When Transfer Is Impossible: Emergency Management of Perforated Necrotizing Enterocolitis in a Neonate With Uncorrected Dextro-Transposition of the Great Arteries.

Dextro-transposition of the great arteries (d-TGA) is a congenital heart disease (CHD) characterized by parallel circulations, in which survival depends on intercirculatory mixing. CHD as a whole is a recognized risk factor for necrotizing enterocolitis (NEC), which carries high morbidity and mortality. Managing perforated NEC in a neonate with uncorrected complex d-TGA presents a complex challenge, particularly in centers lacking specialized pediatric cardiac surgery facilities. We report the successful emergency management of a 28-day-old preterm neonate with uncorrected complex d-TGA, a lar

Incidental Detection of Portal Venous Gas During POCUS in a Neonate With Necrotizing Enterocolitis and Intestinal Perforation: A Case Report.

In a postoperative cardiac neonate undergoing bedside hemodynamic assessment for suspected venous congestion, point-of-care ultrasound performed by a pediatric intensivist unexpectedly revealed hepatic portal venous gas. This finding prompted immediate abdominal reassessment and urgent radiography, which demonstrated pneumoperitoneum. Exploratory laparotomy revealed terminal ileal perforations with ischemic changes and fecal peritonitis. Although the examination was performed for hemodynamic purposes, careful assessment of the portal venous system disclosed a clinically important abnormality u

Serum TNF-α as an indicator reflecting the severity of intestinal barrier injury in a neonatal rat model of necrotizing enterocolitis.

BACKGROUND AND OBJECTIVE: Necrotizing enterocolitis (NEC) is a severe gastrointestinal emergency in preterm infants and is characterized by progressive intestinal barrier injury accompanied by systemic inflammation. This study aimed to determine whether peripheral inflammatory signals reflect structural damage to the intestinal barrier in a graded neonatal rat model of NEC and to identify candidate blood-based indicators of disease severity. METHODS: Neonatal Sprague-Dawley rats were assigned to Control, NEC, and NEC + LPS groups. Ileal injury was assessed by histopathology. The localization

Interpretation of clinical variables in the use of artificial intelligence-based decision support for surgical necrotizing enterocolitis: A prospective study.

PURPOSE: End-of-life decision-making in neonates with surgical necrotizing enterocolitis (NEC) is complex and varies between clinicians depending on their interpretation of clinical variables. This study aimed to create transparency in this variability and assess its impact on the use of Behavioral Artificial Intelligence Technology (BAIT) as a tool to inform clinical decision making. METHODS: Attendees of multidisciplinary team meetings where the decision between surgery and comfort care was made for a neonate with surgical NEC were asked to interpret clinical variables required for BAIT. The prediction models for the optimal timing of surgical intervention for necrotizing enterocolitis: nomogram vs. five machine learning models.

BACKGROUND: Necrotizing enterocolitis (NEC) is one of the most common diseases that pose serious threats to the life of newborns. In clinical practice, NEC is typically treated by surgical intervention, but it is still difficult to identify the timing of surgical intervention for this disease. Therefore, this study was conducted to establish a machine learning (ML) model for identifying the optimal timing of surgical intervention for NEC by comparing logistic regression (LR) models with ML models and to visualize important influencing indicators via a nomogram. METHODS: The basic information,

Analysis of risk factors and establishment of a predictive model for brain injury in neonates with surgical necrotizing enterocolitis: a retrospective study.

BACKGROUND: Neonatal necrotizing enterocolitis (NEC) is prevalent among preterm neonates and is associated with high morbidity and mortality. While surgical intervention remains essential for advanced NEC, postoperative neonates exhibit an elevated risk of brain injury. However, which surgery-related factors exacerbate neonatal brain damage remains insufficiently investigated. This study aimed to identify risk factors for brain injury in neonates with surgical NEC and establish a predictive model to facilitate early identification and intervention, which may ultimately decrease neurodevelopmen

Time to first passage of meconium and defecation frequency preceding necrotizing enterocolitis in preterm infants: a case-control study.

UNLABELLED: Necrotizing enterocolitis (NEC) is associated with significant morbidity and mortality in preterm infants. Early recognition and treatment of NEC are critical to improving outcomes. Enteric nervous system (ENS) immaturity has been proposed as a key factor in NEC pathophysiology. Gastrointestinal dysmotility is associated with ENS immaturity and may serve as a predictive factor for the development of NEC. In this case-control study, preterm infants (gestational age (GA) < 30 weeks) were included in two level-IV neonatal intensive care units. Infants with NEC in the first month of li

DMBT1 expression and neutrophil-to-lymphocyte ratio during necrotizing enterocolitis are influenced by impaired perfusion due to cardiac anomalies.

BACKGROUND: Deleted in malignant brain tumors 1 (DMBT1) is involved in innate immunity and epithelial differentiation. It has been proven to play a role in various states of inflammation or hypoxia of fetal gastrointestinal and pulmonary diseases. Discrimination of pathogenesis in necrotizing enterocolitis (NEC) based on cardiac status improves the understanding of NEC in different patient subgroups. We aimed at examining DMBT1 expressions regarding their association with cardiac status leading to impaired intestinal perfusion, intraoperative bacteria proof, and a fulminant course of NEC. MET

Early laparoscopic-assisted surgery is associated with decreased post-operative inflammation and intestinal strictures in infants with necrotizing enterocolitis.

INTRODUCTION: In 2015, a protocol including early laparoscopy-assisted surgery in the treatment of necrotizing enterocolitis (NEC) was implemented at our institution. Carbon dioxide insufflation during laparoscopy may have an anti-inflammatory effect. We aimed to compare post-operative outcome after early laparoscopy-assisted surgery and classical laparotomy for NEC. MATERIAL AND METHODS: Charts of premature infants undergoing surgery for NEC (2012-2021) were reviewed. Cases operated by early laparoscopy-assisted surgery (2015-2021) were compared to infants operated for NEC between 2012 and 2

A role for neonatal bacteremia in deaths due to intestinal perforation: spontaneous intestinal perforation compared with perforated necrotizing enterocolitis.

OBJECTIVE: To examine the relationship between intestinal perforations (caused by either spontaneous perforation (SIP) or necrotizing enterocolitis (NEC)) and the outcome "death due to intestinal perforation". METHODS: Multivariable logistic regression analyses were used to compare infants <28 weeks' gestation with SIP (n = 32) and perforated-NEC (n = 45) for the outcome perforation-related death. RESULTS: In univariate analyses the incidence of death due to perforation was higher among infants with perforated-NEC (36%) than infants with SIP (13%). However, infants with perforated-NEC were m

A comparison of the clinical presentation and outcome of focal intestinal perforation and necrotizing enterocolitis in very-low-birth-weight neonates.

There is controversy about the identity of focal intestinal perforation (FIP) and necrotizing enterocolitis (NEC). To elucidate the difference between them, we reviewed their clinical presentations. Over the last 20 years, 39 very-low-birth-weight (VLBW) neonates, including 21 extremely-low-birth-weight neonates, underwent a laparotomy for intestinal perforation without mechanical causes. Nineteen patients had typical findings of NEC, and 8 had FIP. FIP is defined as isolated intestinal perforation without gross necrosis. In FIP, the gestational age was significantly lower than in NEC (23.8 +/

Disruption of bile acid homeostasis potentiates Paneth cell ablation by activating the intestinal Farnesoid X receptor in necrotizing enterocolitis.

Although disrupted bile acid (BA) homeostasis is implicated in necrotizing enterocolitis (NEC), its role in NEC pathogenesis remains unclear. We revealed that secondary BA accumulation in the ileum with severe NEC induced Paneth cell (PC) loss via the activation of the intestinal farnesoid X receptor (FXR). Single-cell RNA sequencing (scRNA-seq) showed that high FXR expression was associated with the differentiation of intestinal stem cells (ISCs) in NEC. Mechanistically, intestinal FXR upregulation induced PC loss by inhibiting Wnt/planar cell polarity (PCP) signaling, which regulates ISC lin

Identification of diagnostic biomarkers for neonatal necrotizing enterocolitis via machine learning screening and single-cell virtual gene knockout validation.

Neonatal necrotizing enterocolitis (NEC) continues to be the most severe gastrointestinal emergency affecting preterm infants, with reported mortality rates ranging from 20% to 30%. The absence of distinct early biomarkers results in delayed intervention and unfavorable outcomes, and the molecular mechanisms underlying the dysregulated immune response in NEC remain incompletely defined. We integrated five gene expression omnibus datasets (36 NEC cases and 34 controls) using weighted gene co-expression network analysis and systematically evaluated 113 machine learning algorithm combinations. Local Peritoneal Cytokine Response IL-1β, IL-6, TNF-α in a Standardized Neonatal Rat Model of Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) is a life-threatening inflammatory disease of preterm infants, increasingly viewed as a cytokine-driven disorder of the immature intestine. We aimed to characterize local peritoneal concentrations of interleukin (IL)-1β, IL-6 and tumor necrosis factor-α (TNF-α) in a standardized neonatal rat NEC model and relate them to histopathological injury. Seventy-four SPRD/Mol/Lodz rat pups were allocated to a control group (CTRL; n = 12) or subjected to a hypoxia-hypothermia-formula-feeding NEC protocol (NEC; n = 62). After 72 h, small-intestinal samples were scored usin

Chemokine-receptor-dependent inflammatory responses contribute to two-hit-induced experimental necrotizing enterocolitis and represent a potential therapeutic target.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe multifactorial inflammatory disorder of the preterm intestine for which effective preventive and therapeutic strategies remain limited. Using formula feeding (FF)-associated dysbiosis combined with subsequent viral inflammation, we previously demonstrated that this clinically relevant two-hit strategy induces NEC through a complex mechanism involving the Klebsiella oxytoca-derived metabolite tilivalline (TV) and NK1.1 + cells. Here, we investigated whether chemokine-receptor-dependent inflammatory responses associated with inflammatory ce
