[Hybrid grafts for aortic arch reconstruction: evolution and state of the art].

The analysis of the gained experience in treatment of patients with pathology of the arch and descending portion of the aorta is suggestive that the 'frozen elephant trunk' technique is an effective and safe method. In this connection, there is an increasing need for universal hybrid devices characterized by ease of use and durability in various aortic pathologies. Discussed in the article is the state of the art of the technology of treatment of patient with aortic arch pathology by means of hybrid grafts, followed by comparative analysis of currently used vascular hybrid prostheses, and alth

Zone 0 Thoracic Endovascular Repair After Aortic Wrapping for Acute Aortic Syndrome.

We report a successful treatment of type A acute aortic syndrome (AAS)-associated aortic arch aneurysm in a 71-year-old man with major comorbidities. The ascending aorta was wrapped with artificial graft, and supra-aortic debranching was constructed. Then, Zone 0 thoracic endovascular aneurysm repair (TEVAR) with plug occlusion of the left subclavian artery was successfully performed. The patient was discharged in good physical condition without any complications. To our knowledge, this is the first reported case in the literature of successful Zone 0 TEVAR after ascending aorta wrapping and s

Combined heart transplantation and resection of dissecting aneurysm of ascending aorta and aortic arch: a case report.

A 21-year-old male patient had suffered from palpitation and exertional dyspnea since October, 1997. He was admitted to our hospital, and a series of examinations were performed. Chest computed tomography (CT) revealed marked dilatation of the ascending aorta (about 7.5 cm at the proximal portion) and aortic annulus, an intimal flap in the ascending aorta and aortic arch was also noted. Cardiac catheterization revealed the pulmonary capillary wedge pressure was 33 mmHg, pulmonary artery pressure was 47/38 mmHg with a mean of 35.4. The cardiac index was 1.01 l/min/m2. Poor left ventricular cont

Video-assisted open repair of proximal thoracic aortic pseudoaneurysm.

Open surgery remains an essential alternative for descending thoracic aortic aneurysms repair when anatomical constraints preclude endovascular intervention. We present the case of a 63-year-old woman with a complex pseudoaneurysm located at the junction of the distal aortic arch and proximal descending aorta (zones 3/4), approximately 7 mm distal to the origin of the left subclavian artery. The absence of a proximal landing zone rendered thoracic endovascular aortic repair unfeasible. A limited left thoracotomy was performed at the fourth intercostal space. Cardiopulmonary bypass was establis

[Genetic of thoracic aorta aneurysm].

The thoracic aortic aneurysm corresponds to the dilation of the ascending part of the aorta, which can lead to a dissection (TAAD for Thoracic Aortic Aneurysm and Dissection) or aortic rupture. The etiologies are diverse, but in approximately 20% of cases a genetic origin is found. About thirty genes are reported to be responsible for the development of TAAD. The majority of these genes encode for proteins involved in the extracellular matrix, the contraction of smooth muscle cells or the growth factor TGF-β signaling pathway. Identifying the pathogenic variant responsible for the aortic disea

Thoracic Aortic Rupture Post Cardiopulmonary Resuscitation in a Patient With Previous Thoracic Aneurysm Repair.

Aortic dissection is characterized by a tear or rupture in the intimal layer of the aorta causing blood to flow between the layers of the arterial wall, thus separating them. While cardiopulmonary resuscitation (CPR) is a life-saving intervention, it can unintentionally contribute to the development or worsening of aortic dissection. The forceful chest compressions involved in CPR can put significant pressure on the fragile aortic wall, potentially leading to a tear or rupture. This highlights the delicate balance between life-saving measures and the potential risks they carry. Though studies

Follow-up outcomes after the frozen elephant trunk technique in chronic type B dissection.

OBJECTIVES: Our goal was to present our experience with a hybrid approach to the frozen elephant trunk (FET) technique for the treatment of patients with chronic type B aortic dissection. METHODS: Between January 2013 and July 2019, 86 patients underwent the FET procedure at our centre. In 20 patients, the indication was chronic type B aortic dissection with a concomitant proximal aortic lesion. We evaluated the sites of proximal and distal entries, luminal communication and originating visceral branches in the computed tomography scan data. Primary end points were hospital deaths, complicati

Clinical outcomes of aortic repair in young adult patients with ACTA2 mutations.

OBJECTIVES: Actin, alpha-2, smooth muscle, aorta (ACTA2) mutations are one of the major causes of familial thoracic aortic aneurysms and dissections. The aim of this study was to review our clinical results of young adult patients with aortic disease caused by ACTA2 mutations. METHODS: We reviewed the medical records of 251 patients (<50 years old) who underwent surgery for thoracic aortic diseases between 2004 and 2014. Among them, nine patients (3.5%) had ACTA2 mutations. Their average age was 35 years (range 22-47) and two patients (22.2%) were males. No patients fulfilled the diagnostic c

Hemodynamic effects of stent-graft introducer sheath during thoracic endovascular aortic repair.

Thoracic endovascular aortic repair (TEVAR) has become the standard treatment of a variety of aortic pathologies. The objective of this study is to evaluate the hemodynamic effects of stent-graft introducer sheath during TEVAR. Three idealized representative diseased aortas were designed: aortic aneurysm, coarctation of the aorta, and aortic dissection. Computational fluid dynamics studies were performed in the above idealized aortic geometries. An introducer sheath routinely used in the clinic was virtually placed into diseased aortas. Comparative analysis was carried out to evaluate the hemo

Long-Term Risk for Aortic Complications After Aortic Valve Replacement in Patients With Bicuspid Aortic Valve Versus Marfan Syndrome.

BACKGROUND: Bicuspid aortic valves are associated with valve dysfunction, ascending aortic aneurysm and dissection. Management of the ascending aorta at the time of aortic valve replacement (AVR) in these patients is controversial and has been extrapolated from experience with Marfan syndrome, despite the absence of comparative long-term outcome data. OBJECTIVES: This study sought to assess whether the natural history of thoracic aortopathy after AVR in patients with bicuspid aortic valve disease is substantially different from that seen in patients with Marfan syndrome. METHODS: In this ret

Hypertension drives thoracic aortic aneurysm and dissection in male, but not female, Marfan mice.

Hypertension is regarded clinically to be a primary risk factor for thoracic aortic disease, yet few studies have examined roles of sustained elevations in blood pressure in mouse models that are genetically predisposed to aortopathy. We quantified and compared effects of chronic blood pressure elevation, achieved using a high salt diet plus induced endothelial dysfunction, on the ascending aorta in age- and sex-matched adult wild-type and Fbn1C1041G/+ Marfan syndrome mice. Primary findings included that the aorta was more susceptible to disease progression in male Marfan mice than in either f

Molecular mechanisms of inherited thoracic aortic disease - from gene variant to surgical aneurysm.

Aortic dissection is a catastrophic event that has a high mortality rate. Thoracic aortic aneurysms are the clinically silent precursor that confers an increased risk of acute aortic dissection. There are several gene mutations that have been identified in key structural and regulatory proteins within the aortic wall that predispose to thoracic aneurysm formation. The most common and well characterised of these is the FBN1 gene mutation that is known to cause Marfan syndrome. Others less well-known mutations include TGF-β1 and TGF-β2 receptor mutations that cause Loeys-Dietz syndrome, Col3A1 m

Identification of regional/layer differences in failure properties and thickness as important biomechanical factors responsible for the initiation of aortic dissections.

Thoracic aortic dissections involving the ascending aorta represent one of the most dramatic and lethal emergencies in cardiovascular surgery. It is therefore critical to identify the mechanisms driving them and biomechanical analyses hold great clinical promise, since rupture/dissection occur when aortic wall strength is unable to withstand hemodynamic stresses. Although several studies have been done on the biomechanical properties of thoracic aortic aneurysms, few data are available about thoracic aortic dissections. Detailed mechanical tests with measurement of tissue thickness and failure

Diagnosis and treatment of uncomplicated type B aortic dissection.

A type B dissection involves the aorta distal to the subclavian artery, and accounts for 25-40% of aortic dissections. Approximately 75% of these are uncomplicated with no malperfusion or ischemia. Multiple consensus statements recommend thoracic endovascular aortic repair (TEVAR) as the treatment of choice for acute complicated type B aortic dissections, while uncomplicated type B dissections are traditionally treated with medical management alone, including strict blood pressure control, as open repairs have a prohibitively high morbidity of up to 31%. However, with medical treatment alone,

Morphometric Predictors of Aortic Remodeling after Frozen Elephant Trunk Repair of Type A Dissection.

BACKGROUND: We analyzed the predictors of postoperative aortic remodeling after the frozen elephant trunk technique for type A dissection. METHODS: This retrospective study cohort comprised 20 patients who underwent the frozen elephant trunk technique for type A dissection. The incidence of aortic remodeling was evaluated at 4 levels: middle of the frozen elephant trunk, distal end of the frozen elephant trunk, 10th thoracic vertebra, and proximal to the celiac artery. Several parameters of the residual dissected aorta were analyzed at these 4 levels, and the predictors of aortic remodeling w

Endovascular plugs to occlude proximal entries in chronic aortic dissection.

OBJECTIVES: Patients with expanding chronic aortic dissection and patent proximal entries are sometimes poor candidates for open surgery or TEVAR. Occlusion of proximal entries with endovascular plugs has previously been suggested in selected patients, but clinical results over time are unknown. This study analyses aortic remodelling and clinical outcome after proximal entry occlusion. METHODS: Between 2007 and 2016, 14 patients, with expanding chronic aortic dissection, considered poor candidates for standard treatment, were treated with endovascular plugs in proximal entries located in the

Endovascular stent-graft repair of aortobronchial fistulas.

BACKGROUND: Endovascular repair of the descending thoracic aorta has recently emerged as a feasible treatment option; however, little is known about its application for aortobronchial fistula (ABF). Experience with endovascular repair of the thoracic aorta and the outcome of patients with ABFs was reviewed to assess whether thoracic endovascular repair is a realistic option. METHODS: From February 2001 to May 2011, 386 patients were successfully treated with endoluminal grafts to the distal arch or descending thoracic aorta. Among them, 26 patients with ABF underwent thoracic endovascular rep

Methamphetamine induces thoracic aortic aneurysm/dissection through C/EBPβ.

AIMS: Thoracic aortic aneurysm/dissection (TAAD) is a life-threatening disease with diverse clinical manifestations. Although the association between methamphetamine (METH) and TAAD is frequently observed, the causal relationship between METH abuse and aortic aneurysm/dissection has not been established. This study was designed to determine if METH causes aortic aneurysm/dissection and delineate the underlying mechanism. METHODS AND RESULTS: A new TAAD model was developed by exposing METH to SD rats pre-treated with lysyl oxidase inhibitor β-aminopropionitrile (BAPN). Combination of METH and

Successes and challenges of using whole exome sequencing to identify novel genes underlying an inherited predisposition for thoracic aortic aneurysms and acute aortic dissections.

Thoracic aortic aneurysms involving the aortic root and/or ascending aorta can lead to acute aortic dissections. Approximately 20% of patients with thoracic aortic aneurysms and dissections (TAAD) have a family history of the disease, referred to as familial TAAD (FTAAD) that can be inherited in an autosomal dominant manner with variable expression with respect to disease presentation, age of onset and associated features. Whole exome sequencing (WES) has been used to identify causative mutations in novel genes for TAAD. The strategy used to reduce the large number of rare variants identified
