A patient with anomalous coronary circulation presented with non-ST segment elevation myocardial infarction and ischemic mitral regurgitation and underwent mitral valve repair and coronary revascularization. An unexpected tear occurred in the roof of the left atrium, and its subsequent repair caused iatrogenic damage of the anomalous circumflex artery on its abnormal course requiring an extra graft. Early suspicion of the iatrogenic damage to the anomalous artery was the key for good and complete recovery in this case.

We present a case of a nonfunctioning intrapericardial paraganglioma that presented as a typical chest pain in a 51-year-old woman. The tumor was initially diagnosed on coronary angiography where it had direct irrigation from the right coronary artery. Further computed tomography and magnetic resonance imaging scans showed significant compression of the superior vena cava by the tumor. This was excised through median sternotomy and extracorporeal circulation. Histopathological examination of the mass was characteristic of a paraganglioma.

Simultaneous replacement of the ascending aorta and aortic valve with a valved conduit was performed in a patient with aortic valve disease and totally calcified ascending aorta. Coronary circulation could be re-established by using the Cabrol technique, but only after extensive decalcification of the aortic wall surrounding the coronary ostia. This technique appears particularly suitable in patients who require combined replacement of the ascending aorta and aortic valve in the presence of diffuse calcification of the aortic root.

BACKGROUND: Coronary microvascular dysfunction (CMD) is the leading cause of ischemia with no obstructive coronary arteries disease (INOCA) disease. Diagnosis of CMD relies on surrogate physiological indices without objective proof of ischemia.
OBJECTIVES: Intracoronary electrocardiogram (icECG) derived hyperemic indices may accurately and objectively detect CMD and reversible ischemia in related territory.
METHODS: INOCA patients with proven ischemia by myocardial perfusion scan (MPS) and completely normal coronary arteries underwent simultaneous intracoronary electrophysiological (icECG) a

Approximately half of all coronary angiograms performed for angina do not show obstructive coronary artery disease, and many of these patients have coronary microvascular dysfunction (CMD). Invasive testing for CMD has increased with the advent and wider availability of thermodilution systems. We review CMD pathophysiology and invasive diagnostic testing using the Doppler and thermodilution systems. We report the results of a PubMed search of invasive microvascular testing and discuss limitations of current diagnostic algorithms in the diagnosis of CMD, including controversies regarding the op

The left main coronary artery (LMCA) is responsible for supplying the majority of the left ventricular myocardium. Visual estimation of stenosis severity on angiography has major limitations and methods to assess functional significance, such as fractional flow reserve (FFR), have been shown to yield better outcomes; however, to date, major trials examining the use of such physiological indices have excluded LMCA disease. Furthermore, LMCA disease commonly co-exists with downstream disease, which complicates the interpretation of coronary physiological data. This review summarises existing evi

Non-obstructive prosthetic valve thrombosis is a rare and underestimated complication in patients with left-sided mechanical heart valves. Systemic embolisation, mainly involving the cerebral circulation, often represents the first clinical manifestation. We report a case of multiple, successive embolizations in the coronary and cerebral circulation, presenting with an acute myocardial infarction and stroke in a patient with latent, non-obstructive thrombosis of a mechanical bileaflet aortic valve. Because of scheduled urological surgery, chronic vitamin K antagonist treatment had previously b

RATIONALE: Anomalous left coronary artery from the pulmonary artery (ALCAPA) syndrome, also known as Bland-White-Garland syndrome, is a rare congenital heart defect predominantly diagnosed in infants and associated with high early mortality rates without surgical intervention. While ALCAPA generally presents alone, it can sometimes occur alongside other congenital anomalies, such as a bicuspid aortic valve (BAV), a combination scarcely documented in the literature. This case report explores a rare presentation of ALCAPA syndrome in a 27-year-old adult with concurrent BAV.

A left ventricular assist device (LVAD) was applied to a patient who had profound left ventricular failure following extended myocardial infarction caused by the left main trunk obstruction. The patient was a 61-year-old man, who had severe chest distress and was admitted 11 hours after the onset of the symptom. At the time of admission, he was already in cardiogenic shock. The emergent coronary angiography showed complete obstruction of the left main trunk and the intact right coronary artery which had no collateral flow to the left coronary artery. The intraaortic balloon pumping (IABP) was

The circulating interleukin (IL)-18 level is a strong predictor of death from cardiovascular causes in patients with coronary artery disease. However, the mechanisms of IL-18 in orchestrating the cytokine cascade and the accelerator of IL-18 production in atherosclerosis are still unknown. In the present study, we measured the serum concentration of IL-18 and other markers of inflammation in 35 patients with acute coronary syndrome. To determine the mechanism of accelerating IL-18 production, we examined the release of IL-18 in human endothelial cells using human recombinant (hr) C-reactive pr

Arterial vasodilatation is one of the most important characteristics of cirrrhosis and portal hyptertension. Nowadays, it has been known that progressive vasodilatation is an essential factor contributing to hyperdynamic circulation and multiple organ dysfunction in liver cirrhosis. Over the past decades, numerous investigations have originated from the clinical observations. Clinicians and investigators have learned and applied new concepts of the pathophysiology of portal hypertension. For example, we now have effective pharmacologic treatment for hepatorenal syndrome. This review summarizes

A 59-year-old man with aortic stenosis (AS) showed cardiopulmonary arrest requiring extracorporeal circulation. Although coronary angiography did not show coronary artery stenosis, he had an elevated creatine kinase-myocardial band value of 1,298 U/L. Echocardiography revealed severe AS and global hypokinesia of the thickened myocardium. Contrast-enhanced computed tomography (CT) detected a circumferential subendocardial perfusion defect of the left ventricular myocardium. Eventually, the patient died from brain anoxia. Autopsy revealed circumferential subendocardial infarction of the left ven

Isolated anomalous right coronary artery originating from the pulmonary artery (ARCAPA) is a rare congenital coronary anomaly that is asymptomatic and discovered incidentally in most cases. ARCAPA is generally not considered a fatal defect in infancy or childhood, although cases of sudden death have been reported. Here, we report a 2-month-old female infant who presented with a prolonged fever that was determined to be caused by rhinovirus infection. Myocardial ischemia of the left ventricular posterior wall was already seen on echocardiography, and ARCAPA was discovered incidentally. The pati

We report a case of laparoscopic gastrectomy in a patient with refractory vasospastic angina. An 83-year-old man received 2 types of oral coronary vasodilators but complained of chest pain about twice a week thereafter. He was diagnosed with refractory vasospastic angina. Upper gastrointestinal endoscopy was performed for tarry stools and revealed a type 2 tumor in the anglar posterior wall. CT revealed no lymph node swelling or metastasis. Laparoscopic gastrectomy was performed, and intravenous administration of coronary vasodilators was started before the operation. During the operation, cor

BACKGROUND: Limited data exist on the role of coronary collaterals circulation (CCC) in patients with ST-elevation acute coronary syndrome (STE-ACS). This study aimed to assess CCC and the in-hospital course of patients with CCC undergoing primary percutaneous coronary intervention (pPCI).
METHODS: The study included consecutive STE-ACS patients undergoing pPCI. Good CCC was defined as Rentrop collateral score (RCS) of 2-3. Patients with good and poor CCC were compared regarding clinical characteristics, angiographic patterns, and hospital course.
RESULTS: In the sample of 4683 patients, mea

Grafting an internal thoracic artery to a coronary artery with moderate stenosis remains controversial. Competitive flow from the native coronary artery has been proposed as the cause of distal narrowing and ultimate failure of the internal thoracic artery graft. We investigated intraoperative phasic blood flow in internal thoracic arteries grafted to coronary arteries with various degrees of stenosis and the influence of stenosis on postoperative angiographic findings. One hundred patients who underwent coronary artery bypass grafting of an internal thoracic artery to the left anterior descen

Coronary artery anomalies are common congenital disorders with serious consequences in adult life. Coronary circulation begins when the coronary stems form connections between the aorta and the developing vascular plexus. We recently identified the WNT signaling modulator R-spondin 3 (Rspo3), as a crucial regulator of coronary stem proliferation. Using expression analysis and tissue-specific deletion we now demonstrate that Rspo3 is primarily produced by cardiomyocytes. Moreover, we have employed CRISPR/Cas9 technology to generate novel Lgr4-null alleles that showed a significant decrease in c…
