treatment of individuals with chronic conditions?" Method: The Embase, MEDLINE, PsycINFO, and CINAHL databases were searched, and a variety of study designs were included. Data were categori

Predicting outcome in liver patients admitted to intensive care: A dual-centre non-specialist hospital external validation of the Liver injury and Failure evaluation score.

BACKGROUND: Acute hepatic dysfunction in the critically ill population with pre-existing liver cirrhosis is associated with a high mortality. Several prediction models have been developed to risk stratify patients with liver disease. METHODS: This UK dual-centre non-specialist hospital retrospective study (2015-2019) externally validated the Liver injury and Failure evaluation score (incorporating lactate, bilirubin and International Normalised Ratio), alongside two other general intensive care unit prediction models (Intensive Care National Audit and Research Centre and Acute Physiology and

Chronic disease and smoking cessation intention: associations with oral health status, behaviors, and care in Korea.

BACKGROUND: Oral conditions are immediately perceptible in daily life and may act as proximal factors associated with smokers' intention to quit, yet it is unclear whether these associations differ by chronic disease status. METHODS: We analyzed nationally representative Korea Community Health Survey data (2010-2024) for current smokers aged ≥30 years (N = 522,377). The outcome was cessation intention (yes/no). Oral factors included self-rated oral health, masticatory discomfort, periodontal symptoms, oral-hygiene behaviors (post-lunch toothbrushing; dental flossing/interdental brushes), and

The Aachen ACLF ICU score predicts ICU mortality in critically ill patients with acute-on-chronic liver failure.

Acute-on-chronic liver failure (ACLF) defines a heterogeneous syndrome involving acute decompensation in patients with pre-existing liver disease accompanied by (multi-)organ failure. This study aimed to develop a simple, reliable machine learning (ML) model to predict mortality in ACLF patients receiving intensive care unit (ICU) treatment. Data from 206 patients admitted to the ICU at RWTH Aachen University Hospital between 2015 and 2021 were retrospectively analyzed with ICU mortality as the primary outcome. An ICU mortality prediction model was developed by logistic regression and validate
