Developing the formula for state subsidies for health care in Finland.

AIM: The aim was to generate a research-based proposal for a new subsidy formula for municipal healthcare services in Finland. METHODS: Small-area data on potential need variables, supply of and access to services, and age-, sex- and case-mix-standardised service utilisation per capita were used. Utilisation was regressed in order to identify need variables and the cost weights for the selected need variables were subsequently derived using various multilevel models and structural equation methods. RESULTS: The variables selected for the subsidy formula were as follows: age- and sex-standard

Exploring the lived experience of alcohol use disorder patients in accessing hospitalized treatment care in Rwanda.

BACKGROUND: Alcohol Use Disorder (AUD) is a major mental health challenge in Rwanda, yet little is known about how individuals with AUD navigate the healthcare system. This study explored the lived experience of AUD patients when seeking and accessing inpatient treatment within the Rwandan context. METHODS: Semi structured qualitative interviews were conducted with inpatients diagnosed with AUD across three neuropsychiatric facilities in Rwanda. Thematic analysis was conducted to identify recurring patterns and themes related to care-seeking experiences. RESULTS: From 27 in-depth interviews,

Child health implications of transitioning from charcoal to electric cooking in Uganda: a qualitative stakeholder study.

BACKGROUND: Household air pollution and cooking-related injuries from charcoal use remain major contributors to preventable child morbidity and mortality in low- and middle-income countries, including Uganda. Electric cooking (e-cooking) could reduce children's exposure to these risks, yet adoption remains limited and the policy conditions shaping this transition are poorly understood from a child health perspective. OBJECTIVE: To explore policy stakeholder perspectives on transitioning from charcoal to e-cooking in Uganda and to examine perceived implications for child health related to hous

Progress in family planning in Sierra Leone: a mixed-methods case study.

INTRODUCTION: Expanding access to voluntary family planning is a global health priority, yet progress has been uneven across low-income and middle-income countries. Despite being a fragile state with historically high fertility and limited health resources, Sierra Leone has achieved one of the fastest increases in modern contraceptive prevalence in sub-Saharan Africa. This study explores the drivers of this progress through a mixed-methods case study. METHODS: We analysed national-level data from urban and rural settings, and engaged diverse stakeholders including family planning professional

Regional features in the treatment of Parkinson's disease in the Baltic States compared to Germany.

Parkinson's Disease (PD) is a progressive neurodegenerative disorder characterized by motor symptoms, alongside non-motor manifestations. In advanced stages, people with PD (PwP) require extensive support to maintain mobility and quality of life, creating substantial demand on healthcare and reimbursement systems. Reported prevalence varies across Europe, with Germany at 350 per 100,000, Estonia at 314 per 100,000, Lithuania at 265 per 100,000, and Latvia at 217 per 100,000. This study applies a structured, qualitative comparative policy mapping approach (a subtype of comparative policy analys

Gender differences in well-being among people living with non-communicable disease: The influence of social capital and grants.

BACKGROUND: This study explores how non-communicable diseases (NCDs), social capital, and government grants (social grants) influence subjective well-being (SWB) among individuals aged 40 and older in rural South Africa. Understanding gender differences in these relationships provides insights for improving public health interventions in resource-constrained settings. METHODS: Data from 2,432 participants in the HAALSI Wave 3 study were analyzed to examine the predictors of SWB using regression models. Key covariates included age, education, marital status, employment, wealth, religion, socia

Barriers to community engagement during the response to an Ebola virus disease outbreak in Uganda.

BACKGROUND: Uganda reported an outbreak of Ebola virus disease (EVD) in 2022. As part of the outbreak response, government and partners promoted community engagement, which seeks to involve communities in the design, implementation and evaluation of interventions to raise awareness, build trust between communities and partners and create ownership of interventions. This study, therefore, explored barriers to community engagement during the 2022-2023 EVD outbreak response in Uganda. METHODS: This qualitative study, conducted in five districts (Kampala, Kassanda, Kyegegwa, Mubende and Wakiso),

Motivating health workers up to a limit: partial effects of performance-based financing on working environments in Nigeria.

BACKGROUND: In 2012, the Nigerian government launched performance-based financing (PBF) in three districts providing financial incentives to health workers based on the quantity and quality of service provision. They were given autonomy to use funds for operational costs and performance bonuses. This study aims to understand changes in perceived motivation among health workers with the introduction of PBF in Wamba district, Nigeria. METHODS: The study used a qualitative research design to compare perceptions of health workers in facilities receiving PBF payments in the pilot district of Wamba

["Smoked meat, full of rind, hardly edible"--patient's complaints and doctor's rebuttal in the first German state-run mental sanatory "Rasemühle" between 1903 and 1932].

Around 1900 a psychiatric reform movement propagated the foundation of sanatoriums for the lower middle class in Germany. These sanatoriums were supposed to cure patients suffering from neurasthenia and associated disorders. Many private sanatoriums existed for curing neurasthenia. Visiting them was a luxury beyond most of the patients' means. Therefore, the so called "Volksnervenheilstätten"-movement aimed at providing sanatorium care for free or at very low costs. One of the first sanatoriums that arose from this movement was the "Rasemühle" close to Goettingen. It was founded in 1903. As a

The Oportunidades conditional cash transfer program: effects on pregnancy and contraceptive use among young rural women in Mexico.

CONTEXT: Oportunidades is a large conditional cash transfer program in Mexico. It is important to examine whether the program has any direct effect on pregnancy experience and contraceptive use among young rural women, apart from those through education. METHODS: Data from the 1992, 2006 and 2009 waves of a nationally representative, population-based survey were used to describe trends in pregnancy experience, contraceptive use and education among rural adolescent (15-19) and young adult (20-24) women in Mexico. To examine differences in pregnancy experience and current modern contraceptive u

An analysis of the accessibility of video lottery terminals: the case of Montréal.

BACKGROUND: Researchers and public health officials in Canada, the United States and Australia have for some time noted broader geographic accessibility to gambling establishments, above all in socioeconomically underprivileged communities. This increase in availability could lead to more and more gambling problems. This article focuses, in an ecological perspective, in particular on a spatial analysis of the geographic accessibility of sites possessing a VLT permit in the Montréal area, i.e. Montréal Island, the South Shore and Laval, from the standpoint of the development of an indicator of

Provision of contraceptive and related services by publicly funded family planning clinics, 2003.

CONTEXT: In addition to contraceptive services, publicly funded family planning clinics provide low-income women with a range of reproductive diagnostic, treatment and educational services. Nationally representative information about the scope of services available from clinics is needed to formulate policy and programmatic recommendations. METHODS: In 2003, more than 1,000 U.S. clinics responded to an eight-page survey on service availability and clinic policies. Differences in the proportions of clinics reporting each service or policy were examined by clinic type and receipt of Title X fun

Experience of targeting subsidies on insecticide-treated nets: what do we know and what are the knowledge gaps?

Widespread coverage of vulnerable populations with insecticide-treated nets (ITNs) constitutes an important component of the Roll Back Malaria (RBM) strategy to control malaria. The Abuja Targets call for 60% coverage of children under 5 years of age and pregnant women by 2005; but current coverage in Africa is unacceptably low. The RBM 'Strategic Framework for Coordinated National Action in Scaling-up Insecticide-Treated Netting Programmes in Africa' promotes coordinated national action and advocates sustained public provision of targeted subsidies to maximise public health benefits, alongsid

Health care for the poor: some policy alternatives.

Changes in the financing and organization of medical care are most likely to affect adversely the poor who have significant needs for care, but face increasingly stringent eligibility criteria in Medicaid and other public programs. Americans estimated to have neither private nor public health insurance coverage number 33 million persons, and with increased cost pressures, voluntary and proprietary hospitals are less willing to treat such patients. One quarter of hospitals provide 60 percent of all care to the poor, and many of these nonprofit and public hospitals face economic difficulties and

Political stability, health system resilience, and macroeconomic shocks as factors associated with malaria control outcomes: a comparative analysis of Ecuador and Venezuela (2000-2023).

BACKGROUND: The post-2000 period in South America presents a striking divergence in malaria epidemiology: Ecuador achieved a >99% reduction in malaria incidence approaching elimination, while Venezuela experienced one of the most severe resurgences in modern public health history. We examined the association between governance factors, health policies, intersectoral coordination, and malaria control outcomes by comparing the national experiences of Ecuador and Venezuela. METHODS: We conducted a comparative ecological study using epidemiological data from WHO World Malaria Reports and PAHO upd

Contracting out to improve the use of clinical health services and health outcomes in low- and middle-income countries.

BACKGROUND: Contracting out of governmental health services is a financing strategy that governs the way in which public sector funds are used to have services delivered by non-governmental health service providers (NGPs). It represents a contract between the government and an NGP, detailing the mechanisms and conditions by which the latter should provide health care on behalf of the government. Contracting out is intended to improve the delivery and use of healthcare services. This Review updates a Cochrane Review first published in 2009. OBJECTIVES: To assess effects of contracting out gove
