Beyond CHA2DS2-VASc in atrial fibrillation: the atrium and the risk of stroke.

Pathology affecting the atria have a significant impact on the occurrence of arrhythmias and the risk of stroke. The causal relationship between atrial fibrillation (AF) and ischaemic stroke has been challenged by the recent uncovering of the lack of temporal association between thrombo-embolic cerebral events and paroxysmal AF or tachycardia. General conditions, such as the one considered in the definition of the CHA2DS2-VASc score, or specific atrial pathology (also independently occurring), could predispose to cerebral embolism.

Arterial hyperintensity on fast fluid-attenuated inversion recovery images: a subtle finding for hyperacute stroke undetected by diffusion-weighted MR imaging.

BACKGROUND AND PURPOSE: Diffusion-weighted MR imaging is generally acknowledged to be more sensitive in detecting acute stroke than is conventional MR imaging. Our purpose in the present study was to evaluate the utility of fast fluid-attenuated inversion recovery (FLAIR) MR imaging compared with that of diffusion-weighted MR imaging for the diagnosis of hyperacute stroke. METHODS: We reviewed patient records and cerebral MR images from all patients in a 13-month period from whom diffusion-weighted and fast-FLAIR imaging were obtained within 6 hours after symptom onset (n = 11). Special atten

Early anticoagulation after large cerebral embolic infarction: a safety study.

OBJECTIVE: To assess whether hemorrhage after early anticoagulation in nonseptic embolic infarction is related to clinical severity and infa … TT (p < 0.01). CONCLUSIONS: Infarction size and clinical severity in alert patients with nonseptic embolic stroke carries no additional bleeding complications after early anticoagulation. If anticoagulants are deemed necessary, treatment delay seems unjustified. However, rigorous monitoring of the aPTT is strongly advised to keep the level at 1.5 to 2 times con …

Atrial septal defect closure in a young patient presenting with recurrent cryptogenic stroke: a case report.

BACKGROUND: Having an inter-atrial shunt in the form of a patent foramen ovale or atrial septal defect increases the risk of developing cryptogenic stroke. Prompt action is required in order to prevent stroke recurrence. The source of embolization may not be clear on stroke workup. CASE PRESENTATION: A young female acutely presented with recurrent embolizations to the eye and brain. She was found to have an atrial septal defect. No clear intra-cardiac source of embolization was detected on workup including trans-esophageal echocardiography. Given the options between surgical versus device clo

Mitral Valve Malignant Glomus Tumor Presenting With Multiple Embolic Events.

CASE SUMMARY: We report a case of a malignant glomus tumor involving the mitral valve, resulting in repeat presentations with arterial embolic events including non-ST-segment elevation myocardial infarction and stroke. This is the third existing case report of an intracardiac malignant glomus tumor and highlights the need for a multidisciplinary approach to the diagnosis and management of cardiac tumors. TAKE-HOME MESSAGES: Glomus tumors are extremely rare, and even less commonly malignant. They can involve cardiac structures and pose risk for embolic events. Patients with cardiac glomus tumo

Factor Xa inhibitors vs warfarin for preventing stroke and thromboembolism in patients with atrial fibrillation.

CLINICAL QUESTION: Is treatment with factor Xa inhibitors associated with better efficacy and safety compared with the vitamin K antagonist warfarin for preventing strokes or other systemic embolic events in patients with atrial fibrillation? BOTTOM LINE: Compared with warfarin, factor Xa inhibitors are associated with a lower risk of stroke and other systemic embolic events in patients with atrial fibrillation. Factor Xa inhibitors were associated with lower rates of intracranial hemorrhage and mortality compared with warfarin. Factor Xa inhibitors were associated with a reduction in major b

Acute basilar artery occlusion in a patient with left subclavian artery occlusion due to first rib anomaly: case report.

A previously healthy 22-year-old man presented with thoracic outlet syndrome manifesting as Raynaud's phenomenon in the left hand and embolic occlusion of the basilar artery. Three-dimensional computed tomography angiography showed that the left subclavian artery was occluded as it passed over the abnormal first rib. Retrograde propagation of the thrombus from the site of arterial occlusion and/or reflux of embolic material was suspected. Medical therapy was started. The patient underwent resection of the anomalous rib. Postoperative angiography demonstrated that the subclavian artery was reca

Mitral valve thrombus, embolic events, carotid artery stenosis and patent foramen ovale.

Patent foramen ovale (PFO) is associated with high prevalence of stroke and systemic embolisation. A 53-year-old man had mitral valve thrombus and PFO diagnosed by echocardiography, in addition to carotid artery stenosis and embolic events including transient ischemia attack, retinal artery occlusion and left kidney infarct. Surgical removal of the mitral valve thrombus and concomitant coronary artery bypass were performed under cardiopulmonary bypass. We believe this is the sole reported case of mitral valve thrombus associated with a PFO. Due to their embolic potential, concomitant PFOs shou

Emergent treatment of iatrogenic dissection of the internal carotid artery with the Palmaz-Schatz stent--case report.

A 67-year-old male presented with sudden onset of aphasia and right hemiparesis on January 27, 1995. Iatrogenic dissection of the left internal carotid artery occurred during attempted local thrombolytic therapy for embolic occlusion of the middle cerebral artery. His neurological condition worsened. Following an unsuccessful angioplasty for the dissection, a Palmaz-Schatz stent was deployed over the dissection. Local thrombolytic therapy was then successfully completed. Anticoagulation and antiplatelet medications were given to prevent further embolic stroke. Follow-up angiography at 2 weeks …

Mechanisms of acute carotid stroke.

In a three-year study, the clinical course and results of intracranial angiography were compared in patients having an acute stroke in the carotid artery territory combined with angiographic abnormalities indicating severe extracranial carotid stenosis or occlusion. Two major mechanisms of stroke were delineated. In one group, the angiographic intracranial abnormalities strongly suggested the presence of embolism in the cerebral vessels supplied by the stenotic or occluded carotid artery; many of these patients had no obvious transient ischemic attacks prior to their stroke and experienced a m

Clinical features of meningovascular syphilis.

Meningovascular syphilis presents as a subacute encephalitic syndrome with superimposed cerebrovascular events that may evolve gradually. This picture differs from the more common cases of thrombotic or embolic stroke. Optimal treatment is uncertain, and requires monitoring of spinal fluid abnormalities until they return to normal.

Recurrent events after percutaneous closure of patent foramen ovale.

OBJECTIVES: To determine the incidence of symptomatic recurrent embolic events after patent foramen ovale (PFO) closure and potential causes for these events. BACKGROUND: It is well-known that cerebral or other embolic events may occur after PFO closure. METHODS: This is a retrospective analysis of consecutive patients who underwent PFO closure for secondary prevention of embolic events at a single institution. RESULTS: 1,930 patients (mean age of 50 ± 13.3 years) underwent transcatheter PFO closure. Complete closure occurred in 92%. The annual recurrence rate of embolic events before PFO c

Role of Cerebral Embolic Protection Devices in Patients Undergoing Transcatheter Aortic Valve Replacement: An Updated Meta-Analysis.

BACKGROUND: Cerebral embolic protection devices (CEPD) capture embolic material in an attempt to reduce ischemic brain injury during transcatheter aortic valve replacement. Prior reports have indicated mixed results regarding the benefits of these devices. With new data emerging, we performed an updated meta-analysis examining the effect of CEPD during transcatheter aortic valve replacement on various clinical, neurological, and safety parameters. METHODS AND RESULTS: A comprehensive review of electronic databases was performed comparing CEPD and no-CEPD in transcatheter aortic valve replacem

Ignored Role of Paroxysmal Atrial Fibrillation in the Pathophysiology of Cryptogenic Stroke in Patients with Patent Foramen Ovale and Atrial Septal Aneurysm.

The association between cryptogenic stroke (CS) and patent foramen ovale (PFO) with or without atrial septal aneurysm (ASA) has been a debate for decades in terms of pathophysiologic processes and clinical courses. This issue has become more interesting and complex, because of the concerns associating the CS with so-called normal variant pathologies of interatrial septum, namely ASA and PFO. While there is an anatomical pathology in the interatrial septum, namely PFO and ASA, the embolic source of stroke is not clearly defined. Moreover, in patients with PFO and CS, the risk of recurrent strok

Reinitiation of Anticoagulation After Surgical Evacuation of Subdural Hematomas.

BACKGROUND: Chronic subdural hematoma (cSDH) is an increasingly common condition due to the growing use of anticoagulation. Currently, there remains a lack of evidence to guide the optimal timing of anticoagulant reinitiation for stroke prevention in atrial fibrillation after cSDH evacuation. We aimed to better understand the perceived risks of hemorrhagic and embolic complications along with current practice patterns on restarting anticoagulation after surgical evacuation of cSDH. METHODS: We conducted a survey of Canadian neurosurgeons and stroke neurologists using a novel self-administered

Carotid Disease and Management.

Carotid artery stenosis is a leading cause of stroke, and 25% of patients experience a recurrent stroke within 5 years. Early detection and treatment are important to reduce the risk of stroke. Optimal medical therapy should be ensured among these patients regardless of symptom status. Carotid artery revascularization with carotid endarterectomy or carotid artery stenting (CAS) should be considered among patients with symptomatic carotid stenosis, or among patients with severe asymptomatic carotid artery stenosis. Refined procedural techniques, improvement in stent design, and use of embolic p

Pathological Findings of Embolus Retrieved by Mechanical Thrombectomy in Cerebral Embolism with Libman-Sacks Endocarditis.

Libman-Sacks endocarditis is an important cause of embolic stroke in systemic lupus erythematosus, although the detailed pathogenesis of stroke remains unclear. We herein report two cases of stroke with Libman-Sacks endocarditis in which the emboli were retrieved by mechanical thrombectomy. The embolus consisted of eosinophilic homogeneous acellular structures, whereas fibrin-platelet thrombi were hardly observed in the embolus. Immunohistochemistry showed immunoglobulin deposits in the embolus, suggesting that immunological mechanisms were involved in the growth of the embolus. A pathological…

Rat Model of Middle Cerebral Artery Occlusion.

Stroke is the third-leading cause of death and the leading cause of acquired adult disability worldwide. Several ischemic stroke models are currently available. However, mimicking focal cerebral ischemia (FCI) is the most common. The formation of an embolic or thrombotic occlusion at or near the middle cerebral artery causes most events in FCI. The current protocol closely mimics the etiology of human stroke and ensures that the results obtained are highly relevant. The method described in this protocol yields reproducible results. The success of this model in ischemic research can be examined

Diagnosis and prevention of atherosclerotic cerebral infarction.

Atherosclerotic disease accounts for approximately 25% of ischemic strokes. Atherosclerotic stroke is caused mainly by embolic events from the carotid artery bifurcation or the aortic arch, although intracranial thrombosis can occur, more often in African Americans, Asians, and diabetes patients. Primary prevention of stroke is critical for patients with risk factors for atherosclerosis, including hypertension, diabetes, smoking and hypercholesterolemia. Stroke can be prevented in patients with established atherosclerotic disease by identification and management of patients with carotid artery
