An adaptor for continuous stimulation of the facial nerve (SACS) to be used with the WR-S8 Monitor/Stimulator during otologic surgery has been developed. This device allows the surgeon to use the electrified air drill and microsurgical instruments instead of a probe-tip during dissection. The SACS saves surgical time, is easy and convenient to use, and is safe. Routine monitoring of facial nerve function using SACS has helped prevent iatrogenic facial nerve injuries and has improved our ability to save the facial nerve during otologic and neurotologic surgery.

BACKGROUND: Craniofacial nerve injuries are a major source of long-term morbidity following maxillofacial trauma, often resulting in substantial functional impairment and psychosocial burden. This study aimed to perform a trauma-focused bibliometric analysis of the evolving research landscape on craniofacial nerve injuries and to determine whether the literature reflects a shift from trauma-related injury mechanisms toward surgically modifiable risk factors.
METHODS: A bibliometric analysis was conducted using publications indexed in the Web of Science (WoS) Core Collection and Scopus databas

We have treated 16 patients with gunshot injuries to the cheek, 10 of whom had damage to the parotid. There were nine men and one woman, mean age 40 (range 15-65). All injuries were high velocity, and eight had other injuries. We followed them up for a month; three patients required further operation, three had facial palsy, and one lost his hearing on that side.

OBJECTIVE: The goals were to describe the epidemiologic features of pediatric skating-related injuries sustained from 1993 to 2003 and to co … ncussions (4.3%), compared with roller skaters (0.6%) and in-line skaters (0.8%). The proportion of facial injuries among ice skaters was greater than the proportions among roller skaters and in-line skaters. The majority of roller skating-and in-line skating-related injuries were upper-extremity fractures (53.9% and 59.7%, respectively). Children < or = 6 years …

High-resolution multidetector computed tomography with multiplanar reformations and 3-D postprocessing often provides the detail necessary for preoperative assessment of facial injuries. Maxillofacial fractures are classified in the following manner: upper face fractures, midface fractures (the most frequent), Le Fort fractures, and lower face or mandible fractures. The facial skeleton is a framework of vertical and horizontal buttresses that ensures a better resistance to trauma, but serves also as reference for maxillofacial surgery to restore facial size and shape. Radiologists should know …

Treatment of parotid duct injuries varies with the anatomic location and the type of injury involved. The duct may be injured by sharp or blunt facial trauma. Moderate to severe facial swelling or clear drainage shortly after injury in the region of the parotid gland or its duct should alert the clinician to this potential problem. The anatomic considerations, as well as a variety of techniques to treat these injuries, are presented. A case involving a particularly resistant chronic ductal injury, with an extraoral fistula and its eventual cure, is presented. A second case of a lacerated duct

The aim of this study was to evaluate the effects of protecting the facial nerve and reducing the scar visibility using a modified tragus edge and transmasseteric anteroparotid approach compared to classic preauricular approach for intracapsular and condylar neck fractures. This retrospective study included 64 patients (78 sides) who underwent surgical treatment for intracapsular or condylar neck fractures from January 2014 to June 2018. Patients were divided into the experimental group (treated via a modified tragus edge and transmasseteric anteroparotid approach), and the control group (trea

The management of facial fractures in the polytrauma patient requires the coordination of multiple surgical disciplines to optimize the functional and cosmetic outcome while minimizing overall morbidity and mortality. Although the plastic surgery literature historically advocates the early repair of facial fractures, the risk of general anesthesia in patients with associated injuries sometimes makes early repair unsafe. We compared early operative repair versus delayed operative repair of facial fractures in multitrauma patients. We specifically examined wound infection, overall complication r

OBJECTIVE: To introduce a Thy1-GFP transgenic rat model, whose axons constitutively express green fluorescent protein (GFP), in order to study facial nerve regeneration. Facial nerve injury can cause devastating physical and social sequelae. The functional recovery of the facial nerve can result in synkinesis and permanent axonal misrouting. Facial nerve research has been hindered by the lack of available animal models and reliable outcome measures.
METHODS: Transgenic Thy1-GFP rats underwent a proximal facial nerve crush injury and were imaged at 0, 1, 2, 4, and 8 weeks after injury. Nerve r

PURPOSE: Distinctive mechanisms of primary blast effects have produced a transitional era of facial trauma. Implosion mechanism damage is one of these. Implosion mechanism damage results in injury limited to the gas-containing structures of the auditory canal, paranasal sinuses, gastrointestinal tract, and lungs. Worldwide, the victims of explosive detonations have increased and advanced dramatically. The outcome is greater mortality and morbidity and new types of injuries, especially in the maxillofacial region. Thus, the knowledge of, and experience with, their management should be shared gl

The authors retrospectively studied 13,870 singleton full-term consecutive live births at a major teaching hospital and assessed the frequency of brachial plexus injury, clavicular fracture, and facial nerve injury in newborns delivered from January 1974 through December 1977 and from January 1979 through September 1981. Predisposing factors relating to mechanical birth trauma were defined, and a risk assessment profile was developed to identify the fetus at risk. Mechanical injuries were identified according to the International Classification of Diseases codes 761. The incidence of selected

Individuals with very high spinal cord injuries (e.g. C1-C3) may be ventilator-dependent and therefore unable to support speech breathing. However, their facial musculature is intact, given that these muscles are innervated by cranial nerves. We developed a system using surface electromyography (sEMG) recorded from facial muscles to control a phonemic interface and voice synthesizer and tested the system in healthy individuals. Users were able to use five facial gestures to control an onscreen cursor and the phonemic interface. Users had mean information transfer rates (ITRs) of 59.5 bits/min

INTRODUCTION: The Armed Forces personnel are exposed to various kinds of injuries due to the nature of their duties. Increase in motorized population without taking protective measures and rise in violence has contributed towards maxillofacial injuries. The aim of this study was to determine the incidence, aetiology and management of injuries resulting in fracture of the Zygomatic complex in Armed Forces personnel and their families.
METHODS: This study was conducted at Command Military Dental Centre (EC). Out of 90 maxillofacial injuries, 40 individuals (44.4%) were treated for Zygomatic com…

OBJECTIVE: Facial nerve injury induces autophagy and apoptosis in facial nerve nucleus motoneurons of the CNS, impairing nerve regeneration and functional recovery. The function of P2Y2R after facial nerve injury remains to be determined. This study hypothesizes that inhibiting P2Y2R may play a protective role in facial nerve injury by modulating the autophagy signaling pathway.
METHODS: An in vivo mouse model of facial nerve crush injury was utilized in this study. Mice received either a P2Y2R agonist or antagonist through intrathecal injections of 10 μL/daily for 4 weeks. This study measure

Facial nerve paresis is a known complication to surgery in the facial region. Spontaneous regeneration of such injuries has been described, but little is known about the prognosis and optimal management. This is a case report of two patients with partial facial nerve paresis following surgery. The patients were treated conservatively. Both experienced a regeneration over ten months post-operatively. This suggests a favourable prognosis with conservative treatment in selected cases, which is reassuring for both doctors and patients.
