Classical and real-time neuronavigation in pediatric neurosurgery.

INTRODUCTION: Neuronavigation has become a cornerstone of neurosurgery. Navigation systems are categorized into two main groups: those based on preoperative imaging and those based on real-time intraoperative acquired images. OBJECTIVES: The preoperative imaging systems, either computed tomography (CT)- or magnetic resonance imaging (MRI)-based, are straight-forward systems that are routinely used in most institutions. Image accuracy, however, decreases secondary to brain and lesion shifts that occur during surgery. Intraoperative, real-time navigation systems overcome anatomical shifts by up

The 'Mickey Mouse' sign and the diagnosis of anencephaly in early pregnancy.

OBJECTIVES: To assess the sonographic screening for anencephaly in the first trimester in a low-risk obstetric population. METHODS: Since 1994, 5388 women attended our clinic for a first-trimester scan (11-14 weeks of gestation) and screening for structural and chromosomal abnormalities. The patients underwent transabdominal scanning, and transvaginal scanning if necessary. RESULTS: The ultrasonographic appearances of anencephaly in the first trimester are different from the familiar second-trimester signs. The cerebral hemispheres are present and exposed to the surrounding amniotic fluid. T

Early neonatal intensive care unit therapy improves predictive power for the outcomes of ventilated extremely low birth weight infants.

OBJECTIVE: To assess the predictive value of early therapy for ventilated extremely low birth weight (ELBW) infants beyond information available at delivery. STUDY DESIGN: Prospective, single-center cohort analysis of 177 ventilated ELBW infants. We collected information known at delivery (gestational age, birth weight, singleton, sex, antenatal steroids) and additional information while infants were mechanically ventilated (head ultrasound scanning, clinician intuitions of death before discharge). An adverse outcome was defined as mortality or Bayley Mental Developmental Index or Psychomotor

Familial hemiplegic migraine.

Familial hemiplegic migraine (FHM) is a rare, dominantly inherited subtype of migraine with aura, where hemiplegia occurs during the aura phase. Mutation screening of families with FHM has revealed a range of different mutations. The mutated FHM genes code for ion transport proteins. Animal and cellular studies have associated the mutated FHM genes with disturbed ion homeostasis, altered cellular excitability and altered neurotransmitter release. Abnormal cortical excitability due to dysfunctional ion-channels might facilitate cortical spreading depression (CSD) and thereby migraine aura and m

[Sonography of the parenchyma in Parkinson's disease].

Transcranial sonography (TCS) of the brain parenchyma is a non-invasive and easily applicable neuroimaging technique which is used as a diagnostic tool in Parkinson's disease. Up to 90% of patients with idiopathic Parkinson's disease but only 10-15% of the healthy population show an abnormal echogenicity (hyperechogenicity) of the substantia nigra (SN). TCS has been demonstrated to be a useful tool in the differential diagnosis of patients with essential tremor or atypical parkinsonian syndromes, including the parkinsonian variant of multiple system atrophy (MSA-P) and progressive supranuclear

Cerebral emboli in the genesis of dementia.

Alzheimer's disease (AD) and vascular dementia (VaD) are the two commonest forms of dementia, with evidence of a considerable overlap between them. Vascular risk factors are important in the genesis of both dementias but underlying mechanisms need further exploration. In this respect, asymptomatic spontaneous cerebral emboli (SCE) may be of interest. Such emboli are common in patients at risk of stroke, for example those with symptomatic or severe carotid artery disease, and predict the future risk of strokes. Here we summarise our research which shows that asymptomatic SCE may be involved in

Left ventricular non-compaction: clinical features and cardiovascular magnetic resonance imaging.

BACKGROUND: It is apparent that despite lack of family history, patients with the morphological characteristics of left ventricular non-compaction develop arrhythmias, thrombo-embolism and left ventricular dysfunction. METHODS: Forty two patients, aged 48.7 +/- 2.3 yrs (mean +/- SEM) underwent cardiovascular magnetic resonance (CMR) for the quantification of left ventricular volumes and extent of non-compacted (NC) myocardium. The latter was quantified using planimetry on the two-chamber long axis LV view (NC area). The patients included those referred specifically for CMR to investigate susp

Integrating neurocritical care approaches into neonatology: should all infants be treated equitably?

To improve the neurologic outcomes for infants with brain injury, neonatal providers are increasingly implementing neurocritical care approaches into clinical practice. Term infants with brain injury have been principal beneficiaries of neurologically-integrated care models to date, as evidenced by the widespread adoption of therapeutic hypothermia protocols for hypoxic-ischemic encephalopathy. Innovative therapeutic and diagnostic support for very low birth weight infants with brain injury has lagged behind. Given that concern for significant future neurodevelopmental impairment can lead to d

Semiquantitative software SPECT analysis in aneurysmal subarachnoid hemorrhage-related vasospasm.

BACKGROUND: Cerebral vasospasm is a major cause of morbidity and mortality among patients after subarachnoid hemorrhage (SAH) from ruptured intracranial aneurysms. Patients are often monitored using transcranial Doppler (TCD) ultrasound and single photon emission computed tomography (SPECT) techniques. Historically SPECT has been evaluated qualitatively, though quantitative analysis software is now used to evaluate other cerebral pathologies. OBJECTIVE: Our aim was to evaluate the utility of semiquantitative SPECT using Hermes brain registration and analysis software (BRASS) relative to TCD u

Outcome of cerebellar injury in very low birth-weight infants: 6 case reports.

Cerebellum injury is a severe and underestimated complication in very low birth-weight infants. Six cases of extreme premature babies are reported: 3 of them died during the neonatal period; the other 3 survived with severe neurological disabilities. Microcephaly, strabismus, and severe developmental delay without major motor handicap are very peculiar for the preterm in the long-term outcome. Easy diagnosis is possible by specific ultrasound windows and early diagnosis is required regarding the possible severe prognosis and to organize precisely the follow-up.

Use of umbilical-cerebral Doppler ratios in predicting fetal growth restriction in near-term fetuses.

OBJECTIVE: To compare the sensitivity and specificity of different umbilical-cerebral ratios in the prediction and detection of fetal growth restriction in near-term fetuses when the umbilical arterial waveform is within normal. METHODS: A prospective cross-sectional observational study was carried out recruiting consecutive singleton pregnancies with clinically suspected fetal growth restriction after 34 weeks gestation. The umbilical-cerebral ratios were then calculated from the S/D, RI and PI values and correlated with immediate perinatal outcome. RESULTS: A total of 187 patients were rec

Ultrasound screening: Status of markers and efficacy of screening for structural abnormalities.

Aneuploidy is a major cause of perinatal morbidity and mortality and can have a significant impact on expecting parents and their families. With early screening and diagnosis it is important to be able to educate parents regarding the potential impact of the diagnosis. This knowledge allows parents the opportunity to consider management options early in the pregnancy, permitting more time to mentally and emotionally prepare both for the course of the pregnancy, and after the birth of the child should the pregnancy continue. Prenatal screening provides pregnant women a non-invasive risk assessm

A Prognostic Neonatal Neuroimaging Scale for Symptomatic Congenital Cytomegalovirus Infection.

BACKGROUND: Congenital cytomegalovirus (cCMV) can cause brain inflammation/destruction and teratogenic effects. The only validated neuroimaging prognostic categorization for symptomatic cCMV available is based on destructive lesions seen on computed tomography (CT). OBJECTIVE: The aim of this study was to establish the predictive ability of a comprehensive neonatal neuroimaging scale in symptomatic cCMV. METHODS: Twenty-six infants were studied by neonatal cranial ultrasound scans (US; n = 25), CT (n = 11) and magnetic resonance imaging (MRI; n = 9). A previously validated neuroimaging scale

Value of cranial ultrasound and magnetic resonance imaging in predicting neurodevelopmental outcome in preterm infants.

In this prospective study, cranial ultrasound was performed to detect periventricular-intraventricular hemorrhage and periventricular leukomalacia in 33 preterm infants of less than 32 weeks' gestation. At 44 weeks postmenstrual age magnetic resonance imaging was performed to detect the stage of myelination. Neurodevelopmental outcome was assessed at 3 years of age in 31 children (2 children died in the first year of life). Significant correlations were found between neurodevelopmental outcome and ultrasound findings (chi 2 = 32.8; P less than .0001) and stage of myelination (chi 2 = 20.5; P l

Hemodynamic and electrophysiological evaluation following extracranial/intracranial bypass surgery.

We examined 50 patients with an extracranial/intracranial bypass using transcranial Doppler blood flow mapping, somatosensory evoked potentials, and computer-assisted EEG analysis. The investigation was supplemented by temporary compression of the extracranial supplying vessel. Transcranial Doppler sonography revealed in addition to 4 patients with nonpatent anastomoses, a bypass-mediated retrograde flow in the ipsilateral middle cerebral artery beyond a depth of 40 mm in the majority of patients. In 9 patients, retrograde flow could be demonstrated in the whole proximal (M1) MCA segment. The

A possible genotype-phenotype correlation in Ashkenazi-Jewish individuals with Aicardi-Goutières syndrome associated with SAMHD1 mutation.

Aicardi-Goutières syndrome is a genetic neurodegenerative disorder with clinical symptoms mimicking a congenital viral infection. Mutations in 6 genes are known to cause the disease: 3 prime repair exonuclease1, ribonucleases H2A, B, and C, SAM domain and HD domain 1, and most recently ADAR1. HD domain 1 mutations were previously reported in the Ashkenazi-Jewish community. We report an additional patient of Ashkenazi-Jewish descent and review the other 3 cases affected with Aicardi-Goutières syndrome due to SAM domain and HD domain 1 (SAMHD1) mutations described in Israel. We propose that ther
