[Electroconvulsive therapy--sources of error and aids in insufficient duration of convulsions].

Modern electroconvulsive therapy (ECT) involves the administration of six to ten convulsions of sufficient duration (25 seconds) by unilateral electrical stimulation of the non-dominant hemisphere. Missed or abortive seizures are a common clinical problem but may often be accounted for by concurrent medication and technical problems with unilateral ECT seizure induction. In the absence of such factors, controlled hyperventilation and intravenous caffeine are adjunctive means to achieve an adequate unilateral ECT treatment course.

[Cavinton in the prevention of the convulsive syndrome in children after birth injury].

The authors studied the efficacy of cavinton as an agent helpful in preventing neurologic disorders in the newborn with hypoxic ischemic encephalopathy due to intracranial birth trauma. The short-term results of the treatment were elucidated in 61 children. In group I including 20 persons given conventional therapy, the disappearance of seizures was recorded in 6 patients; out of 41 children (group II) given additionally cavinton, in 27. Twenty-nine children were followed up for a year. In group I, convulsive paroxysms recurred in 4 patients, whereas in the group II children, no convulsive syn

Risk of epilepsy after febrile convulsions: a national cohort study.

OBJECTIVE: To identify children with febrile convulsions, classify their febrile convulsions into simple and complex, and determine the number and type of subsequent afebrile seizures in those children. DESIGN: National population based study. SETTING: United Kingdom. SUBJECTS: 16,004 neonatal survivors born during one week in April 1970. MAIN OUTCOME MEASURES: Information about febrile and afebrile seizures obtained from questionnaires at 5 and 10 years of age and from hospital records. RESULTS: Information was available for 14,676 of the cohort children. 398 (2.7%) of them had had at le

Cerebrospinal fluid nucleotide metabolites following short febrile convulsions.

Cerebrospinal fluid (CSF) markers of cerebral energy depletion were measured in 32 infants and children following short (less than 10 minutes) febrile convulsions, and in 19 controls. Specific and sensitive indices of high-energy phosphate compound depletion (hypoxanthine, xanthine and uridine) showed no marked changes. Values for patients and febrile controls were significantly higher than for afebrile controls, which is consistent with increased cerebral metabolism in febrile patients. There were no differences in pH, lactate or creatine kinase levels in the CSF of patients and controls. The

Bilateral anterior dislocation of the shoulder following a grand-mal convulsion.

Bilateral dislocation of the shoulders is an uncommon event. The majority are posterior and occur during a seizure, electrical shock or electroconvulsive therapy, and in patients with neuromuscular disorders and psychiatric disturbances. The literature contains only eight cases of traumatic bilateral anterior dislocations. This report describes only the second case sustained during a grand-mal convulsion.

Convulsant versus typical barbiturates: effects on conflict behavior in the rat.

Typical barbiturates produce a spectrum of behavioral effects, including anti-convulsant, muscle relaxant, sedative hypnotic and anti-anxiety actions. In contrast to these typical barbiturates, there exists a group of barbiturates which are pro-, rather than anti-convulsant. The effects of these convulsant barbiturates on anxiety-related behaviors have not been examined. Therefore, the present studies were designed to compare the effects of the convulsant barbiturate CHEB to those of a number of typical barbiturates in the Conditioned Suppression of Drinking (CSD) paradigm, an "animal model" f

Treatment of malaria and febrile convulsions: an educational diagnosis of yoruba beliefs.

An understanding of community perceptions of illness, especially disease definitions that are unique to a particular culture, is essential for developing culturally appropriate primary health care programs. Malaria is endemic in the Ibarapa District of Oyo State, Nigeria, and one of its major complications, febrile convulsions, affects nearly one-third of preschool children at least once in their lifetime. Perceptions among the local Yoruba people categorize malaria and convulsions as part of two different disease processes. Ideas of causation, severity, seasonality, and treatment are in many

Convulsions as the etiology of lactic acidosis in acute diazinon toxicity in rats.

Diazinon, in acute doses (40 mg/kg, i.p.) in rats produced tremors and convulsions with lactic acidosis which was accompanied by depletion of glycogen and activation of glycogen phosphorylase activity in triceps and diaphragm muscles, 2 h after its administration. Prevention of convulsions with phenobarbitone administered immediately before diazinon, resulted in neither the development of lactic acidosis nor mobilization of muscle glycogen or activation of glycogen phosphorylase. Lactic acidosis was due to depletion of glycogen through enhanced activity of glycogen phosphorylase in muscles on

Effect of CGS-8216 on high-dose flurazepam convulsive threshold.

Flurazepam, a clinically proven and widely accepted sedative hypnotic, has been shown by a number of investigators to produce convulsions at toxic doses. In the present study, CGS-8216, a benzodiazepine receptor antagonist, reduced the dose of flurazepam required to produce convulsions. This suggests that the convulsant action of FLZ is exerted at a site other than the benzodiazepine receptor.

Neuropeptides: anticonvulsant and convulsant mechanisms in epileptic model systems and in humans.

Neuropeptides represent a new class of compounds with important implications for the understanding of the mechanisms and treatment of epilep … expression by regulating arousal systems; however, some TRH analogs have proven to be effective anticonvulsants. Long-term alterations in somatostatin regulation could be components of focal epilepsies. ACTH is particularly useful in the treatment of infantile spasms. Pharmacological effects of these and other peptides have potentials for defining new classes …

Prophylactic treatment with valproic acid or diazepam in children with febrile convulsions.

The purpose of this study was to limit prophylactic treatment of children with febrile convulsions to patients who have the highest risk of recurrence. Two hundred and thirty-one children with a first febrile seizure were divided into high- and low-risk groups according to estimated risk of recurrence. All high-risk children were offered treatment with valproic acid. If this was declined they were offered treatment with diazepam instead. Low-risk children were untreated. Valproic acid and diazepam were found to be equally effective in reducing the risk of recurrence of febrile convulsions. By

Potentiation of the propunishment, but not the convulsant action of the beta-carboline DMCM by naltrexone.

The ability of naltrexone (NTX) to potentiate the propunishment and convulsant properties of DMCM, a benzodiazepine receptor inverse agonist, was studied in mice. Doses (0.39 and 1.56 mg/kg) of DMCM which were below the threshold for propunishment effects showed a marked ability to enhance the suppressive effects of punishment on locomotor activity in the presence of naltrexone (0.5 or 2.5 mg/kg IP), higher doses of DMCM and NTX (3.13 mg/kg and 10 mg/kg, respectively) had a depressant effect of their own on both punished and unpunished locomotor activity. DMCM given alone induced clonic convul

Cerebrospinal fluid leucine-enkephalin-like levels in febrile convulsions.

Enkephalin levels were measured in the cerebrospinal fluid (CSF) of 17 patients with febrile convulsions and 11 control patients without a history of pain or convulsive disorders. Enkephalins were extracted from CSF by adsorption with synthetic resin XAD-2 eluted with methanol, and assayed using the radioreceptor assay (RRA). Results were expressed as Leucine-Enkephalin (Leu-E) equivalents. Enkephalin levels in CSF were 2.27 +/- 0.42 pmol/ml (mean +/- SE) in the convulsion group and the 4.8 +/- 0.8 pmol/ml (mean +/- SE) in the control group. Statistical comparison in both groups shows signific

Parental age and birth order effects in children with febrile convulsions.

This is a study of the parental age and birth order of 100 Chinese children with febrile convulsions compared with 100 controls. The birth order of children with febrile convulsions (1.6 +/- 0.8) was found to be lower than that of the controls (2.1 +/- 1.0), a difference significant at P less than 0.001, whereas there was no significant difference for both paternal and maternal age. Using multiple regression analysis, it was found that the birth order effect was significant at P less than 0.0001. This finding suggests that perinatal factors may play a role in the pathogenesis of febrile convul

The risk of seizure disorders among relatives of children with febrile convulsions.

We studied the risk for seizure disorders among relatives of probands with febrile convulsions. The risk for febrile convulsions was raised in siblings, offspring, and nieces and nephews of probands. Risks to siblings were higher if one or both parents also had febrile convulsions, or the proband had no identified neurologic abnormality before the febrile convulsion, if the proband had recurrent febrile convulsions, or the febrile convulsions were complex. The risk for epilepsy was raised in siblings of probands, but not in other relatives. If the proband had febrile convulsions followed by ep

Multiple actions of convulsant barbiturates on mouse neurons in cell culture.

The convulsant barbiturate 5-(2-cyclohexylidene-ethyl)-5-ethyl barbituric acid (CHEB) depolarized most (greater than 90%) mouse spinal cord (SC) neurons in primary dissociated cell culture in a concentration-dependent fashion with threshold effects at 10 to 50 nM. Dorsal root ganglion (DRG) neurons were also depolarized by CHEB, but only about 50% of the neurons responded. The threshold concentration for depolarization of DRG neurons was several hundred-fold higher than for SC neurons. CHEB depolarizations may be mediated by an increase in a cation conductance which is calcium-dependent becaus

Participant of serotonin turnover rate in the brain on barbital withdrawal convulsion.

The correlation between the development of barbital (B)-withdrawal signs and alterations in the metabolism of brain 5-hydroxytryptamine (5-HT) was studied. Barbital (B)-dependent rats were prepared by the B-Admixed Food method (DAF method). The B-dependent rats were grouped according to the following 5 states: G-I, B-dependent state; G-II, B-withdrawn state; G-III, cross-administration of nitrazepam (NZP) following B-withdrawal; G-IV, cross-administration of chlorpromazine (CPZ) following B-withdrawal; and G-V, cross-administration of phenytoin following B-withdrawal.
