INTRODUCTION: This study aims to identify radiographic and clinical risk factors of perioperative periprosthetic femur fracture associated with the direct anterior approach (DAA) using a metaphyseal fit and fill stem. We hypothesize stem malalignment with this femoral implant places increased stress on the medial calcar region, which leads to an increased risk of periprosthetic fracture.
METHODS: We compared patients with periprosthetic femur fractures following DAA total hip arthroplasty (THA) utilizing the Echo Bi-Metric Microplasty Stem (Zimmer Biomet, Warsaw, IN) to a cohort of patients w

BACKGROUND: Association of arterial stiffness and osteoporosis has been previously reported in women. However, this association is still controversial for men. Therefore, we investigated correlation of arterial stiffness and osteoporosis by measuring brachial-ankle (ba) pulse wave velocity (PWV) and bone mineral density (BMD).
METHODS: We reviewed medical charts of 239 people (women: 128, men: 111) who visited the Health Promotion Center, retrospectively. ba-PWV was measured by automatic wave analyzer. Lumbar spine (L1-L4) BMD and femur BMD were measured by dual energy X-ray absorptiometry. M

Two case reports and an updated critical review on bilateral fractures of the femoral neck are presented. Bilateral fractures of the femoral neck have been investigated for at least 80 years and are treated as rare cases. The primary cause is usually considered an external shock; however, aside from high energy shocks (e.g., falling and impact with hard, rigid surfaces, traffic accidents, etc.) the underlying causes of femoral neck frailty have not yet been fully understood. Although not exhaustive, the review spans cases reported as early as 1944 and compares their conclusions in line with me

BACKGROUND: Stable fixation promotes union in the common femoral neck fractures, but high non-union rates due to fixation failure remain with traditional fixations. To enhance stability, a plate interlocking pins, but without further fixation to femur has been developed. To our knowledge, no comparison to other conventional fixation methods has been performed. We tested the hypothesis that the novel implant biomechanically leads to a more stable femoral neck fixation.
METHODS: Fifty synthetic femurs with a cervical wedge osteotomy were allocated to intervention with three hook-pins interlocke

BACKGROUND: It has been controversial that whether hardware removal will increase the risk of osteonecrosis of femoral head (ONFH) in fracture-healed patients who underwent internal fixation for femoral neck fractures (FNFs). This meta-analysis aimed to clarify the association of hardware removal with secondary hardware removal-induced ONFH (HR-ONFH).
METHODS: Four electronic databases (PubMed, Embase, Web of Science, Cochrane Library) were searched for eligible studies published up to March 10, 2023. Studies reporting the relative risk of hardware status (i.e., risk rate, odds ratio [OR], or

In this study, we used 3-dimensional analysis to comprehensively map the osseous morphology of the acetabulum. Human cadaveric specimens were dissected to the joint capsule for computer navigation analysis. Data points outlining acetabular anatomy-determined using optical sensors-were translated into graphical environments. A clock face template was laid over the transverse plane to determine the projections of acetabular arcs onto the transverse plane. A custom-written software program was used to compute the resulting surface area and was applied to the acetabular articular surface and the f

Congenital femoral deficiency (CFD) Paley type 1b is characterized by severe bony deformity of the upper femur, extra-articular contractures of the hip, and, delayed ossification of the femoral neck and/or subtrochanteric region. The Systematic Utilitarian Procedure for Extremity Reconstruction of the hip (SUPERhip) procedure for the correction of CFD deformities was developed in 1997. Initially, a non-fixed angle device (rush rod) was used for fixation. Late complications of persistent delayed ossification and recurrent varus deformity occurred. In order to reduce and treat such complications

We report a patient with subacute myelo-optico-neuropathy (SMON) in whom spinal anesthesia was employed to treat fracture of the femur neck. An 87-year-old woman was diagnosed as having SMON at the age of 45. The patient was admitted to our hospital with fracture of the femur neck. Aspiration pneumonia was also suspected with shadow in the right lung on the chest X-P The percutaneous oxygen saturation (Spo2) with room air was 77%. Spinal anesthesia with 5 mg of 0.5% hyperbaric bupivacaine and 20 mcg of fentanyl was performed at L3-4. The level of anesthesia was T4. During surgery, no severe pa…

The choice of the appropriate implant continues to be critical for fixation of unstable hip fractures. Therefore, the goal of this study was to develop a numerical model to investigate the mechanical performance of hip fracture osteosynthesis. We hypothesized that decreasing fracture stability results in increasing load share of the implant and therefore higher stress within the implant. We also investigated the relationship of interfragmentary movement to the fracture stability. A finite element model was developed for a cephalomedullary nail within a synthetic femur and simulated a pertrocha

A cadaver study was undertaken to evaluate (a) the relations between the rotation around the axis of the neck of the femur, intracapsular effusion, and intracapsular pressure; and (b) the importance of the atmospheric pressure in stabilizing the hip joint. The following conclusions were reached: (1) There is no increase in intracapsular pressure within the normal range of rotation ("flexion") around the axis of the neck of the femur. (2) Intracapsular fluid (e.g., blood, pus, synovial edema and/or free synovial fluid) decreases this pressureless range of rotation, a likely cause of pain and su

INTRODUCTION: Enchondroma is a solitary, benign, and intramedullary cartilaginous tumor occurring most commonly in small bones of hands and feet contributing to 3-10% of all bone tumors. They originate from the growth plate cartilage which later on proliferates to form enchondroma. Lesions are central or eccentric and metaphyseal involvement is most common for long bones. We report a case of atypical occurrence of enchondroma in the head of femur in a young male.
CASE REPORT: A 20-year-old male patient presented with a history of pain in the left groin for 5 months. Radiological examination s

AIMS: Osteonecrosis of the femoral head (ONFH) after femoral neck fracture poses challenges in children, particularly at Ficat stage III. Limited effective treatments are available. This study explores basicervical femoral neck rotational osteotomy (BFNRO) for ONFH following fracture of the femoral neck in children and adolescents, and evaluates its outcomes.
METHODS: Children and adolescents with ONFH (Fiat stage III) following fracture who underwent BFNRO at our centre from June 2017 to September 2022 were included. Follow-up exceeded one year, with data on modified Harris Hip Score (mHHS),

SUMMARY: The medial femoral condyle flap is well-described for reconstruction of small bone defects of the upper and lower extremities. There are limited case reports of its use in other anatomic sites, particularly for reconstruction of complex head and neck defects. In the setting of previous radiation and contaminated fields, vascularized bone is generally preferred to bone grafts, cadaveric allografts, or synthetic implants. The authors present a case series of complex craniofacial defects involving the midface that were reconstructed using medial femoral condyle flaps, focusing on the typ

Background and Objectives: Total hip arthroplasty (THA) is considered the most successful surgical procedure in orthopedics. However, dislocation remains the main indication for surgical revision. New designs of dual mobility cups (DMC) have lowered the classical complications and have extended the indications of DMC in elective surgeries. Our aim is to assess the trend of DMC indications in THA as well as the incidence of their dislocation. Materials and Methods: We retrospectively reviewed all patients undergoing THA with DMC during the years 2015 and 2021. The original indication for DMC in
