Near-infrared light transillumination (NIR-LT) vs clinical-visual inspection/FOTI to detect interproximal caries lesions in vivo.

OBJECTIVE: To evaluate the ability of near-infrared light transillumination (NIR-LT) to detect interproximal enamel and dentinal caries lesions compared to clinical-visual inspection aided by fiber-optic transillumination (FOTI). METHOD AND MATERIALS: From 170 Finnish adolescents aged 15 to 17 years, 5,294 interproximal surfaces of premolars and molars were examined first clinical-visually aided by FOTI (VI+FOTI) using the International Caries Detection and Assessment System (ICDAS) classification. Subsequently, the surfaces were examined using NIR-LT. The extent of lesions was determined

Fiberscope-Based Measurement of Coaptation Height for Intraoperative Assessment of Mitral Valve Repair.

BACKGROUND: Restoring adequate coaptation height is a key principle of mitral valve (MV) repair. This study aimed to evaluate the utility of fiberscope (FS) technology to assess MV coaptation height for intraoperative use. METHODS: Ex-vivo testing was performed on five adult porcine hearts. The left atrium (LA) was resected, and the left ventricle (LV) was pressurized retrograde to 27 ± 1mm Hg. An endoscope was inserted into the LV apex, centered under the MV orifice. An FS system (Milliscope II camera, LED light source, and 0.7 mm diameter × 15 cm long) 90° semirigid scope with 1.2 mm focal

Investigation into the application of remimazolamin conjunction with low-dose propofolfor pediatricfiberoptic bronchoscopy.

This study delves into the effectiveness of combining remimazolam with low-dose propofol in pediatric fiberoptic bronchoscopy. Ninety children scheduled for fiberoptic bronchoscopy in our hospital were enrolled as research participants. Based on the intraoperative anesthetic drug regimen, the children were divided into three groups: group R (remimazolam 0.2-0.4 mg/kg), group P (propofol 1-3 mg/kg), and group RP (remimazolam0.2 mg/kg, propofol 0.5 mg/kg). Immediately post-anesthesia, group P exhibited lower blood pressure and heart rate (HR) compared to both group R and group RP (P < 0.05). As

Transurethral light delivery for prostate photoacoustic imaging.

Photoacoustic imaging has broad clinical potential to enhance prostate cancer detection and treatment, yet it is challenged by the lack of minimally invasive, deeply penetrating light delivery methods that provide sufficient visualization of targets (e.g., tumors, contrast agents, brachytherapy seeds). We constructed a side-firing fiber prototype for transurethral photoacoustic imaging of prostates with a dual-array (linear and curvilinear) transrectal ultrasound probe. A method to calculate the surface area and, thereby, estimate the laser fluence at this fiber tip was derived, validated, app

[Intubation of a first time parturient with Klippel-Feil syndrome].

We describe a case of a first time parturient with Klippel-Feil syndrome (KFS). KFS is defined by fusion of cervical vertebrae and the clinical triad of low posterior hairline, short wide neck and limited neck movement. KFS represents a complex challenging anaesthesiologic condition due to limited movement of the neck and the risk of irreversible neurologic sequelae if manipulated, as well as unpredictable effect of neuroaxial anaesthesia. We recommend awake fiberoptic intubation for airway management in this rare disorder and stress the necessity of early anaesthesiologic assessment in any ce

Ultrasound assessment of vocal fold paresis: a correlation case series with flexible fiberoptic laryngoscopy and adding the third dimension (3-D) to vocal fold mobility assessment.

BACKGROUND: Perioperative examination ofthe vocal folds with flexible fiberoptic laryngoscopy is not always feasible. Prior studies suggest vocal fold ultrasound may provide a useful screening tool, however, correlation to laryngoscopic findings is necessary. The purpose of the case series was to validate vocal fold ultrasound in the adult population and to correlate the ultrasound findings to the assessment provided by flexible fiberoptic laryngoscopy. MATERIALS AND METHODS: This IRB approved study accrued sixteen patients. Vocal fold ultrasound performed by the anesthesiologist was correlat

The laryngeal mask airway for difficult airway in temporomandibular joint ankylosis--a case report.

Patients with Temporomandibular Joint Ankylosis are among difficult airways and anesthesia and airway management of these patients encounter anesthesiologists with challenge. Herein we report a case of Temporomandibular Joint Ankylosis with difficult ventilation after failed attempts to intubate with fibroptic broncoscopy a disposable Laryngeal Mask Airway reestablishes the ventilation. The case suggests that the disposable Laryngeal Mask Airway may be useful in airway management of patients with a Temporomandibular Joint Ankylosis.

Retromolar fibreoptic orotracheal intubation in a patient with severe trismus undergoing nasal surgery.

PURPOSE: A considerable challenge arises when passage of an endotracheal tube between the teeth is impossible because of severe trismus and the presence of concomitant contraindications to nasotracheal intubation. We report a novel technique to circumvent the need for tracheostomy by using the retromolar space for oral fibreoptic intubation. CLINICAL FEATURES: A 50-yr-old female with a history of pharyngeal cancers treated with surgery and radiotherapy presented for right dacryocystorhinostomy. She had undergone left dacryocystorhinostomy after nasotracheal intubation one week earlier. This t

[Life-threatening macroglossia following cleft palate palatoplasty].

The case of a 13-month-old child who developed a life-threatening macroglossia with airway obstruction following palatoplasty for a cleft palate is reported. As direct laryngoscopy was not feasible a laryngeal mask (LM) was inserted to secure the airway. Under fiber optic guidance an endotracheal tube was then introduced via the LM. In this article the incidence, pathophysiology, clinical dynamics, options for emergency anesthesia management and organizational implications of this rare but typical complication in the field of oral and craniomaxillofacial surgery are reported.

Rolling mechanical imaging for tissue abnormality localization during minimally invasive surgery.

We describe a novel approach for the localization of tissue abnormalities during minimally invasive surgery using a force-sensitive wheeled probe. The concept is to fuse the kinaesthetic information from the wheel-tissue rolling interaction into a pseudocolor rolling mechanical image (RMI) to visualize the spatial variation of stiffness within the internal tissue structure. Since tissue abnormalities are often firmer than the surrounding organ or parenchyma, a surgeon then can localize abnormalities by analyzing the image. Initially, a testing facility for validating the concept in an ex vivo

[Videofiberscopic study of laryngeal abnormalities in patients with severe motor and intellectual disabilities].

A total of 43 patients with severe motor and intellectual disabilities were enrolled in videofiberscopic investigation of laryngeal abnormalities. Their ages ranged from 2 to 41 years (mean: 19.9 years) and all of them had severe quadriplegia due to cerebral palsy or other conditions. Thirty-two out of the 43 patients (74.4%) had laryngeal abnormalities;laryngeal stenosis including that of iatrogenic origin, hypersecretion and laryngomalacia. Direct visualization of the occurrence of aspiration could be achieved in some patients. Seven (87.5%) out of 8 patients with tracheostomy and 25 (71.4%)

Chandelier illumination to complete Descemet stripping through severe hazy cornea during Descemet-stripping automated endothelial keratoplasty.

We describe a technique that uses chandelier illumination during Descemet-stripping automated endothelial keratoplasty (DSAEK) in severe bullous keratopathy. A 25-gauge or 27-gauge chandelier illumination fiber inserted through the corneal side port serves as sclera-scattering illumination from the sclerocorneal margin and endoillumination from the anterior chamber and provides excellent visibility for Descemet stripping and intraocular manipulation without obstruction by a hazy cornea. In cases complicated by dense cataract, the chandelier fiber can be inserted transconjunctivally into the pa

Awake fibrecapnic intubation: a novel technique for intubation in head and neck cancer patients with a difficult airway.

Awake fibreoptic intubation is the gold standard for difficult airway management but failures are reported in the literature in up to 13% of cases. In case of failure, a tracheotomy is often indicated. We describe a novel technique for intubation in head and neck cancer patients with a difficult airway that we call awake fibrecapnic intubation. The aim of this study was to investigate the feasibility of this technique. We studied prospectively 15 consecutive intubations in head and neck cancer patients before diagnostic or therapeutic surgical procedures. After topical anaesthesia, a fibrescop

Virtual versus conventional laryngeal endoscopy.

We have evaluated the capacity of virtual laryngoscopy and conventional laryngoscopy conducted with a rigid or flexible instrument to visualize laryngeal structures in 64 patients with normal endoluminal anatomy. Virtual laryngoscopy allowed total visualization of laryngeal structures, including those that could not be reached by a flexible instrument. There was good correlation between virtual laryngoscopy and "real" images, indicating satisfactory diagnostic accuracy (p < .05). Although virtual laryngoscopy does not provide histologic data, it is a fast and noninvasive technique that can be

Advancing the tracheal tube over a flexible fiberoptic bronchoscope by a sleeve mounted on the insertion cord.

UNLABELLED: The advancement of an endotracheal tube (ETT) over a flexible fiberoptic bronchoscope (FOB) is often impeded at the glottis. This is attributed to the creation of a cleft by the difference in the outer diameter of the fiberscope and the internal diameter of the tube. We designed a conical-shaped polyvinyl chloride sleeve to fit the insertion cord. This report compares the ease of advancement of the tube over a sleeved versus a nonsleeved bronchoscope. General anesthesia was induced, and one anesthesiologist introduced the FOB (a 3.8-mm Olympus LF2). Patients were randomly assigned
