Bacterial flora in hepatic encephalopathy in bilharzial and non-bilharzial patients.

In this work 25 of hepatic encephalopathy cases were studied. 15 cases were due to hepatic schistosomiasis and 9 were due to hepatic schistosomiasis and hepatitis B. virus and one due to hepatic schistosomiasis and drug toxicity of tarter emetic. E. coli was the causative organism in 67% of positive cases, bacteroids were the causative organism in about 33% of positive cases. Urinary tract infection is increased in this study as it was found in 32% of cases and the causative organism were E. coli in about 76% of cases.

Buspirone blocks motion sickness and xylazine-induced emesis in the cat.

Cats were tested for motion sickness following s.c. pretreatment with four doses of buspirone. Buspirone blocked motion sickness with an ED50 of 0.46 mg . kg-1 base. Buspirone pretreatment (4.0 mg . kg-1 base) also significantly blocked vomiting in cats later injected with 0.66 mg . kg-1 (base) s.c. of the emetic drug xylazine. The results are interpreted as showing that buspirone is acting at the vomiting center, the point of convergence for the separate mechanisms subserving chemically-induced emesis and motion sickness.

Ergometrine or oxytocin? Blood loss and side-effects at spontaneous vertex delivery.

Blood loss and the frequency of vomiting were assessed at 88 spontaneous vertex deliveries. An i.v. injection of oxytocin 10 u was as effective as ergometrine 0.5 mg in controlling bleeding from the uterus after delivery. The continuous infusion of a dilute solution of oxytocin in the first stage of labour was not followed by an increased blood loss at delivery. Oxytocin infusions were maintained for 1 h after delivery. Vomiting or retching occurred in 13% of the mothers who received i.v. ergometrine. None of the women who received oxytocin suffered emetic sequelae.

[Establishment of routines for elective laparoscopic surgery].

BACKGROUND: During the autumn of 1999, elective laparoscopic cholecystectomy was introduced as a new surgical procedure in the Central Hospital Østfold, Askim. A prospective evaluation of perioperative patient logistics and patient satisfaction was performed. MATERIAL AND METHODS: 214 patients were evaluated in the ambulatory; 116 of them were scheduled for operation. The operations were performed in general anaesthesia with continuous infusion of propofol and short acting opioides. Analgetics and anti-emetics were given pre- and perioperatively as prophylaxis against postoperative pain and n…

Acute poisonings among adolescents and young adults with anorexia nervosa.

The diagnosis and management of acute poisoning episodes in adolescents and young adults with anorexia nervosa are complicated by their unde … ing metabolic abnormalities. Decontamination efforts were complicated by the patients' prior use of emetics and cathartics. Seventeen patients required hospitalization; 4 patients suffered a complicated hospital course and a fifth patient died.

[The miraculous effects of taking laxatives. Success and failure of pre-modern medical treatment from the patients' perspective].

The massive use of laxatives, emetics and blood-letting in pre-modern medicine has often intrigued historians. Why did generations of prominent and often quite empirically minded physicians resort to methods which, in hindsight, were usually much more likely to do harm than good? And why did the patients not rebel?Based on patients' personal documents and, in particular, on the letter consultations many of them wrote ot renowned early modern physicians, this essay traces, how the sick and their relatives experienced and responded to this kind of treatment. These sources provide ample evidence,…

Hallaran's circulating swing.

William Saunders Hallaran (c.1765-1825) was physician superintendent at the County and City of Cork Lunatic Asylum for 40 years, where he distinguished between mental insanity and organic (systemic) delirium. In treatment he used emetics and purgatives, digitalis and opium, the shower bath and exercise, and argued that patients should be saved from 'unavoidable sloth' by mental as well as manual occupation. However, it is as an exponent of the circulating swing, proposed by Erasmus Darwin and used by Joseph Cox, that he is remembered. His best results were achieved, as he recorded in An Enquir…

Supplemental oxygen reduces the incidence of postoperative nausea and vomiting.

Postoperative nausea and vomiting (PONV) are unpleasent for patients and increase the risk of aspiration pneumonia. PONV is the leading cause of unexpected admission following planned day surgery. Despite new anaesthetic drugs and antiemetics, the incidence of PONV remains high. The incidence of PONV depends numerous factors including age, gender, obesity, anxiety, gastroparesis, history of motion sickness, previous PONV, and the duration and type of surgery. Anaesthesia related factors include premedication, ventilation techniques, and postoperative pain management. Recently, the intraoperati…

Bluish vomiting: a rare clinical presentation of poisoning.

Bluish vomiting is a symptom of poisoning that is rarely seen in Western emergency departments. Consequently, physicians are not aware of the diagnosis, complications, and treatment of this unusual form of intoxication. In this article, we report a case of bluish vomiting that occurred after an accidental ingestion of copper sulphate. In the discussion, we review three life-threatening causes of bluish vomiting (copper sulphate, boric acid, and paraquat ingestion), and we discuss their respective clinical manifestations, specificities, complications, and management therapies.

Chemical aversion treatment of alcohol dependence.

Developments in the application of chemical aversion therapy to the treatment of alcohol dependence are discussed. Historical factors leading to the early use of chemical aversion therapies are delineated and the theoretical underpinnings of chemical aversion interventions are evaluated. Ethical and procedural considerations are addressed and an assessment of the efficacy of the therapy is attempted. Future research activities that would lead to refinement of chemical aversion therapy protocols are highlighted. The effectiveness of chemical aversion treatment of alcohol dependence is discussed

The impact and management of hyperemesis gravidarum: Current and future perspectives.

Hyperemesis gravidarum (HG) is a severe form of nausea and vomiting in pregnancy affecting around 1%-3% of pregnancies worldwide and is the most common reason for hospital admission in early pregnancy. HG can have lasting consequences for both pregnant individuals and their offspring. Current best-practice management includes symptomatic treatment with anti-emetic pharmacological treatment, rehydration if needed, and psychological support. There is a lack of high-quality evidence on treatment for HG. Future research should focus on understanding the cause, developing effective treatment, and s

Cyclic Vomiting Syndrome and Cannabinoid Hyperemesis Syndrome: Their Intersection and Joint Existence.

Cyclic vomiting syndrome (CVS) and cannabinoid hyperemesis syndrome (CHS) are 2 disorders characterized by recurrent episodes of nausea and vomiting that are stereotypical in nature with intervening periods of baseline health. While they share overlapping clinical features, their underlying pathophysiology, epidemiology, and management strategies differ. CVS is primarily a disorder of gut-brain interaction with a strong association with migraine and autonomic dysfunction, whereas CHS is directly linked to chronic and heavy use of cannabis. The prodromal, emetic, recovery, and interepisodic bas

Medically unexplained myopathy due to ipecac abuse.

The use of ipecac, once recommended as an emetic for use in toxic ingestions, has more recently been discouraged for use in home and emergency room settings. It remains readily available, and has been associated with abuse in eating disorders and Munchausen syndrome by proxy. This case discusses an adolescent boy who surreptitiously abused ipecac in the context of distress over parental conflict, and the extensive medical workup undertaken to evaluate unexplained symptoms of proximal muscle weakness, abdominal pain, and, eventually, cardiomyopathy that are sequelae of ipecac toxicity.
