The results of volumetric studies of the amygdala and hippocampus in patients with epilepsy who also have psychopathology are described. The main data relate to the increased size of the amygdala in patients with epilepsy and psychosis. The associations between the amygdala and affective disorders, notably dysthymia and euphoria, are also discussed.

Epilepsy and psychosis are diseases of the brain which cause considerable impact on quality of life particularly when they occur in the same individual. This review comments on postictal psychosis, chronic schizophrenia like psychosis in epilepsy, and the psychoses related to the treatment of epilepsy with antiepileptic drugs and surgery. Possible mechanisms of psychosis including the concept of forced normalization and dopaminergic overactivity in the mesolimbic system are discussed.

THE EXTENT OF THE PROBLEM: A patient can be misleadingly considered as epileptic although he is not. Conversely an epileptic man not be recognised as such. Moreover, in some patients epilepsy seizures may co-exist with pseudo-seizures. THE PROBLEMS IN DIAGNOSIS: Each stage of the diagnosis of epilepsy is limited, whether regarding anamnesis, electroencephalogram, not only standard but also video, or even the semiological analysis because of the clinical polymorphism of partial epileptic seizures. THE MAJOR DIFFERENTIAL DIAGNOSES: Migraine with aura may be difficult to differentiate from occipi…

To study the spectrum of epilepsy in children with cerebral palsy, 105 consecutive children with cerebral palsy and active epilepsy, between 1 and 14 years of age, were studied prospectively. A detailed history and examination, electroencephalography (EEG), and computed tomography (CT) were done in all cases. The social quotient was assessed using the Vineland Social Maturity Scale. A retrospective cohort of 452 cases of cerebral palsy was studied to find the prevalence of epilepsy in cerebral palsy. A control group of 60 age-matched children with cerebral palsy but no epilepsy was also studie…

An occurrence of depression or depressive symptomatology has been reported in 30% of patients with epilepsy. Depression has been reported peri- and interictally. To make a differentiation may be difficult in patients with frequent seizures. However, complex partial seizures, particularly if are located on temporal lobe, appear to be etiologic factors, especially in men with left-sided epileptic foci. Depression is also more frequent in patients treated with polytherapy, particularly with phenobarbital and vigabatrin. The depression appears to be endogenous and has also been described in patien…

OBJECTIVE: To estimate the number of sudden unexpected epilepsy deaths occurring annually in England and Wales in those 16-50 years of age.
METHODS: All 1997 death entries mentioning epilepsy as a cause of death in those 16-50 years were examined and classified as sudden unexpected death in epilepsy (SUDEP), other epilepsy related deaths, or non-epilepsy deaths.
RESULTS: 612 death entries were obtained with postmortem examination having been performed in 498 cases. Forty four deaths were certified as being attributable to SUDEP and a further 292 deaths were considered to be probable SUDEP ca…

One hundred and six epilepsy patients were assessed over a period of 6 months for psychiatric morbidity, social support, stressful life events in previous year and disability. 45 patients (42.45%) had a psychiatric diagnosis. Organic depressive disorder headed the list (16.98%) followed by mild cognitive disorder (11.32%) and tobacco dependence (8.49%). There was no significant difference in the mean age, sex, mean education, age at onset of epilepsy, duration of epilepsy, psychiatric diagnosis, mean scores on social support scale, presumptive stressful life event scale and disability assessme…

For pediatric patients, the earlier in life epilepsy occurs, the lower are long-term mental abilities. Three rare but important conditions among infants and children--pyridoxine-deficiency seizures, Lennox-Gastaut syndrome, and West syndrome--usually require multidrug therapy to control seizures. Epilepsy occurs more frequently among the elderly and therapy is complicated by treatment of other chronic medical conditions. Numerous effective antiepileptic medications for both pediatric and geriatric patients have been newly marketed in the past decade, with other promising agents in the investig…

OBJECTIVE: To describe the prevalence and characteristics of epilepsy in patients with cerebral palsy in a tertiary center.
METHODS: a total of 100 consecutive patients with cerebral palsy were retrospectively studied. Criteria for inclusion were follow-up period for at least 2 years. Types and incidence of epilepsy were correlated with the different forms of cerebral palsy. Other factors associated with epilepsy such as age of first seizure, neonatal seizures and family history of epilepsy were also analysed.
RESULTS: follow-up ranged between 24 and 151 months (mean 57 months). The overall …

Neuroimaging examines the relationship between abnormalities of brain function in epilepsy patients (seizures, impaired cognitive function, psychiatric co-morbidity etc.) and focal or more widespread brain pathology. Since the mid-1980s, the introduction of magnetic resonance imaging (MRI) into clinical neurology has had an impact on the diagnosis, treatment, and research of epilepsy only comparable with the advent of the electroencephalography (EEG) fifty years ago. MRI plays the important role of identifying single or multiple structural lesions responsible for the epileptic seizures. Thus, …

Idiopathic epilepsies comprise a wide variety of partial and generalized syndromes that have in common a known or presumed genetic etiology and the lack of overt abnormalities other than the epilepsy itself. Most of these epilepsies have a benign natural history and/or show a favorable response to antiepileptic drug (AED) therapy, but pharmacoresistance does occur in some patients. In general, therapeutic algorithms in idiopathic partial epilepsies (IPEs) are similar to those used for symptomatic partial epilepsies, but aggressive pharmacologic therapy is rarely indicated in these patients. In…

Tumour associated epilepsy (TAE) is a poorly understood manifestation of many gliomas, meningiomas and metastatic brain tumours that has important clinical and social implications. Etiological mechanisms underlying tumour associated epilepsy include theories invoking peritumoural amino acid disturbances, local metabolic imbalances, cerebral oedema, pH abnormalities, morphological changes in the neuropil, changes in neuronal and glial enzyme and protein expression and altered immunological activity. It has also been suggested that the pathology involves perturbations in distribution and functio…

Epilepsies are a diverse group of disorders with both shared and distinct features. Classification allows a coherent, systematic approach and serves as a 'universal language' between neurologists all over the world. There are two systems of classifications of epilepsies in use today. The International League Against Epilepsy (ILAE) Classification of Epileptic Seizures 1981 divides seizures into 3 morphologic types--generalised, partial and unclassifiable, with subtypes of each. This system has the advantage that seizures can be classified relatively easily and choice of antiepileptic is dictat…

BACKGROUND: Taenia solium neurocysticercosis (NCC) has been documented as one of the major causes of epilepsy in developing countries. However, methodological limitations have hindered the evaluation of the epidemiological relationship between cysticercosis and epilepsy at the community level.
METHODS: We used the WHO protocol for epidemiological evaluation of neurological disorders to conduct a door-to-door survey among 2723 residents of San Pablo del Lago, an Ecuadorean rural community in which T. solium taeniasis/cysticercosis was known to be endemic. The WHO protocol was complemented by n…

Myoclonic jerks occur in a number of different syndromes. There is many classifications of myoclonus. It is preferred the Fejerman classification, slightly modified that present the following five groups: 1. Myoclonus without encephalopathy and without epilepsy, which includes physiological myoclonus; 2. Encephalopathies with non epileptic myoclonus, which includes Kinsbourne syndrome and certain types of hyperekplexia which pose differential diagnosis problems with reflex myoclonic epilepsy; 3. Progressive encephalopathies with myoclonic seizures which includes typical and atypical progressiv…

THE CONCEPT OF BENIGN EPILEPSY: The benign nature of epilepsy is difficult to establish. The diagnosis of epilepsy often has an effect disproportionate to the severity. In everyday practice, most cases of epilepsy follow a benign course. A benign evolution is the rule in certain syndromes which are usually diagnosed early. In other cases however, the benign prognosis cannot be determined except after long-term follow-up. BENIGN EPILEPSY: There are 3 large categories of benign epilepsy: partial idiopathic epilepsy, generalized idiopathic epilepsy, occasional seizures. They are particularly freq…

Obstructive sleep apnoea was first brought to prominence by Henri Gastaut, a French epileptologist. Since that time the interface between epilepsy and sleep disorders has received less attention than might be justified, recognizing that sleep deprivation is a poignant provocateur for seizures. Sleep deprivation is often used as a diagnostic procedure during electroencephalography (EEG) when waking EEG has failed to demonstrate abnormality. Patients referred to an outpatient neurological clinic for evaluation of possible seizures in whom sleep disorder was suspected, either due to snoring durin…

INTRODUCTION: The quality of life of an epileptic patient largely depends on the interplay of the control of the convulsive crises and the degree to which higher mental functions are affected. Some types of epilepsy lead to mental retardation during development and with others the basic cerebral mechanisms for processing data are damaged and this may lead to transient or permanent cogniscitive deterioration.
DEVELOPMENT: There are certain important clinical factors such as the site of the discharge regarding the lobe, hemisphere, type of cerebral area involved and the age of the patient. We d…
