Elephant endotheliotropic herpesvirus (EEHV) causes lethal hemorrhagic disease (HD) in Asian and African elephants. Although rapid detection of viremia and supportive treatments may improve survival rates, an effective vaccine would mitigate the devastating effects of this virus. In elephants, chronic infection with EEHV leads to adaptive immunity against glycoproteins gB and gH/gL, the core entry machinery for most herpesviruses. We previously evaluated two EEHV gB vaccines in mice but not a gH/gL vaccine. Here, we found that inoculation of mice with an adjuvanted EEHV gH/gL subunit vaccine i…

Thoracic endovascular aortic repair (TEVAR) and the frozen elephant trunk (FET) are well-established treatment options for aortic dissection (AD). Inadvertent misdeployment of a TEVAR stent graft or FET into the false lumen (FL) can lead to serious complications. We report two cases of FL misdeployment. First, a 68-year-old female with acute Stanford type B AD underwent TEVAR; follow-up computed tomography angiography (CTA) revealed stent-graft misdeployment into the FL. Second, a 44-year-old female with Marfan syndrome, who had undergone supracoronary ascending aortic grafting for type A AD a

CASE REPORT: a 77-year-old man presented to our Emergency Department for worsening dyspnea. A chest CT scan showed a type A aortic dissection with a dilatation of the ascending and descending aorta. After evaluating the case with cardiac surgeons, it was decided to perform an aortic arch replacement through the frozen elephant trunk technique. Three months after cardiac surgery, a TC scan showed a residual 58 mm dissecting aneurysm of the descending thoracic aorta and a collapse of the true lumen so we decided to perform an endovascular treatment of the thoracoabdominal aorta according to the

RATIONALE: Acute aortic dissection of Kommerell's diverticulum in the right aortic arch is extremely rare. There are several different procedures for the disease. With advances in endovascular treatment, hybrid surgical and endovascular management may provide a treatment of choice for this kind of disease.
PATIENT CONCERNS: A 43-year-old man was admitted to the hospital with intermittent pain of left arm, chest, and back.
DIAGNOSES: Computed tomography demonstrated an aberrant left subclavian artery originating from Kommerell's diverticulum in the right aortic arch, acute aortic dissection o

INTRODUCTION: Reconstruction of chronic type B dissection and thoracoabdominal aortic aneurysm (TAAA) remaining after the emergency reconstruction of the ascending thoracic aorta and aortic arch for acute type A dissection represents one of the major surgical challenges. Complications of chronic type B dissection are aneurysmal formation and rupture of an aortic aneurysm with a high mortality rate. We presented a case of visceral hybrid reconstruction of TAAA secondary to chronic dissection type B after the Bentall procedure with the 'elephant trunk' technique due to acute type A aortic dissec

BACKGROUND: Narrowed true lumen and patent false lumen through to the terminal aorta is a high-risk condition for malperfusion syndromes (MS) in acute type-A aortic dissection. It is important to ascertain how the true and false lumens behave after surgery.
PATIENTS AND METHODS: We retrospectively investigated 45 patients with this pathology. The true lumen sizes at the narrowest levels above and below the superior mesenteric artery were followed by computed tomography after surgery (0-36 months).
RESULTS: Thirty-seven MS were seen in 23 patients. Hospital mortality was 8.9%. The narrowed tr

OBJECTIVE: To explore the feasibility of open single-branched stent graft placement for simplified total arch replacement in acute Stanford type A aortic dissection.
METHODS: Between June 2008 and September 2009, 23 patients with acute Stanford type A aortic dissection underwent total arch replacement plus open single-branched stent graft placement. When core nasopharyngeal temperature dropped to 68 H, the perfusion to lower body was discontinued. Left common carotid artery was transected at its ostium and its proximal stump closed. The arch was transected at a predetermined line between inno

Although the structural integrity of the hypopharynx can be readily restored in individuals who have undergone laryngopharyngoesophagectomy by transferring a segment of the jejunum by a microsurgical technique, rehabilitation is often hindered by voice loss. The outcomes achievable with conventional approaches of artificial voice box reconstruction have been, generally speaking, unsatisfactory. A new reconstructive technique was devised to create a connection between the tracheal stump and the neo-hypopharynx to shunt airflow for phonation. The technique consisted of modifying one of two jejun

INTRODUCTION: Arch or proximal descending aortic aneurysms complicated by Stanford type B aortic dissection (TBAD) may require extensive open repair with an open proximal anastomosis. Although a staged modified elephant trunk strategy is a reasonable option, the need to undergo two major operations is burdensome. The frozen elephant trunk (FET) technique enables concomitant treatment of the descending aorta after arch replacement; however, persistent retrograde false-lumen perfusion may result in incomplete aneurysm exclusion and inadequate aortic remodeling. We report a single-stage approach

INTRODUCTION: The night-time economy comprises various sectors, including hospitality, transportation and entertainment, which generate substantial revenues and contribute to employment opportunities. Furthermore, the night-time economy provides spaces for leisure activities, cultural expression and social interaction. On-trade alcohol premises (places where consumers can buy and consume alcohol such as bars, pubs, clubs and restaurants) are a significant component of this night-time economy, functioning as focal points for socialising, entertainment and cultural events. However, when on-trade

We report two cases of chronic aortic dissection (CAD) that were successfully treated with total arch replacement (TAR) using frozen elephant trunk (FET). Case 1 was a 64-year-old female with incidentally detected CAD. Surgery was planned due to progressive aortic enlargement. The primary entry was in the distal arch, and TAR with FET was performed. Case 2 was a 53-year-old male with an abnormal chest radiography, later diagnosed as CAD. He also underwent TAR with FET. Both patients were discharged without complications. Postoperative computed tomography at six months showed true lumen expansi

Introduction: The management of aortic dissection has evolved significantly over the decades, with the frozen elephant trunk (FET) procedure emerging as a key technique for treating complex aortic pathologies. Recent practices involve deploying the FET prosthesis more proximally in the aorta (Zone 0) to reduce complications, leading to questions about its impact on long-term aortic remodeling compared to traditional Zone 2 deployment. Methods: This retrospective analysis utilized 3D segmentation software to assess the volumetric changes in aortic remodeling after acute Type A aortic dissection

BACKGROUND: Total arch replacement using elephant trunk (ET) has been accepted as a standard technique for thoracic aortic dissection. However, there are few complications related to the ET, such as kinking of the ET, paraplegia, splitting of the anastomosis, and thromboembolic complications. We report a successful thoracic endovascular aortic repair (TEVAR) in a patient with ET entrapment in a new isolated dissecting aortic aneurysm.
CASE PRESENTATION: A 50-year-old woman who underwent total arch replacement (TAR) with the ET technique 6 years ago was admitted with refractory hypertension an

BACKGROUND: Type Ia endoleaks are common after thoracic endovascular aortic repair (TEVAR). However, the repair of type Ia endoleaks involving the distal arch is challenging because of the presence of the interventional endografts, potential damage to the aortic arch vessels, and the location and size of the aneurysmal body. We retrospectively reviewed our experience of the surgical treatment of type Ia endoleaks with distal arch involvement using left subclavian artery (LSCA)-left common carotid artery (LCCA) transposition with a stented elephant trunk.
METHODS: Sixteen patients (male = 16;

The chimney technique can be combined with thoracic endovascular aortic repair (TEVAR) to both obtain an appropriate landing zone and maintain blood flow of the arch vessels. However, surgical repair becomes more complicated if retrograde type A aortic dissection occurs after TEVAR with the chimney technique. We herein report a case involving a 73-year-old woman who developed a retrograde ascending dissection 3 months after TEVAR for acute type B aortic dissection. To ensure an adequate proximal sealing distance, the proximal edge of the stent graft was located at the zone 2 level and an addit
