Utility of a clinical support tool for outpatient evaluation of pediatric chest pain.

This study evaluates a clinical pathway currently being employed at a large single-center pediatric cardiology practice. The dataset includes 1,997 pediatric patients with the primary complaint of chest pain. A logistic regression model was developed to predict cardiac disease and identify strong indicators of cardiac pathology. The area under the ROC curve was 0.73 and the Matthews correlation coefficient was 0.23. Given the low incidence of pathology disease, this study was unable to identify strong predictors of major cardiac pathology. The analysis did support syncope, palpitations and the

Geriatric hip fracture clinical pathway: the Hong Kong experience.

Geriatric hip fracture is one of the commonest fractures in orthopaedic trauma. There is a trend of further increase in its incidence in the coming decades. Besides the development of techniques and implants to overcome the difficulties in fixation of osteoporosis bone, the general management of the hip fracture is also very challenging in terms of the preparation of the generally poorer pre-morbid state and complicate social problems associated with this group of patients. In order to cope with the increasing demand, our hospital started a geriatric hip fracture clinical pathway in 2007. The

Midventricular obstruction and clinical decision-making in obstructive hypertrophic cardiomyopathy.

The presence of intraventricular obstruction is a powerful predictor of outcome in patients with hypertrophic cardiomyopathy (HCM) and, when associated with severe, drug-refractory symptoms, should be managed aggressively. Resting left ventricular outflow obstruction is found in approximately 20% of the patients, classically occurs at the subaortic level, and is associated with mitral valve systolic anterior motion (SAM). In a minority of patients, however, the impedance to flow occurs at midventricular level, unrelated to SAM. Symptomatic midventricular obstruction represents a clinical chall

[Design, implementation, and results of the clinical pathway for herniated lumbar disk].

INTRODUCTION: Clinical or critical pathways are a useful tool in the continuous quality improvement. They develop the main characteristics of the quality programs: implementation of the evidence based medicine, increase staff and patient satisfaction and adequacy in hospital costs. This is the application of evidence-based medicine to a process management and it promotes integration of clinical guides, protocols and algorithms. OBJECTIVE: We present our experience in the design, and first nine month's implementation and development of a clinical pathway for lumbar hemilaminectomy. PATIENTS A…

Unraveling the care pathway for CAR T-cell therapy in large B-cell lymphoma: insights from a nationwide study in France (2018-2023).

This study describes the inpatient care pathway for 1914 Large B-cell Lymphoma (LBCL) patients treated with CAR T-cell therapy in France between 2018 and 2023 and identifies factors influencing their management. Based on a retrospective analysis of the French national hospital database, we found the average CAR T-cell infusion stay was shorter for patients in the second line of treatment versus later lines (20.4 vs. 23.8 days). The specific product used was also influential; for example, lisocabtagene maraleucel was associated with shorter stays and fewer ICU admissions than axicabtagene cilol

A Multidisciplinary Perioperative Diabetes Optimisation Pathway: Glycaemic and Hospital Pathway Outcomes in a Prospective Cohort.

AIMS/BACKGROUND: Suboptimal glycaemic control may delay surgery and increase perioperative risk, yet national guidance provides limited detail on how preoperative diabetes optimisation should be organised. This study evaluated a multidisciplinary perioperative diabetes optimisation pathway in a tertiary hospital. METHODS: This prospective single-centre cohort included 174 consecutive adults referred to The Queen Elizabeth Hospital Birmingham, United Kingdom, between 1 January and 31 December 2024, with glycated haemoglobin (HbA1c) >69 mmol/mol (>8.5%) before elective surgery. The pathway comp

Orthopaedic care pathway for diabetic foot disease : experience from a UK rural district general hospital.

AIMS: The diabetic foot is the major cause of non-traumatic amputations of the lower limb in the UK. There are well-established guidelines and pathways for the management of these patients in tertiary centres, but their management in district general hospitals (DGHs) remains inconsistent. The aim of this study was to describe a dedicated orthopaedic pathway, set up and managed by a multidisciplinary foot team in a DGH. METHODS: Patients are managed on either an emergency, urgent, or elective pathway. We describe how these pathways have worked in our DGH: a total of 104 patients were managed i

Timeliness of Care and Treatment Patterns for Pancreatic Cancer in Victoria, Australia: Comparison With Optimal Care Pathways Targets.

INTRODUCTION: Optimal care pathways (OCP) were developed in Australia as a framework to define quality and timeliness targets. The 2022 National Pancreatic Cancer Roadmap highlighted a paucity of population-level data on pancreatic cancer management in Australia, such that compliance with OCP targets is uncertain. This study used real-world data to compare pancreatic cancer care in Victoria, Australia, against OCP targets. METHODS: Data collected on pancreatic cancer patients between January 2016 and July 2025 were extracted from the PURPLE registry for five Victorian tertiary hospitals. Care

Implementation and evaluation of an ambulatory care pathway for intra-arterial treatment of primary liver cancer: study protocol for a multicentre randomised controlled hybrid type 1 trial (CHOC).

INTRODUCTION: Primary liver cancers, including hepatocellular carcinoma and intrahepatic cholangiocarcinoma, are one of the leading causes of cancer-related mortality. Transarterial chemoembolisation (TACE) and radioembolisation (TARE) are established palliative treatments yet they are traditionally performed during inpatient hospitalisation. Recent technical and organisational advances allow for safe same-day ambulatory procedures. In particular, the extent to which ambulatory intra-arterial therapy aligns with patients' expectations, comfort and quality of life remains poorly documented. The

[Care pathway to diagnosis in fibromyalgia and inflammatory rheumatic diseases: A patient-centered approach using a comparative mixed-methods study].

BACKGROUND: Many doctors have a complicated relationship with patients with fibromyalgia (FM), whereas they feel more comfortable with patients with well-defined clinico-biological illnesses. The aim of the study was to compare the care pathways as experienced by patients with FM or those facing well-defined rheumatic diseases. METHODS: Consecutive participants diagnosed with FM, spondyloarthritis (SpA), or rheumatoid arthritis (RA) were prospectively recruited from a tertiary outpatient center. Utilizing a semi-structured guide, participants were interviewed. A quantitative analysis compared

Clinical care pathway for the evaluation of patients with suspected VITT after ChAdOx1 nCoV-19 vaccination.

Vaccine-induced immune thrombotic thrombocytopenia (VITT) is a rare complication after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) adenoviral vector vaccination. In British Columbia (BC), Canada, a provincial clinical care pathway was developed to guide clinicians in evaluating for VITT among patients who present with thrombocytopenia or thrombosis symptoms within 4 to 28 days after adenoviral vector vaccine exposure. All patients had enzyme-linked immunosorbent assay (ELISA) testing for platelet factor 4 (PF4) antibodies, and all cases with positive PF4-ELISA or d-dimer level

Staged reflexive artificial intelligence driven testing algorithms for early diagnosis of pituitary disorders.

BACKGROUND: Sellar masses (SM) frequently present with insidious hormonal dysfunction. We previously showed that, by utilizing a combined reflex/reflecting approach involving a laboratory clinician (LC) on common endocrine test results requested by non-specialists, and subsequently adding further warranted tests, previously undiagnosed pituitary disorders can be identified. However, manually employing these strategies by an LC is not feasible for wider screening of pituitary disorders. OBJECTIVE: The aim of this study was to compare the accuracy and financial impact of an Artificial Intellige

How do I best manage insomnia and other sleep disorders in older adults with cancer?

Insomnia is common in older adults with cancer, with a reported prevalence of 19-60% in prior studies. Cancer treatments are associated with increased risk of insomnia or aggravation of pre-existing insomnia symptoms, and patients who are receiving active cancer treatments are more likely to report insomnia. Insomnia can lead to significant physical and psychological consequences with increased mortality. We discuss physiological sleep changes in older adults, and illustrated the various sleep disorders. We present a literature review on the prevalence and the effects of insomnia on the qualit

Systematic mechanism-orientated approach to chronic pancreatitis pain.

Pain in chronic pancreatitis (CP) shows similarities with other visceral pain syndromes (i.e., inflammatory bowel disease and esophagitis), which should thus be managed in a similar fashion. Typical causes of CP pain include increased intrapancreatic pressure, pancreatic inflammation and pancreatic/extrapancreatic complications. Unfortunately, CP pain continues to be a major clinical challenge. It is recognized that ongoing pain may induce altered central pain processing, e.g., central sensitization or pro-nociceptive pain modulation. When this is present conventional pain treatment targeting

Would a new definition and classification of nocturia and nocturnal polyuria improve our management of patients? ICI-RS 2014.

The following is a report of the proceedings of the Nocturia Think Tank sessions of the annual International Consultation on Incontinence-Research Society, which took place September 22-24, 2014 in Bristol, UK. The report is organized into sections pertaining to the main topic of discussion focussing on the question as to whether a new definition and classification of nocturia and nocturnal polyuria would improve the outcome of management in our patients. First, discussions identified theoretical and practical shortcomings of current definitions. Secondly, the utility of several nocturnal poly
