Neurogenic intermittent claudication and stenosis of the lumbar spinal canal.

Neurogenic intermittent claudication is briefly reviewed and correlated with twenty consecutive cases of lumbar spinal canal stenosis. Irreversible damage may occur if surgical decompression is unduly delayed. A unilateral decompression may be sufficient in carefully selected cases.

Case of lumbar spinal stenosis and chronic tophaceous gout.

BACKGROUND: Rarely, chronic tophaceous gout can result in lumbar spinal stenosis and neural compression. CASE DESCRIPTION: A 67-year-old male presented with the radiographic and magnetic resonance findings of gout involving and causing compression of the lumbar spine that responded to surgical decompression. CONCLUSION: It is difficult to diagnose lumbar spinal stenosis secondary to tophaceous gout. Notably, the treatment, based on the clinical presentation, may include both medication and surgical decompression.

Orbital decompression in Graves' orbitopathy: efficacy and safety.

BACKGROUND: Vision-threatening compressive optic neuropathy occurs in a minority of patients with Graves' orbitopathy. Surgical orbital decompression, systemic glucocorticoids and orbital irradiation are treatment options. Orbital decompression is being performed on an increasing number of patients for other indications such as corneal exposure and disfiguring proptosis. AIMS: To examine the outcomes of surgical orbital decompression for Graves' orbitopathy by one surgeon. METHODS: An analysis of a retrospective case series of 88 consecutive patients (151 orbits) who underwent orbital decomp

Pseudogout: A Rare Cause of Acute Carpal Tunnel Syndrome and Acute Guyon Canal Syndrome.

Pseudogout is an acute inflammatory arthropathy that often presents as a hot, swollen, painful joint. Rarely, the inflammatory response caused by pseudogout has led to acute neuropathic symptoms of the hand. We present a case of pseudogout causing acute neuropathic symptoms in the median and ulnar nerves, ultimately necessitating urgent surgical decompression. The patient's symptoms were alleviated after surgical decompression. Pseudogout should be considered a rare cause of acute neuropathic compression of the hand. Its management may require surgical intervention and should involve postopera

Reoperation rate after surgery for lumbar spinal stenosis without spondylolisthesis: a nationwide cohort study.

BACKGROUND CONTEXT: Lumbar spinal stenosis is one of the most common degenerative spine diseases. Surgical options are largely divided into decompression only and decompression with arthrodesis. Recent randomized trials showed that surgery was more effective than nonoperative treatment for carefully selected patients with lumbar stenosis. However, some patients require reoperation because of complications, failure of bony fusion, persistent pain, or progressive degenerative changes, such as adjacent segment disease. In a previous population-based study, the 10-year reoperation rate was 17%, an

Emergency decompression of tension retrobulbar emphysema secondary to orbital floor fracture.

Orbital floor fractures are generally the result of blowout orbital and may be associated with orbital emphysema leading to proptosis and even to loss of vision. A 49-year-old woman fractured the orbital floor in a fall. After blowing her nose, she developed exophthalmos and severe reduction in vision. She consulted our department and underwent emergency surgical management with orbital drainage. Decompression led to immediate resolution of the exophthalmos and postoperative improvement in visual acuity. Urgent decompression is indicated by the presence of proptosis, elevated intraocular press

Biportal endoscopic non-facetectomy foraminal decompression and discectomy (ligamentum flavum turn-down technique).

This study introduces a novel biportal endoscopic foraminal decompression technique that minimizes bone removal while ensuring safe and effective nerve root decompression. Leveraging the accessory process as a key surgical landmark, this technique enables precise navigation and controlled turn-down of the ligamentum flavum (LF). A key advantage of this technique is its reduced requirement for bone resection, differing from traditional microscopic or uniportal endoscopic surgeries that often necessitate resection of the lateral isthmus or superior articular process. This technique is particular

Interspinous process spacers versus traditional decompression for lumbar spinal stenosis: systematic review and meta-analysis.

BACKGROUND: Interspinous spacers are used in selected patients for the treatment of lumbar spinal stenosis. The uses of interspinous devices are still debated, with reports of significantly higher reoperation rates and unfavourable cost-effectiveness compared to traditional decompression techniques. METHODS: Six electronic databases were searched from their date of inception to December 2015. Relevant studies were identified using specific eligibility criteria and data was extracted and analyzed based on predefined primary and secondary endpoints. RESULTS: Eleven comparative studies were obt

Does pain relief by CT-guided indirect cervical nerve root injection with local anesthetics and steroids predict pain relief after decompression surgery for cervical nerve root compression?

BACKGROUND: The relationship of pain relief from a recently presented CT-guided indirect cervical nerve root injection with local anesthetics and steroids to surgical decompression as a treatment for single-level cervical radiculopathy is not clear. This retrospective study aimed to compare the immediate and 6-week post-injection effects to the short- and long-term outcomes after surgical decompression, specifically in regard to pain relief. METHODS: Patients (n = 39, age 47 ± 10 years) who had undergone CT-guided indirect injection with local anesthetics and steroids as an initial treatment

Native paraneurial tissue and paraneurial adhesions alter nerve strain distribution in rat sciatic nerves.

Paraneurial adhesions have been implicated in the pathological progression of entrapment neuropathies. Surgical decompression of adhesions is often performed, with the intent of restoring nerve kinematics. The normal counterpart of adhesions, native paraneurium, is also thought to influence nerve deformation and mobility. However, influences of native or abnormal paraneurial structures on nerve kinematics have not been investigated. We measured regional strains in rat sciatic nerves before and immediately after decompression of native paraneurial tissue, and before and after decompression of a
