Successful Endovascular Treatment of Calcified Superior Mesenteric Artery Complicated by Intramural Hematoma in Chronic Mesenteric Ischemia.

BACKGROUND Chronic mesenteric ischemia (CMI) is defined as ischemic symptoms caused by insufficient supply of blood to the gastrointestinal tract. Patients diagnosed with advanced symptomatic CMI should be treated subsequently, as the transition from CMI to acute mesenteric ischemia can be unpredictable. However, there is little information regarding the management of potential procedural complications during endovascular therapy (EVT) for CMI. CASE REPORT A 70-year-old man was admitted to our hospital with recurrent abdominal pain just after hemodialysis. The angiogram showed significant sten

Three-Dimensional Peripheral Bloodstream Model of the Uterus for Laparoscopic Radical Hysterectomy.

Trainees require extensive experience to perform radical hysterectomy. Before starting training during an actual operation, trainees should be familiar with the pelvic anatomy and should simulate surgical procedures. Many simulators are available for virtual reality training of laparoscopic operations, but they are very expensive. The materials required to construct our model included sponges and colored wires sold in home improvement stores that allowed for superior cost effectiveness. The model represented almost all peripheral vessels and nerves around the uterus, including the minor vessel

Platelet-hitchhike nanomedicine for theranostics of coronary artery dissection via X-CT imaging and inhibiting NLRP3 signaling pathway.

BACKGROUND: Primary or secondary coronary artery dissection can lead to serious cardiovascular diseases such as acute myocardial infarction, arrhythmia, or sudden cardiac death. However, the early identification and timely intervention for coronary artery dissection remain significant clinical gaps. RESULTS: We propose a practical strategy for the theranostics of coronary artery dissection using the stimuli-responsive nanomedicine CIFu. By leveraging the natural process of platelet activation and recruitment to the dissection tissue, CIFu can specifically adhere to activated platelets, thereb

What does anatomically eligible mean in contemporary thoracic endovascular aneurysm repair for type B aortic dissection?

Modern management of type B aortic dissection (TBAD) in some cases necessitate treatment by thoracic endovascular repair (TEVAR). Some patients are not anatomically eligible for simple, standard TEVAR, since critical side branches of the aorta would be covered by the stent graft and might require revascularisation. This commentary will outline some strategies for complex thoracic aortic repair in the aortic arch that could be considered in patients with dissection anatomy that does not allow for stand-alone TEVAR. Current guidelines provide vague guidance in assessment of anatomic eligibility

Predictors of Distal Stent Graft-Induced New Entry after Frozen Elephant Trunk in Acute Type A Aortic Dissection.

OBJECTIVES: Distal stent graft-induced new entry (d-SINE) is a significant complication of frozen elephant trunk (FET) for acute Stanford type A aortic dissection (ATAAD). This study aimed to identify the predictive factors for d-SINE occurrence. METHODS: Patients who underwent FET procedures for ATAAD at our institution between January 2015 and December 2024 were retrospectively analysed. Patients were classified into d-SINE positive and d-SINE negative groups, and predictive factors were identified using Fine and Gray competing risk regression. RESULTS: A total of 84 consecutive patients w

Landing zones optimization using transcatheter electrosurgical septotomy for endovascular repair of post-dissection aortic aneurysms.

OBJECTIVE: Aortic dissection is the second most common cause of aneurysmal degeneration. Although endovascular repair is preferred over open techniques, multiple secondary interventions are required to manage complications related to the dissection of potential landing zones (LZs). This study aims to report the outcomes of adjunctive use of transcatheter electrosurgical septotomy (TEAS) to optimize LZs for endovascular repair of post-dissection aortic aneurysms (PD-AAs). METHODS: Consecutive endovascular repairs with adjunctive TEAS performed for PD-AAs between 2022 and 2023 at two institutio

Tetraplegia after branched frozen elephant trunk implantation for acute type A aortic dissection.

A 58-year-old male patient presented with acute type A aortic dissection. Complete arch and ascending aorta replacement were performed using a Thoraflex Hybrid prosthesis. The left subclavian artery was ligated and the remaining supra-aortic trunks were reimplanted using the branches of the prosthesis. After an uneventful early postoperative period, sudden onset of hypotension and bradycardia occurred, with severe vasoplegia, requiring vasopressors. Ischemia of the upper left limb and compartment syndrome ensued, leading to left carotid subclavian bypass. After discontinuation of sedation, tet

Staged Hybrid Single Lumen Reconstruction (TIGER) in Management of Chronic Symptomatic Complex Type B Aortic Dissection, Techniques, and Literature Review.

BACKGROUND: Endovascular intervention for chronic symptomatic type B aortic dissection (CS-TBAD) induces aortic wall stress with negative hemodynamic cardiovascular consequences. CS-TBAD risks increased morbidity and mortality due to septum maturation with significant impact on false lumen modulation, and partial lumen thrombosis conveying the worst outcome. The aim of the TIGER technique is total aortic remodeling with true lumen expansion, false lumen regression and complete thrombosis, and stabilization of overall aortic diameter. METHODS: We report 5 cases of aortic dissection with a mean

Incorrect frozen elephant trunk deployment into the false lumen of a patient with complicated type B acute dissection.

Total arch replacement with frozen elephant trunk (FET) is used for type B acute aortic dissection with insufficient proximal landing zone. Herein, we report incorrect deployment of FET into the false lumen. A 45-year-old man underwent femorofemoral bypass for complicated type B acute aortic dissection. However, chest pain recurred 4 days postoperatively, and an oozing rupture of the dissecting descending aorta was diagnosed. As preoperative computed tomography showed insufficient proximal landing zone and separate chronic aortic dissection of the proximal arch, total arch replacement with FET

Modified Candy-Plug Technique for Rescue Type B Aortic Dissection with False Lumen Rupture.

Despite recent advances in surgical techniques and endovascular devices, the management of ruptured type B aortic dissection remains challenging. Herein, we describe a patient with a history of chronic type B aortic dissection who presented to the emergency department with a massive left hemothorax and profound shock. Computed tomography demonstrated a thoracic dissecting aneurysm with false lumen rupture. He underwent emergent thoracic endovascular aortic repair and carotid-carotid crossover bypass in a hybrid operating room. Although the thoracic endovascular aortic repair resulted in suitab

Impact of Retrograde Arch Extension in Acute Type B Aortic Dissection on Management and Outcomes.

BACKGROUND: Optimal management of acute type B aortic dissection with retrograde arch extension is controversial. The effect of retrograde arch extension on operative and long-term mortality has not been studied and is not incorporated into clinical treatment pathways. METHODS: The International Registry of Acute Aortic Dissection was queried for all patients presenting with acute type B dissection and an identifiable primary intimal tear. Outcomes were stratified according to management for patients with and without retrograde arch extension. Kaplan-Meier survival curves were constructed. R

Hybrid repair of type II dissecting thoracoabdominal aneurysm using Amplatzer vascular plugs for entry tear closure.

Endovascular repair of chronic aortic dissections (CAD) intend to promote false lumen thrombosis (FLT). This article describes a technique using Amplatzer vascular plugs (AVPs) for entry tear closure of CAD. A 70-year-old man presented with a type II dissecting thoracoabdominal aneurysm. Computed tomography scan showed a very tight true lumen, partial FLT, and 2 entry tears at the level of the left subclavian artery and the visceral aorta, respectively. During a first procedure, aortic debranching was performed using the ascending aorta as bypass inflow. In a second intervention entry tears we
