Contact dermatitis with a highlight on silver: a review.

Contact dermatitis is an inflammatory reaction in the skin that arises from exposure to an irritant or allergen. Irritant contact dermatitis is the most common form of dermatitis and occurs when any substance disrupts the epidermal skin barrier. All individuals are susceptible to this type of reaction. Allergic contact dermatitis is a delayed Type IV hypersensitivity reaction that occurs in individuals that have previously been sensitized to a particular antigen. Patients with wounds are at particular risk of developing either an allergic or irritant contact dermatitis. Various contribut

Treatment of irritant and allergic contact dermatitis.

The treatment of contact dermatitis lies principally in the avoidance of the offending agent. In certain circumstances, avoidance protocols are insurmountable, and therapy is rendered to assuage the inflammatory component and its consequent objective and subjective findings. However, the options thereafter vary, as some patients will require continuous symptomatic therapy despite avoidance of the purported offending agent. This manuscript will review established treatment options for contact dermatitis, such as corticosteroids and dietary manipulation, as well as discuss some promising new the

Allergic contact dermatitis to an expired moisturiser.

A 64-year-old man developed an allergic contact dermatitis while using an expired sorbolene cream. Patch tests to the product ingredients were all negative. Repeated open application test to the expired product produced a strong reaction while an identical-brand non-expired product did not elicit a reaction, suggesting the presence of an unidentified degradation product or compound, acting as an allergen in the expired product. This case is intriguing in that it contrasts the ubiquitous use of over-the-counter topical agents with the exceedingly low incidence of contact dermatitis to expired t

Irritant contact dermatitis: is there an immunologic component?

Irritant contact dermatitis (ICD) describes a multifactorial disease resulting from exposure to occupational irritants. Though often characterized as a localized reaction, there are at least 10 forms of ICD, several of which possess systemic involvement. The exact mode of irritant action in ICD is not completely understood, but recent literature suggests that an immunologic-like component may be present. The correlation between the morphology, clinical manifestations and chemokines involved in allergic contact dermatitis and irritant contact dermatitis will be discussed and studies indicating

Atopic, contact, and seborrheic dermatitis in adolescents.

Atopic dermatitis, contact dermatitis, and seborrheic dermatitis are chronic or recurrent inflammatory dermatoses that commonly affect adolescents. They may present a diagnostic and therapeutic challenge for physicians, and can be frustrating for patients. This article reviews the pathogenesis, diagnosis, and management of these diseases.

Allergic and irritant contact dermatitis to calcipotriol.

Calcipotriol (Daivonex R; Leo Pharmaceuticals, Zurich, Switzerland) may cause irritation of the skin, whereas allergic reactions are less common. In the present study we describe two patients with different types of reaction patterns, one presenting as an allergic, the other as an irritant contact dermatitis. Irritative skin reactions were observed only at higher testings doses, in contrast to the allergic type of reaction, which occurred at a lower testing dose. The present observation suggests, that a batch of different testing doses, including lower testing doses may help to differentiate b

Perfluoropolyethers in the prevention of irritant contact dermatitis.

BACKGROUND: Perfluoropolyethers (PFPEs) are non-reactive perfluorinated (no hydrogen atoms) liquid polymers that showed promise as ingredients of protective preparations in occupational dermatology. OBJECTIVE: The aim of this study was to investigate if oil-in-water (o/w) emulsions containing 0.5, 1.0, 2.0 and 4.0% PFPEs (molecular weight 6,250 D) prevent epidermal barrier disruption induced by a repetitive irritation test (RIT). METHODS: PFPE-containing o/w emulsions and the emulsion base alone were evaluated against a set of 4 irritants [10% sodium lauryl sulphate (SLS), 0.5% sodium hydrox

[Analysis of contact dermatitis by cytokines. II].

We examined the mechanism of the onset of contact hypersensitivity reaction (CHR) induced by several kinds of chemical substances in mice, focusing on cytokines. Before doing this experiment, we investigated the involvement of T cells in the formation of CHR using each primer of CD4 and CD8 (surface markers of lymphocytes) positive cells. It was suggested that both cells participate in the formation of CHR. On the basis of these results we analyzed cytokines produced in the reaction site on the mRNA level in with or without the presensitization of two kinds of strong contactants. Consequently,

Studies on the reproducibility of allergic contact dermatitis.

Diagnosis of allergic contact dermatitis depends upon the use of epicutaneous patch tests with suspected antigens. It has been claimed that the method is unreliable because of variability. Therefore, we have tested this by comparing responses to various forms of replicate, standardized challenge in nickel-sensitive subjects. Nickel-containing coins applied at 2-4 different sites on the back elicited similar responses in up to 95% of tests. Serial dilutions of nickel sulphate (NiSO4) applied, in duplicate, gave highly reproducible responses on the back. However, the forearm skin was clearly les
