A case of primary HIV type 1 and cytomegalovirus coinfection presenting with widespread clinical disease.

Coinfection of HIV-1 and cytomegalovirus (CMV) may occur given the shared routes of transmission, and the clinical presentations of each process overlap. We present a case of acute HIV-1 and CMV coinfection presenting with an acute febrile illness complicated by meningitis, hepatitis, and retinopathy. This and other similar cases demonstrate the need to consider CMV coinfection in acute HIV-1 disease, particularly in situations with significant end-organ damage.

Coinfection of Fusobacterium nucleatum and Actinomyces israelii in mastoiditis diagnosed by next-generation DNA sequencing.

Some bacterial infections involve potentially complex mixtures of species that can now be distinguished using next-generation DNA sequencing. We present a case of mastoiditis where Gram stain, culture, and molecular diagnosis were nondiagnostic or discrepant. Next-generation sequencing implicated coinfection of Fusobacterium nucleatum and Actinomyces israelii, resolving these diagnostic discrepancies.

Sustained virological response after early antiviral treatment of acute hepatitis C virus and HIV coinfection.

BACKGROUND: Limited data exist describing the clinical outcome and immunological response primed during simultaneously acquired acute hepatitis C virus (HCV) and human immunodeficiency virus (HIV) coinfection. We present detailed clinical and immunological analysis of 3 individuals after concomitant infection with acute HCV and primary HIV. METHODS: In addition to longitudinal clinical parameters, virus-specific T cell responses were assessed using Elispot, standard proliferative (carboxyfluorescein diacetate succinimidyl ester), and in vitro CD4(+) T cell assays. RESULTS: In all patients, a…

Plasmodium falciparum and helminth coinfection in a semi urban population of pregnant women in Uganda.

BACKGROUND: Helminth infections and malaria are widespread in the tropics. Recent studies suggest helminth infections may increase susceptibility to Plasmodium falciparum infection. If confirmed, this increased susceptibility could be particularly important during pregnancy-induced immunosuppression. OBJECTIVE: To evaluate the geographical distribution of P. falciparum-helminth coinfection and the associations between P. falciparum infection and infection with various parasite species in pregnant women in Entebbe, Uganda. METHODS: A cross-sectional study was conducted at baseline during a tr…

Schistosomiasis and HIV-1 infection in rural Zimbabwe: implications of coinfection for excretion of eggs.

BACKGROUND: Stunted development and reduced fecundity of Schistosoma parasites in immunodeficient mice and the impaired ability of human immunodeficiency virus 1 (HIV-1)-infected humans to excrete schistosome eggs have been described. This study explores the effect that HIV-1-associated immunodeficiency has on the excretion of schistosome eggs in a large cohort of coinfected individuals. METHODS: In a cross-sectional survey, urine and stool samples were obtained from and HIV-1 status was determined for 1545 individuals. More extensive data, including quantitative measures of intensity of infe

Epstein-Barr virus coinfection and recombination in non-human immunodeficiency virus-associated oral hairy leukoplakia.

Human immunodeficiency virus-associated oral hairy leukoplakia (HLP) is characterized by coinfection with multiple types and strains of Epstein-Barr virus (EBV) and recombination within the EBV genome. HIV-seronegative immunosuppressed and immunocompetent patients with HLP were examined to determine the pathogenic contribution of EBV coinfection and recombination to the development of HLP. Multiple coinfecting EBV strains were detected in both HLP specimens and peripheral blood lymphocytes (PBL) of HIV-seronegative persons with HLP. One specific EBV strain was detected in HLP specimens from 3 …

Coinfection by Mucoraceae and Aspergillus Species in a Patient With Acute Leukemia: A Clinical Case Report.

The concurrent occurrence of invasive mold infections caused by two distinct fungal species in hematologic patients represents a rare and challenging condition. The mortality rate associated with these invasive fungal infections in neutropenic patients is high, underscoring the importance of timely diagnosis and prompt initiation of appropriate treatment to improve clinical outcomes. Herein, we present a concomitant acute invasive fungal rhinosinusitis (AIFR) and invasive pulmonary fungal infection caused by Aspergillus and Mucoraceae species in a patient with pre-B-cell acute lymphoblastic le

Tissue-specific transcriptomic adaptation in three strains of chickens during coinfections with parasites.

BACKGROUND: The widespread adoption of non-caged production systems contributes to the pathogenesis of coinfections with gastrointestinal nematodes and Histomonas meleagridis, triggering local and systemic immune and metabolic responses in chickens. This study investigated transcriptomic adaptation of tissues two weeks after experimental coinfections with Ascaridia galli, Heterakis gallinarum, and H. meleagridis in Lohmann Brown (LB), Lohmann Dual (LD), and Ross-308 (R) male chickens, which differ in growth rates (R > LD > LB). RESULTS: RNA sequencing of the jejunum, caecum, and liver tissues…

Tick-borne coinfections modulate CD8+ T cell response and progressive leishmaniosis.

Leishmania infantum causes human visceral leishmaniasis and leishmaniosis (CanL) in reservoir host, dogs. As infection progresses to disease in both humans and dogs, there is a shift from controlling type 1 immunity to a regulatory, exhausted T cell phenotype. In endemic areas, the association between tick-borne coinfections (TBCs) and Leishmania diagnosis and/or clinical severity has been demonstrated. To identify immune factors correlating with disease progression, we prospectively evaluated a cohort of L. infantum-infected dogs from 2019 to 2022. The cohort was TBC-negative with asymptomati…

Coinfection of Leprosy and Chronic Mucocutaneous Candidiasis in a Family with STAT1 Gain-of-Function Mutation.

This report documents the first known cases of lepromatous leprosy in patients with chronic mucocutaneous candidiasis (CMC) linked to a gain-of-function mutation in the STAT1 gene. Two related patients, a mother and daughter, who both suffer from CMC and lepromatous leprosy, carry a heterozygous STAT1 mutation (c.821G>A; p.R274Q). Both individuals exhibited similarly high levels of total and phosphorylated STAT1 in CD4+ T cells and decreased interleukin 17A transcripts. This mutation underscores a complex relationship between genetic susceptibility to infections and the necessity to evaluate e
