The - weak - role of memory in tool use: Evidence from neurodegenerative diseases.

OBJECTIVE: Although tool use disorders are frequent in neurodegenerative diseases, the question of which cognitive mechanisms are at stake is still under debate. Memory-based hypotheses (i.e., the semantic knowledge hypothesis and the manipulation knowledge hypothesis) posit that tool use relies solely on stored information about either tools or gestures whereas a reasoning-based hypothesis (i.e., the technical-semantic hypothesis) suggests that loss of semantic knowledge can be partially compensated by technical reasoning about the physical properties of tools and objects. METHOD: These thre

Learning like a radiologist: a medical vision-language model for radiological image analysis via curriculum learning.

Medical vision-language models (MVLMs) offer promise in clinical practice but face limitations in generalizability, data quality, and clinically meaningful evaluation. We propose RadiSim-CL, an MVLM trained via curriculum learning by simulating the three-phase pathway of a radiologist: foundational knowledge understanding, anatomical knowledge, and advanced diagnostic reasoning. To support this, we curate RadiSim, a 12-million image-text pair dataset aligned to these phases. We evaluate the model using a five-stage coarse-to-fine validation framework: (1) modality recognition, (2) anatomical r

Application of tabu search-based Bayesian networks in exploring related factors of liver cirrhosis complicated with hepatic encephalopathy and disease identification.

This study aimed to explore the related factors and strengths of hepatic cirrhosis complicated with hepatic encephalopathy (HE) by multivariate logistic regression analysis and tabu search-based Bayesian networks (BNs), and to deduce the probability of HE in patients with cirrhosis under different conditions through BN reasoning. Multivariate logistic regression analysis indicated that electrolyte disorders, infections, poor spirits, hepatorenal syndrome, hepatic diabetes, prothrombin time, and total bilirubin are associated with HE. Inferences by BNs found that infection, electrolyte disorder

Factor structure of the Wechsler Intelligence Scale for Children-Fifth Edition: Exploratory factor analyses with the 16 primary and secondary subtests.

The factor structure of the 16 Primary and Secondary subtests of the Wechsler Intelligence Scale for Children-Fifth Edition (WISC-V; Wechsler, 2014a) standardization sample was examined with exploratory factor analytic methods (EFA) not included in the WISC-V Technical and Interpretive Manual (Wechsler, 2014b). Factor extraction criteria suggested 1 to 4 factors and results favored 4 first-order factors. When this structure was transformed with the Schmid and Leiman (1957) orthogonalization procedure, the hierarchical g-factor accounted for large portions of total and common variance while the

Cognitive functioning in Lesch-Nyhan syndrome: a 4-year follow-up study.

Lesch-Nyhan syndrome (LNS) is a rare disorder of metabolism caused by a defective gene on the X chromosome. It is typically characterized by choreoathetosis, hypertonia, hyperreflexia, and self-mutilation. The present study is a 4-year follow-up investigation of the cognitive status of six subjects with a mean age of 17 years 10 months (range 14 years 9 months to 23 years). The Stanford-Binet Intelligence Scale: IV was used. Each of the four domains assessed by this battery (verbal reasoning, abstract/visual reasoning, quantitative, and short-term memory) was compared with previous findings of

A Generational Approach to Neurology Education: Engaging Generation Z Learners.

Neurophobia remains a persistent barrier in neurology education because learners often struggle to connect neuroanatomy, localization, examination findings, and clinical reasoning. At the same time, Generation Z learners now comprise much of undergraduate and graduate medical education and have trained in an environment shaped by ubiquitous mobile technology, short-form media, rapid information retrieval, and heightened attention to feedback, well-being, and belonging. This review synthesizes literature on neurophobia, Gen Z learner preferences, cognitive load, active learning, simulation, mul…

Chronic anosognosia: a case report and theoretical account.

Unawareness of motor disorders (anosognosia) has often been reported after brain lesions, and it has been considered a temporary condition common in the acute and post-acute phases. The presence of anosognosia in a chronic phase (i.e. lasting more than few weeks) is a rare occurrence, thought to be the result of reasoning deficits which prevent patients from performing an adequate check of reality. Although this assumption is widely shared amongst researchers, only a few studies have actually addressed this issue. We report on the case of a patient (NS) who was still showing anosognosia for he

Discordance Between Textual Reasoning and Visual Interpretation in Large Language Models for Low Back Pain: Cross-Sectional Quantitative Evaluation and Exploratory Multimodal Stress Test.

BACKGROUND: Large language models (LLMs) are rapidly evolving from text-based agents to multimodal systems capable of interpreting medical images. While their textual reasoning has improved, the safety implications of this shift remain underexplored, specifically regarding the alignment between visual interpretation and textual advice in low back pain (LBP) management. OBJECTIVE: This study aims to evaluate the performance of 10 commonly used and highly representative LLMs, including paid models such as ChatGPT 5 Plus (OpenAI) and reasoning-oriented models like Qwen3-Thinking (Alibaba/Qwen te

Vibe Coding in nephrology education: clinician-led, AI-assisted development of open-source interactive learning tools.

Medical education increasingly incorporates digital technologies; however, many tools remain passive and text-based. Vibe Coding is a clinician-led design framework that embeds expert reasoning and the cognitive 'feel' of clinical decision-making into interactive educational tools. This study demonstrates its application in nephrology training through the rapid development of open-source, AI-assisted, web-based applications. We conducted a proof-of-concept development study using a structured, physician-led, AI-assisted process combining (1) deconstruction of clinical algorithms, (2) natural-l

Development and pilot evaluation of a large language model-based clinical simulation system for medical education in gastroenterology.

BACKGROUND: Medical education relies on experience to develop clinical reasoning skills, yet patient access is often limited. Large language models (LLMs) offer an accessible alternative for simulating patient encounters. We aimed to develop and evaluate an LLM-based clinical simulation system for gastroenterology that simulates entire clinical encounters in natural language, from history-taking through to treatment and follow-up planning. METHODS: The study was conducted in three phases. First, a system comprising an LLM-based clinical case generator, an LLM-based patient simulator, and a we…

Treating chronic oedema of the lower limb using circular knit garments: how garment characteristics affect outcomes.

The use of compression therapy to treat chronic oedema of the lower limbs can be a challenge, especially when this is undertaken by clinicians who have limited experience and knowledge of the theoretical principles which underpin its use in clinical practice. This articles aims to discuss the reasoning which underpins the use of compression hosiery in the management of lymphoedema and how this understanding can improve treatment outcomes and reduce the burden of disease on patients and clinical resources.

Knowledge Engine, Wisdom System: Aligning Generative AI With the Relational and Epistemic Foundations of Primary Care.

Many patients now turn to generative AI for health advice before seeing a doctor. However, there is little evidence about how well it works in primary care. Research focused on general practice is limited, and real-world harms are often missed because there is no standard way to record AI-related incidents. The gap between what AI provides and what general practice needs is not just technical; it is structural and requires active oversight. AI can recognise patterns, summarise documents, and organise data, but it does not have the judgement, long-term patient knowledge, or moral reasoning that

Beyond the Image Frame: An Art-Based Pedagogical Framework for Teaching Diagnostic Reasoning in Breast Ultrasound to Medical Students.

Breast ultrasound is a key diagnostic method for breast cancer and relies heavily on the interpretation of visual cues. At the same time, medical education is increasingly being driven by time constraints, which favors rapid pattern recognition, limiting the scope for reflective image analysis and the diagnostic process. Therefore, the aim of this study was to propose and evaluate an artistic and pedagogical teaching model, inspired by the interpretive practices of Italian High Renaissance painting, as a tool to support the development of diagnostic reasoning in breast ultrasound. This model f

LLM-Powered Dysphagia Screening With Multimodal Physiological Signal Analysis and Medically Informed Prompts.

Dysphagia is a common complication among stroke patients, significantly increasing the risk of aspiration pneumonia, malnutrition, and mortality. Traditional diagnostic techniques, such as bedside screening and videofluoroscopic swallowing studies, are limited by accessibility, reliability, and invasiveness. To address the challenges of limited data and complex multimodal signals, we propose a large language model (LLM)-based framework for dysphagia screening. This framework integrates multimodal physiological signals-including laryngeal vibration, nasal airflow, and swallowing sound-and lever
