Corticosteroid pharmacokinetics in the inner ear fluids: an animal study followed by clinical application.

OBJECTIVE: Autoimmune disease (e.g., Cogan syndrome) and other inflammatory inner ear diseases may ravage the labyrinth if not treated aggressively with antiinflammatory medication. Corticosteroids are the mainstay of treatment, yet, partly because of the existence of the blood-labyrinthine barrier, the ideal drug, dose, and route of administration are currently unknown. STUDY DESIGN: In the present study, we established cochlear fluid pharmacokinetic profiles of hydrocortisone, methylprednisolone, and dexamethasone in the guinea pig following oral, intravenous, and topical (intratympanic) ad

pH regulation of the globular substance in the otoconial membrane of the guinea-pig inner ear.

Physiological and pharmacological characteristics of the globular substance, a precursor of otoconia, are not well understood. In the present study, we investigated the variations and regulation of internal pH (pHi) of the globular substance of the guinea-pig inner ear. The otoconial membrane was dissected out from the utricular macula and loaded with the pH-sensitive fluorophore, carboxyseminaphthorhodafluor-1. Dynamic changes of fluorescence were directly observed under a confocal laser scanning microscope, and pHi was calculated from dual emission ratio. In the NaCl standard solution buffer

The role of micro-noise trauma in the etiology of aging-related changes in the inner ear.

Eleven chinchillas between 1 and 2.4 years of age had the malleus/incus complex removed from one middle ear and then lived in the Washington University animal facilities for 4 years post-surgery. Each animal had one ear (termed ambient-noise) in which the conductive apparatus was intact; the other ear (termed noise-protected) had a 50-60 dB conductive hearing loss. The background sound level in the animal facility was 59 dBA with periodic brief sounds up to 102 dBA. After the 4-year experimental period, both ears were fixed, embedded in plastic and dissected for microscopic examination as flat

Molecular cloning of chick beta-tectorin, an extracellular matrix molecule of the inner ear.

The tectorial membrane is an extracellular matrix lying over the apical surface of the auditory epithelium. Immunofluorescence studies have suggested that some proteins of the avian tectorial membrane, the tectorins, may be unique to the inner ear (Killick, R., C. Malenczak, and G. P. Richardson. 1992. Hearing Res. 64:21-38). The cDNA and deduced amino acid sequences for chick beta-tectorin are presented. The cDNA encodes a protein of 36,902.6 D with a putative signal sequence, four potential N-glycosylation sites, 13 cysteines, and a hydrophobic COOH terminus. Western blots of two-dimensional

Evidence for supporting cell mitosis in response to acoustic trauma in the avian inner ear.

Acoustic overstimulation can lead to sensory cell (hair cell) loss in the auditory epithelium. Damaged hair cells in the organ of Corti (the mammalian auditory end-organ) degenerate and are replaced by non-sensory cells (supporting cells) which construct an irreversible scar. In birds, however, auditory hair cells which are damaged by acoustic trauma or ototoxic drugs may be replaced by new hair cells. As first step in determining the mechanism of hair cell regeneration, we developed an assay for cell divisions in the auditory epithelium after acoustic trauma. The results of these experiments

Identification of two types of melanocyte within the stria vascularis of the mouse inner ear.

We have distinguished two types of melanocyte within the intermediate layer of the stria vascularis in the cochlea of normally pigmented mice: light and dark intermediate cells. The light intermediate cells are present in the stria from birth and have the typical appearance of a melanocyte. They are large and dendritic with electron-lucent cytoplasm containing numerous vesicles that show tyrosinase activity, and pigment granules in various stages of development. These granules have the ultrastructural and histochemical characteristics of premelanosomes and melanosomes. The light intermediate c

Intraoperative electrocochleographic monitoring of inner ear surgery for endolymphatic hydrops. A review of cases.

The purpose of this review of cases was to determine whether or not the SP/AP amplitude ratio changes during unidirectional inner ear valved shunt surgery for the decompression of the hydropic labyrinth in Meniere's disease. A series of 62 patients underwent shunt surgery over a 20 month period. In 43 cases (43 ears), ECoG responses were successfully recorded throughout the duration of surgery. A pair of two-tailed paired t-tests were computed for SP/AP amplitude ratios acquired during intraoperative ECoG monitoring at the baseline and closing stage of surgery. The first t-test compared the me

[Magnetic resonance imaging of the inner ear and its application to cochlear implant candidates].

Magnetic resonance (MR) images of normal and abnormal inner ears obtained with a 1.5T unit without the use of a surface coil were analyzed to evaluate the effectiveness of this method in the screening of cochlear implant candidates. In a series with sixteen normal adult volunteers, 22 to 45 years in age, 100% of the cochleae, vestibules, and lateral and posterior semicircular canals were clearly demonstrated in T2 weighted images. Satisfactory images were not obtained for 22% of the superior semicircular canals, 22% of the cochlear aqueduct, and 81% of the vestibular aqueduct in axial images,

Hydrocortisone applied into the round window niche causes electrophysiological dysfunction of the inner ear.

A suspension of 2% hydrocortisone, micronized in sterile water, was instilled through a perforation in the tympanic membrane into the round window (RW) niche of 9 healthy rats once a day for 5 consecutive days. Three animals were used as controls, and were exposed to sterile water only instilled into the RW niche. Auditory brainstem response (ABR) thresholds were determined for eight frequencies: 2, 4, 6, 8, 12, 16, 20 and 31.5 kHz. Hydrocortisone caused impaired ABR thresholds in the frequency range of 12-31.5 kHz after 5 days of instillation. The impaired thresholds remained unchanged for 2

The presence and localization of receptors for atrial natriuretic peptide in the inner ear of the guinea pig.

Atrial natriuretic peptide (ANP) is a cardiac hormone known to mediate increased capillary permeability, vasodilation, and natriuresis. Since specific receptors for ANP exist in both the eye and brain, possibly playing a role in ocular and cerebrospinal fluid regulation, we postulated that ANP might also be involved in inner ear fluid dynamics. Autoradiography was used to evaluate whether and where ANP receptors are present in the inner ear. Frozen sections of the cochlea and vestibular organ from guinea pigs (n = 18) were incubated in either 125J-ANP alone or together with an excess of unlabe

Hair cell damage in the inner ear of the Guinea pig due to noise in a workshop.

Guinea pigs were exposed to noise in various workshops at a shipyard. The equivalent sound level ranged from 87 to 90 dB(A) and the exposure time was 30 days of 8 hours. The numbers of lost outer hair cells were greater than in control animals, thus establishing a relation between the number of lost hair cells and the total noise dose. A comparison between the results obtained here and results obtained in laboratory experiments shows that although many physical sound parameters may be identical, the effect on the hair cells can be quite different. In particular, the total noise dose is not a p

Overview of mechanical damage to the inner ear: noise as a tool to probe cochlear function.

The majority of experiments causing mechanical damage to the cochlea involve the use of sound pressure waves to cause overstimulation. This presentation is an overview of the research during the past years on the structural damage produced by noise. The effect of noise on the cochlea depends on the type of noise exposure-impulse or continuous. Experiments have been conducted to determine the effect of increasing intensity, the effect of increasing duration, and the effect of equal energy presented over varying periods of time. The initial mechanism of damage, the progression of damage over tim

Experimental cytomegalovirus infection and the developing mouse inner ear: in vivo and in vitro studies.

Mouse cytomegalovirus causes perilabyrinthitis in the cochlea following intracranial inoculation of the newborn mouse. In spite of infection of cranial nerve ganglion cells and Schwann cells the organ of Corti achieves normal adult morphology including efferent nerve endings. No involvement of neuroaxons is seen. In comparison human cytomegalovirus infection causes endolabyrinthitis without involving periotic connective tissues or nerves even in the presence of meningoencephalitis. The lack of epithelial infection and susceptibility of periotic connective tissues to mouse cytomegalovirus was c
