BACKGROUND: An inverse relationship between surgical volume and outcomes has been suggested, with higher-volume hospitals and surgeons achieving better results, prompting debate over the centralization of surgical services. However, minimum volume thresholds are unclear, and volume is a poor proxy for quality. Despite the significant global burden of colorectal cancer, the benefits of high-volume care remain uncertain. This umbrella review synthesized the evidence on volume-outcome associations in colorectal surgery.
METHODS: An umbrella review (PRISMA 2020) was conducted to evaluate systemat…

BACKGROUND: In colorectal cancer care, planned surgery is sometimes performed without preoperative histological confirmation of malignancy, raising concerns that diagnostic uncertainty may affect surgical quality. Do surgeons maintain high quality oncological resection practices when colorectal cancer is suspected yet not histologically confirmed?
DESIGN: We conducted a nationwide retrospective cohort study using the Danish Colorectal Cancer Group database, including patients undergoing planned colorectal cancer surgery between 2016 and 2020. Patients were grouped according to whether maligna

INTRODUCTION: Physiotherapy is a pivotal component of enhanced recovery protocols, particularly in initiating early mobilisation and supporting the return to functional independence. Despite this, there is a lack of guidance on optimal post-mobilisation physiotherapy strategies. This systematic review aims to synthesise and critically appraise the most recent evidence on postoperative physiotherapy interventions within enhanced recovery pathways for general surgical patients. This review seeks to advance scientific understanding and provide guidance for the optimisation of physiotherapy practi

Robotic-assisted surgery (RAS) has established itself as an advanced technique in colorectal surgery, providing better postoperative outcomes due to its greater precision and less invasiveness. However, RAS requires dedicated and supervised training to avoid complications in the initial learning curve. This study evaluated the safety and efficacy of a supervised RAS training program focusing on postoperative complications, and its classification according to the Clavien-Dindo (CD) grade. This is a retrospective, single-center cohort study that included only patients undergoing RAS performed by…

INTRODUCTION: Colorectal cancer (CRC) is a leading cause of cancer-related morbidity and mortality worldwide. While TNM staging remains the cornerstone for prognostic stratification, growing evidence highlights the role of systemic inflammation and nutritional status in influencing oncologic outcomes. This study aimed to evaluate the prognostic significance of preoperative immunonutritional indices in patients undergoing curative resection for stage I-III CRC.
METHODS: This retrospective cohort study included 616 patients treated between January 2013 and December 2021 at a single tertiary cen

AIM: Colorectal cancer (CRC) is one of the most prevalent malignancies, which is commonly treated with curative surgical resection, often leading to intraoperative hypothermia. Therefore, this study aimed to compare and analyze the risk factors for intraoperative hypothermia associated with laparoscopic and open CRC resections under general anesthesia.
METHODS: This study included 120 CRC patients admitted between January 2023 and January 2024. Data from these patients were analyzed using logistic regression analysis to investigate the risk factors for hypothermia during CRC surgery. Addition

BACKGROUND: Surgical site infections (SSIs) are among the most common postoperative complications in colorectal surgery, and associate with increased morbidity, prolonged recovery, and higher healthcare costs. Most studies combine all SSI categories, potentially overestimating economic burden due to higher severity of organ/space infections. The specific cost impact of incisional SSIs-superficial and deep infections-remains underexplored, despite its prevention potential. This study aimed to evaluate hospital costs associated with incisional SSIs following colorectal resection.
METHODS: A ret…

BACKGROUND: There is controversy regarding the maximum number of elements that can be included in a surgical site infection prevention bundle. In addition, it is unclear whether a bundle of this type can be implemented at a multicentre level.
METHODS: A pragmatic, multicentre cohort study was designed to analyse surgical site infection rates in elective colorectal surgery after the sequential implementation of two preventive bundle protocols. Secondary outcomes were to determine compliance with individual measures and to establish their effectiveness, duration of stay, microbiology and 30-day…

BACKGROUND: Accurate preoperative risk assessment for major colorectal cancer (CRC) surgery remains challenging. Body composition (BC) and cardiopulmonary exercise testing (CPET) can be used to evaluate risk. The relationship between BC and CPET in patients undergoing curative CRC surgery is unclear.
METHODS: Consecutive patients undergoing CPET prior to CRC surgery between 2010 and 2020 were identified between two different UK hospitals. Body composition phenotypes such as sarcopenia, myosteatosis, and visceral obesity were defined using widely accepted thresholds using preoperative single a

BACKGROUND: The success of elective colorectal surgery is mainly influenced by the surgical procedure and postoperative complications. The most serious complications include anastomotic leakages and surgical site infections (SSI)s, which can lead to prolonged recovery with impaired long-term health. Compared with other abdominal procedures, colorectal resections have an increased risk of adverse events due to the physiological bacterial colonisation of the large bowel. Preoperative bowel preparation is used to remove faeces from the bowel lumen and reduce bacterial colonisation. This bowel pr…

BACKGROUND: Flow Cytometry is an analytical technique for the precise quantification of cellular phenotype. Intraoperative Flow Cytometry (iFC) utilizes flow cytometry for DNA content/ploidy and cell cycle distribution analysis during surgery for cancer cell characterization and evaluation of tumor margins. Various types of cancers, including intracranial, head and neck, breast and liver malignancies have been evaluated with iFC. In the current study we present an intraoperative Flow Cytometry protocol for colorectal cancer cell detection and potential resection margin evaluation.
