Optimizing CRT - Do We Need More Leads and Delivery Methods.

Cardiac resynchronization therapy (CRT) is an established therapeutic option in symptomatic heart failure with reduced ejection fraction and evidence of left ventricular (LV) conduction delay (QRS width ≥120 ms), especially when typical left bundle branch block is present. The rationale behind CRT is restoration of aberrant LV electrical activation. As there is considerable heterogeneity of the LV electrical activation pattern among CRT candidates, an individualized approach with targeting of the LV lead in the region of latest electrical activation while avoiding scar tissue may enhance CRT r

Refractory ventricular tachycardia secondary to cardiac sarcoid: electrophysiologic characteristics, mapping, and ablation.

BACKGROUND: Cardiac sarcoidosis is a recognized cause of ventricular tachycardia (VT) and sudden death that has not been well studied. OBJE … nts, in the left ventricle in 5 (63%) of 8 patients, and in the epicardium in 2 patients undergoing epicardial mapping. Ablation abolished one or more VTs in 6 (75%) of 8 patients, but other VTs remained inducible in all but one patient. Postablation, some form of sustained VT recurred in 6 of 8 patients within 6 months. However, at longer follow-up (range 6 months …

Relationships between three-dimensional fibrosis distribution, atrial adiposity, and voltage abnormalities associated with persistent atrial fibrillation.

AIMS: Persistent atrial fibrillation (PsAF) is often refractory to pulmonary vein isolation, a well-established AF treatment, owing to the fibrosis-remodelled substrates sustaining re-entries. Three-dimensional fibrosis distribution and atrial adiposity have been found to be essential contributors to PsAF arrhythmogenesis. We aimed to utilize late gadolinium enhancement (LGE)-MRI-derived personalized heart digital twins (DTs)-a recent promising tool for non-invasively assessing patient arrhythmogenesis-as well as contrast-enhanced cardiac computed tomography (CCT) images and electroanatomic ma

Incremental diagnostic value of artificial intelligence-derived coronary artery calcium in 18F-flurpiridaz positron emission tomography myocardial perfusion imaging.

BACKGROUND: Positron emission tomography (PET) myocardial perfusion imaging (MPI) is a powerful tool for predicting coronary artery disease (CAD). Coronary artery calcium (CAC) provides incremental risk stratification to PET-MPI and enhances diagnostic accuracy. We assessed the additive value of the CAC score, derived from PET/CT attenuation maps to stress total perfusion deficit (TPD) results using the novel 18F-flurpiridaz tracer in detecting significant CAD. METHODS: Patients from the 18F-flurpiridaz phase III clinical trial who underwent PET/CT MPI with the 18F-flurpiridaz tracer, had ava

EDEN (Electrocardiographic Radial Depth Navigation): A Novel Approach to Navigate Inside Heart Muscle.

BACKGROUND: Intramyocardial guidewire navigation is a novel technique that allows free transcatheter movement within ventricular muscle. Guidewire radial depth, between endocardial and epicardial surfaces, is ambiguous by x-ray and echocardiography. OBJECTIVES: The aim of this study was to develop a simple tool, EDEN (Electrocardiographic Radial Depth Navigation), to indicate radial depth during intramyocardial guidewire navigation. Combined with routine imaging, EDEN facilitates a new family of intramyocardial catheter procedures to slice, reshape, pace, and ablate the heart. METHODS: We ma

Inflammation as a Predictor of Recurrent Ventricular Tachycardia After Ablation in Patients With Myocarditis.

BACKGROUND: Little is known about the risk stratification of patients with myocarditis undergoing ventricular tachycardia (VT) ablation. OBJECTIVES: This study sought to describe VT ablation results and identify factors associated with arrhythmia recurrences in a cohort of patients with myocarditis. METHODS: The authors enrolled 125 consecutive patients with myocarditis, undergoing VT ablation. Before ablation, disease stage was evaluated, to identify active (AM) versus previous myocarditis (PM). The primary study endpoint was assessment of VT recurrences by 12-month follow-up. Predictors of

A unified functional/anatomic substrate for circus movement atrial flutter: activation and refractory patterns in the canine right atrial enlargement model.

OBJECTIVES: This study was designed to test the concept of a functional/anatomic interaction in a canine model of reentry based on right atrial enlargement and to elucidate the electrophysiologic basis for functional conduction block. BACKGROUND: The monotonic feature of atrial flutter suggests a uniform substrate for the arrhythmia. Atrial flutter in the sterile pericarditis model is due to single-loop circus movement around a functional or a functional/anatomic obstacle near the atrioventricular (AV) ring. Sustained circus movement requires a critical interaction of a functional arc of bloc

Distinguishing shear and tensile myocardial wall stiffness using ex vivo anisotropic Magnetic Resonance Elastography.

The organized myofiber structure within the myocardium indicates its mechanical anisotropy. By projecting the MR Elastography (MRE) stiffness matrix along either the myocardial fiber or sheet orientations determined by Diffusion Tensor Imaging (DTI), anisotropic MRE (aMRE) maps axial and transverse shear and Young's moduli into three tensile and six shear deformation modes. Ten healthy ex vivo swine hearts were imaged three times at 3T using MRE and DTI sequences. aMRE results showed a within-subject coefficient of variation at 19 % for the fiber model and 28 % for the sheet model across speci

Atrial reentry around an anatomic barrier with a partially refractory excitable gap. A canine model of atrial flutter.

We have characterized, in dogs, a model of inducible regular atrial tachycardia that resembles atrial flutter. The model involves creating a Y-shaped lesion comprised of an intercaval incision and a connected incision across the right atrium. It is suitable for serial studies of the effects of pacing or antiarrhythmic drugs in chronically instrumented animals studied in the awake state for at least several months. The postoperative cycle length of the induced tachycardia varies from 143 to 188 msec, depending on the size of the dog. The tachycardia cycle length was consistent for each dog, and

Diagnosis of multiple infarcts from complex electrocardiograms during normal rhythm, left bundle-branch block, and ventricular pacing.

INTRODUCTION: Because of better treatment of chronic coronary artery disease, patients with multiple ischemic insults are living longer and requiring long-term management. Electrocardiograms (ECGs) in patients with multiple myocardial infarctions (MMIs) are a challenge to the clinician or cardiologist. This is because second myocardial infarctions (MIs) often modify or obscure the ECG effects of the first MI and vice versa. This can result in complex tracings, which are difficult to interpret and predict the locations of damaged regions. It is important to identify MMI because, additively, the

Ventricular arrhythmogenic remodelling in diet-induced metabolic syndrome driven by right-to-left regional differences in action potential duration and dominant frequency gradients.

Metabolic syndrome (MetS) has been associated with an increased prevalence of cardiac arrhythmias and sudden cardiac death caused by ventricular fibrillation (VF), but the exact underlying mechanisms are not known. Our aim here was to study the effects that diet-induced MetS produces on ventricular remodelling and its potential electrophysiological arrhythmogenic mechanisms. Thirty-five male NZW rabbits were assigned to a control (n = 16) or MetS group (n = 19), fed for 28 weeks with a high-fat and high-sucrose diet. Echocardiography and electrocardiography were performed before diet and at we

Body Surface Mapping of Ventricular Repolarization Heterogeneity: An Ex-vivo Multiparameter Study.

BACKGROUND: Increased heterogeneity of ventricular repolarization is associated with life-threatening arrhythmia and sudden cardiac death (SCD). T-wave analysis through body surface potential mapping (BSPM) is a promising tool for risk stratification, but the clinical effectiveness of current electrocardiographic indices is still unclear, with limited experimental validation. This study aims to investigate performance of non-invasive state-of-the-art and novel T-wave markers for repolarization dispersion in an ex vivo model. METHODS: Langendorff-perfused pig hearts (N = 7) were suspended in a

Intracardiac echocardiography guided anatomical approach to cardioneuroablation: feasibility and outcomes.

AIMS: Anatomical studies have documented a close topographical relationship between the ganglionated plexi (GP) containing parasympathetic inputs to the sinus node (SN) and atrioventricular node (AVN) and the epicardial fat pads (FPs) within the Waterston's interatrial groove. We aimed to investigate the feasibility and outcomes of a novel anatomical approach to cardioneuroablation (CNA) that targets the atrial areas adjacent to the interatrial FPs identified with intracardiac echocardiography (ICE). METHODS AND RESULTS: About 17 patients [37.3 ± 10.2 years, 47% female] undergoing CNA for rec

Electrophysiological Characterization of Subclinical and Overt Hypertrophic Cardiomyopathy by Magnetic Resonance Imaging-Guided Electrocardiography.

BACKGROUND: Ventricular arrhythmia in hypertrophic cardiomyopathy (HCM) relates to adverse structural change and genetic status. Cardiovascular magnetic resonance (CMR)-guided electrocardiographic imaging (ECGI) noninvasively maps cardiac structural and electrophysiological (EP) properties. OBJECTIVES: The purpose of this study was to establish whether in subclinical HCM (genotype [G]+ left ventricular hypertrophy [LVH]-), ECGI detects early EP abnormality, and in overt HCM, whether the EP substrate relates to genetic status (G+/G-LVH+) and structural phenotype. METHODS: This was a prospecti

Distinctive Left Ventricular Activations Associated With ECG Pattern in Heart Failure Patients.

BACKGROUND: In contrast to patients with left bundle branch block (LBBB), heart failure patients with narrow QRS and nonspecific intraventricular conduction delay (NICD) display a relatively limited response to cardiac resynchronization therapy. We sought to compare left ventricular (LV) activation patterns in heart failure patients with narrow QRS and NICD to patients with LBBB using high-density electroanatomic activation maps. METHODS AND RESULTS: Fifty-two heart failure patients (narrow QRS [n=18], LBBB [n=11], NICD [n=23]) underwent 3-dimensional electroanatomic mapping with a high densi

Hypernatremia and intercalated disc edema synergistically exacerbate long-QT syndrome type 3 phenotype.

Cardiac voltage-gated sodium channel gain-of-function prolongs repolarization in the long-QT syndrome type 3 (LQT3). Previous studies suggest that narrowing the perinexus within the intercalated disc, leading to rapid sodium depletion, attenuates LQT3-associated action potential duration (APD) prolongation. However, it remains unknown whether extracellular sodium concentration modulates APD prolongation during sodium channel gain-of-function. We hypothesized that elevated extracellular sodium concentration and widened perinexus synergistically prolong APD in LQT3. LQT3 was induced with sea ane

A Novel Approach to Cardiac Magnetic Resonance Scar Characterization in Patients Affected by Cardiac Amyloidosis: A Pilot Study.

Background and Objectives: Cardiac magnetic resonance (CMR) imaging has become an essential instrument in the study of cardiomyopathies; it has recently been integrated into the diagnostic workflow for cardiac amyloidosis (CA) with remarkable results. An additional emerging role is the stratification of the arrhythmogenic risk by scar analysis and the possibility of merging these data with electro-anatomical maps. This is made possible by using a software (ADAS 3D, Galgo Medical, Barcelona, Spain) able to provide 3D heart models by detecting fibrosis along the whole thickness of the myocardial
