Physical examination of the athlete's elbow.

Elbow injury is encountered less frequently than are other joint conditions. The bony architecture, muscle, ligament, and nerve anatomy are complex, and the forces leading to injury in the athlete's elbow are unique. Appreciating the pathomechanics leading to injury and a detailed knowledge of elbow anatomy are the foundation for conducting a directed history and physical examination that achieves an accurate diagnosis. Recent advances in physical examination have improved our ability to accurately diagnose and treat athletic elbow disorders. This article reviews general and focused physical e

Abnormal Patterns of Biceps and Triceps Co-Contraction Following Elbow Surgery May Result in Elbow Stiffness.

OBJECTIVES: This study examines the pattern of muscular contraction and the intensity of this contraction of the biceps and triceps following elbow surgery. METHODS: We performed a prospective electromyographic study of 16 patients undergoing 19 surgical procedures on the elbow joint. We measured the resting EMG signal intensity of the biceps and triceps of the operated and the normal sides at 90 degrees. We then calculated the peak EMG signal intensity during passive elbow flexion and extension of the operated side. RESULTS: Seventeen of 19 elbows (89%) displayed a co-contraction pattern of…

Prior to Repair Functional Deficits in Above- and Below-Elbow Ulnar Nerve Injury.

PURPOSE: Clinical deficits might vary, depending on whether an ulnar nerve lesion is above or below the elbow. Lack of strength and clawing are common manifestations of ulnar nerve paralysis. However, the magnitude of strength deficit relating to different pinch patterns and the rate and range of proximal interphalangeal extension deficits are poorly described. METHODS: I prospectively evaluated 14 patients with above-elbow and 16 with below-elbow unrepaired ulnar nerve injuries. The completeness of flexion of the ring and little fingers was tested at the metacarpophalangeal and distal interp

Glenohumeral arthrodesis improves elbow flexion in patients with adult brachial plexus injuries.

Restoring elbow flexion is a focus in treating brachial plexus injuries (BPI), aiming for anti-gravity strength (mBMRC grade ≥3). However, patients with flail or subluxated glenohumeral (GH) joints may struggle to achieve adequate strength due to energy lost stabilizing the GH joint. This study evaluates whether glenohumeral arthrodesis (GHA) can improve elbow flexion in BPI patients with prior failed elbow reconstruction and identifies preoperative factors affecting outcomes. A retrospective review of adult BPI patients with prior elbow flexion reconstruction who later underwent GHA was condu

Organic electrolyte gated field effect transistor for detecting alpha synuclein longitudinally in Parkinsonism mouse model.

BACKGROUND: Enthesopathy is an evolving area for applied clinical research. MRI is the gold standard in the diagnosis of elbow joint pathology, but recent reports indicate that ultrasound imaging is more sensitive and accurate than MRI in detecting enthesopathy of the heels and knees. Too many patients are under-diagnosed and/or misdiagnosed because the early pathological changes of enthesitis in the different types of seronegative arthropathies are not detected. OBJECTIVES: This study was undertaken to describe the ultrasound features of elbow enthesitis in patients with seronegative arthrop…

Canine elbow dysplasia. The early radiographic detection of fragmentation of the coronoid process.

Ununited anconeal process, osteochondrosis, and fragmentation of the coronoid process (FCP), separately or together, comprise canine elbow dysplasia. Radiographic detection of ununited anconeal process or osteochondrosis is common, whereas identification of an FCP is often inferred by finding joint incongruity and secondary changes. Early identification of FCP can be improved by evaluating the elbow joint for subtle changes visible early in the course of this disease. Proper radiographic technique and positioning are essential in optimizing identification of the changes associated with mild or…

[Heterotopic ossification as a complication of fracture-dislocations of the forearm in children].

Under observation there were 59 patients ranging in age from 4 to 14 years with posttraumatic ossifications in the elbow joint areas. All the patients were given conservative treatment, 23 patients needed operative interventions on the elbow joint. Long-term results were studied at the period from 6 months to 8 years. Good and satisfactory functional results were noted in 96.8% of the patients after the conservative treatment. After the operative treatment good and satisfactory functional and anatomical results were obtained in 90% of the patients.

Debridement arthroplasty for advanced primary osteoarthritis of the elbow. Results of a new technique used for 29 elbows.

We report the technique and results of a new method of debridement arthroplasty for advanced primary osteoarthritis of the elbow. Triceps and the periosteum of the olecranon are reflected towards the ulnar side and the joint is opened by dividing the radial collateral ligament. Osteophytes are removed, the olecranon and coronoid fossae are deepened and the fibrosed anterior joint capsule is excised. The degenerative changes are always more advanced on the radial side, with erosion of the capitellum, and it is usually necessary to remodel the head of the radius. In 29 elbows reviewed at a mean

Chronic Structural Adaptations of the Shoulder and Elbow Are Correlated in Professional Baseball Pitchers.

BACKGROUND: Pitchers with deficits in total shoulder rotation range of motion (ROM) are 2.6 times more likely to experience an elbow injury. Despite the effects of shoulder ROM on elbow injury, it is currently unclear whether specific tissue adaptations of the shoulder relate to the tissue adaptations of the elbow in baseball pitchers. PURPOSE/HYPOTHESIS: The purpose was to evaluate the relationship between chronic structural adaptations of the shoulder (humeral retroversion [HR], posterior capsule thickness [PCT], infraspinatus/teres minor pennation angle, and muscle thickness) and chronic s

Septic arthritis of the elbow with Streptococcus pneumoniae in a 9-month-old girl.

A 9-month-old girl presented with a swollen, erythematous and hot left elbow, which was tender on palpation. Physical examination was unremarkable except temperature of 38.4°C and restriction of elbow movement. Blood tests showed an elevated C reactive protein of 116 mg/L. Plain radiograph and ultrasound of the elbow showed no pathological findings. MRI revealed an intra-articular effusion and associated myositis, but did not demonstrate intra-articular synovitis. Arthroscopic lavage of the joint yielded purulent fluid and gross synovitis involving the entire joint. Streptococcus pneumoniae wa

Tuberculosis of the Elbow in a Chronic Kidney Disease Patient with Arteriovenous Fistula: A Case Report.

INTRODUCTION: Tuberculosis (TB) involving the elbow joint is an uncommon form of osteoarticular infection and often presents with non-specific symptoms, leading to delayed diagnosis and treatment. The condition may mimic other chronic inflammatory or infective arthropathies, posing significant diagnostic challenges. In patients with chronic kidney disease (CKD) undergoing hemodialysis, the presence of an arteriovenous (AV) fistula in the affected limb further complicates both clinical evaluation and surgical management. Reports describing TB of the elbow in the setting of CKD with an ipsilater…

Restoration of elbow flexion in arthrogryposis multiplex congenita.

In 15 patients with arthrogryposis multiplex congenita, 18 operations were performed to improve elbow function. These operations were for seven cases of pectoralis transfer, seven cases of anterior transfer of triceps, two cases of posterior elbow release, and two cases of Steindler flexoroplasty. Results were evaluated on two simple criteria: single-hand feeding and improved elbow joint motion. These criteria showed that the pectoralis transfer was the most effective procedure.

Current concepts in the management of "Terrible Triad" injuries of the elbow.

Terrible triad injuries of the elbow are complex injuries which can result in long term complications and significant disability. They must be identified correctly, and managed appropriately in order to maximise functional outcomes. A clear understanding of the bony and ligamentous anatomy is essential to plan appropriate surgical reconstruction to provide elbow stability. Urgent reduction of the elbow, followed by 3-dimensional imaging and surgical repair or replacement of the injured structures is the mainstay of treatment in the majority of cases. This review presents a summary of the relev

Long-term functional outcomes following paediatric traumatic elbow dislocation, a clinical retrospective cohort study.

BACKGROUND: Paediatric traumatic elbow dislocation occurs in 6 per 100,000 children per year and if not treated promptly can result in a poor outcome. Despite this, the long-term clinical and functional outcome of these injuries has not been well described using modern patient-reported outcome tools. The aim of our study was present the outcome of these injuries in the long term. METHODS: Twenty children with an acute traumatic elbow dislocation who presented between February 2007 to February 2016 were included in our study. Patient demographics, management and complications were recorded fro

Comparison of Surgical Techniques for Fixation of Terrible Triad Injuries of the Elbow: A Meta-Analysis.

Treatment of the terrible triad injury of the elbow remains a difficult problem. There are several ways to treat the radial head and coronoid fractures; controversy still exists regarding the best way to treat each. The purpose of this meta-analysis was to compare the clinical outcomes of radial head and coronoid fractures treated using current protocols. No differences in functional outcomes were found between the different surgical techniques. There is no superior current protocol for treating these injuries. Surgical treatment should be dictated by fracture type and surgeon experience. [Ort

[Treatment of complex elbow injuries with the hinged fixation device DJD II. Technique and results].

Fractures and luxations with persistent instability of the elbow are severe injuries often followed by arthrofibrosis. The need for prolonged immobilization results in shortage of the ligaments and shrinking of the capsule. Treatment with dynamic hinged external fixation offers an option of protected early mobilization providing better results and avoiding stiffness and instability. In a retrospective study 22 patients with complex injuries of the elbow were evaluated with respect to the range of motion and subjective parameters (DASH score, DASH disabilities of the arm, shoulder and hand).

MR imaging findings of haemophilic arthropathy of the elbow in children.

Haemophilic arthropathy of the elbow is a rare cause of elbow pain in children and adolescents. The purpose of this study is to determine the MR appearance of the spectrum of lesions found in haemophilic arthropathy of the elbow at initial MR imaging. It is important to be aware of the early changes in this entity, since early diagnosis and treatment of the disease may prevent progressive joint destruction.

The effect of the anatomy of the distal humerus and proximal ulna on the positioning of the components in total elbow arthroplasty.

Determining and accurately restoring the flexion-extension axis of the elbow is essential for functional recovery after total elbow arthroplasty (TEA). We evaluated the effect of morphological features of the elbow on variations of alignment of the components at TEA. Morphological and positioning variables were compared by systematic CT scans of 22 elbows in 21 patients after TEA. There were five men and 16 women, and the mean age was 63 years (38 to 80). The mean follow-up was 22 months (11 to 44). The anterior offset and version of the humeral components were significantly affected by the an
