The Diagnosis and Management of Mallet Finger Injuries.

BACKGROUND: Mallet finger is a common injury of the extensor tendon insertion causing an extension lag of the distal interphalangeal joint. METHODS: We reviewed the most current literature on the epidemiology, diagnosis, and management of mallet finger injuries focusing on the indications and outcomes of surgical intervention. RESULTS: Nonoperative management has been advocated for almost all mallet finger injuries; however, complex injuries are usually treated surgically. There is still controversy regarding the absolute indications for surgical intervention. CONCLUSIONS: Although surgery

Absent Ring Finger Flexor Digitorum Profundus Presenting as a Jersey Finger.

We report the case of a patient who presented with an apparent acute avulsion of the ring finger flexor digitorum profundus (FDP), or jersey finger. At surgery, the FDP to the ring finger was found to be absent; also absent were the lumbrical to the ring finger and the A5 pulley. Absence of the FDP was confirmed with postoperative imaging. Although absent profundus tendons have been previously reported, none have involved the FDP to the ring finger. However, various reports have described abnormal connections between the ring and little finger flexor tendons.

Anatomy of the collateral ligaments of the proximal interphalangeal joint.

PURPOSE: To study and clarify the anatomy of the proper collateral ligaments and accessory collateral ligaments of the proximal interphalangeal joint. METHODS: The collateral ligaments of 8 proximal interphalangeal joints were dissected under an operating microscope to gain an appreciation of their fiber direction and the anatomy of their origin and insertion. Two undissected joints were studied histologically. RESULTS: The proper collateral ligament was found to arise widely from dorsal and proximal to and from the fovea on the side of the head of the proximal phalanx and insert for some di

Anatomical attachments to the proximal phalangeal base--a case for stability.

Transverse midshaft fractures of the proximal phalanx are often unstable and require open reduction and internal fixation. In contrast, fractures of the base of the proximal phalanx are occasionally amenable to conservative treatment in the form of manipulation and a graded mobilisation programme. This stability may be the result of splinting of the fracture by the surrounding anatomical structures, but to our knowledge this has not been previously elucidated anatomically. In this study we define the extent of attachment of the various anatomical structures to the base of the proximal phalanx.

Diagnosis of closed central slip injuries. A cadaveric analysis of non-invasive tests.

Four non-invasive tests for central slip integrity were analysed using 20 fresh frozen cadaver fingers. A pre-boutonnière deformity was simulated by dividing the central slip. A passively correctable boutonnière was simulated by dividing the central slip, triangular ligament and oblique fibres of the extensor expansion. The test described by Boyes, which evaluates distal interphalangeal joint flexion, was found not to be reliable for the diagnosis of either injury. The test described by Elson, which evaluates distal interphalangeal joint rigidity while actively extending the flexed proximal in

Comparison of low-profile plate-screw and Kirschner-wire osteosynthesis outcomes in extra-articular unstable proximal phalangeal fractures.

BACKGROUND: We aimed to present the radiological and clinical results of Kirschner-wire (K-wire) fixation and low-profile plate-screw fixation applied to unstable proximal phalangeal fractures without extension to the joint. METHODS: Clinical and radiological data of 22 patients who underwent fixation with open reduction and low-profile 1.5-mm titanium mini plate and unlocked screws and 18 patients who underwent fixation with K-wire due to closed, unstable extra-articular proximal phalangeal fractures were retrospectively examined. All evaluation and statistical analyses were based on sixth p

[Clinical observation on combined repair of avulsion injury of the distal phalanx of the finger with dorsalis branch of digital artery flap and cross-finger flap].

OBJECTIVE: To investigate the clinical efficacy of combined dorsalis branch of digital artery flap and cross-finger flap in the repair of avulsion injury of the distal phalanx of the finger. METHODS: Between March 2019 and June 2023, 13 patients with degloving injury of the distal phalanx of the finger or amputated finger without replantation conditions were treated with combined homodigital dorsalis branch of digital artery flap and cross-finger flap, including 11 males and 2 females, aged from 21 to 55 years old. Among them, 6 cases were complicated with distal phalanx fracture. The injury

Ollier's disease treated with grafting using alpha-tricalcium phosphate cement. A case report.

Ollier's disease is a rare disorder characterized by multiple enchondromas with a unilateral predominance, especially in fingers in early ch … t required. All activities of daily life could be resumed immediately after surgery and none of the finger joints showed further restriction of motion. X-rays taken three years after the operation showed new bone ingrowth surrounding the material with little evidence of absorption. To our knowledge, this may be the first case of Ollier's disease treated with a …

3-T direct MR arthrography of the wrist: value of finger trap distraction to assess intrinsic ligament and triangular fibrocartilage complex tears.

PURPOSE: To determine the value of applying finger trap distraction during direct MR arthrography of the wrist to assess intrinsic ligament and triangular fibrocartilage complex (TFCC) tears. MATERIALS AND METHODS: Twenty consecutive patients were prospectively investigated by three-compartment wrist MR arthrography. Imaging was performed with 3-T scanners using a three-dimensional isotropic (0.4 mm) T1-weighted gradient-recalled echo sequence, with and without finger trap distraction (4 kg). In a blind and independent fashion, two musculoskeletal radiologists measured the width of the scapho

The clenched fist syndrome: case report of a clinical rarity of special interest for psychiatrists and hand surgeons.

BACKGROUND: The Clenched Fist Syndrome (CFS) is a type of a psychiatric disorder, in which the patients show flexion finger contractures. Although no organic etiology can be identified, the syndrome in most cases presents with pain and paradoxical stiffness. CASE PRESENTATION: We, herein, report the case of a 52-year old woman with a 6-month history of progressive hand flexion contracture and intermittent numbness in the first 3 fingers, mimicking carpal tunnel syndrome. On examination, all digits, including the thumb, were held in a tight flexion at the metacarpal and interphalangeal joints.

Congenital contractural arachnodactyly suspected by abnormally long extremities by fetal ultrasound.

Congenital contractural arachnodactyly (CCA) is a rare disease with the clinical features of limited extension of multiple joints, arachnodactyly, camptodactyly, thin and long extremities, and so on. In the point of long extremities, CCA resembles Marfan syndrome (MFS). CCA is easily differentiated from MFS after birth due to the flexion of multiple joints, including elbows, knees, hips and fingers. During the fetal period, observation of arachnodactyly and folded fingers by fetal ultrasound is the means of differential diagnosis between these two diseases. We report on a case of CCA diagnosed

Pachydermodactyly: An Unknown Entity.

Pachydermodactyly is a rare benign disease that usually affects the dorsal skin and subcutaneous tissue of the proximal interphalangeal joints of the index, middle, ring, and little fingers; the thumb is usually spared. Hyperkeratosis on the dorsal side of the affected areas is common. We present a case of a 19-year-old man who was diagnosed with pachydermodactyly after evaluation of a painless, progressive swelling and hyperkeratosis at the proximal interphalangeal and distal interphalangeal joints of all fingers, bilaterally, for 6 years before presentation. Conservative treatment was prescr

[Reconstruction of metacarpophalangeal and interphalangeal joints with endoprostheses following injuries].

Implants have been used since 1969 by the authors for the replacement of damaged finger joints following trauma. In several cases reconstruction of other damaged tissues in the same finger was also performed. A method is described for replacement of the central slip of the extensor tendon over the PIP joint. Analysis of twenty-one MP and sixty-eight PIP endoprostheses placed in eighty-three patients until 1979 is given. Significant improvement in the range of movement is seen. Preoperative pain was relieved in all cases. There was a need to change occupation in ten percent of patients. Careful…

[Diabetes-specific hand changes in type I diabetes mellitus].

Diabetes-specific hand alterations (cheirarthropathy) were observed in 44 out of a total of 215 children and adolescents with insulin-dependent diabetes. In 28 of them limitations of movement in the proximal or (and) distal interphalangeal joints of one finger, and in the remaining 16 patients changes of at least two fingers were seen. Hand and shoulder joints or the vertebral column were not affected. In a control group of 110 probands without metabolic disorder only five showed cheirarthropathy. The frequency of diabetes-specific alterations of the hand increased with the age of the patients

Effects of finger posture on carpal tunnel pressure during wrist motion.

Persistent elevations in carpal tunnel pressure may aggravate carpal tunnel syndrome. This study examined the effects of finger posture on carpal tunnel pressure during wrist motion. Carpal tunnel hydrostatic pressure was measured using a saline-filled catheter inserted into the nondominant wrists of 14 healthy individuals. Range of motion tasks of wrist flexion-extension and radioulnar deviation were repeated with metacarpophalangeal (MCP) joint angles of 0 degrees, 45 degrees, and 90 degrees flexion. Pressures were significantly greater with the fingers straight (MCP = 0 degrees) than when t

Current aspects of osteoarticular pathology in patients undergoing hemodialysis: study of 80 patients. Part 1. Clinical and radiological analysis.

Eighty patients undergoing longterm hemodialysis were evaluated for joint function and radiographic abnormalities. Mean age of the population was 53 years, and mean duration of dialysis was 76 months (24 patients underwent dialysis for more than 10 years). Fifty-eight patients (72%) had joint symptoms; nonspecific arthralgia was the most frequent. Fifteen patients had morning stiffness and interrupted sleep due to arthralgias (shoulders 9, fingers 3) and 10 patients had diminished finger mobility. Radiographic evaluation showed 7 classes of lesions which are not characteristic of renal osteody

Two Cases of Pachydermodactyly Presenting as Polyarthritis.

Pachydermodactyly is characterized by asymptomatic, progressive swelling of the lateral aspects of the 2nd to 4th finger along the proximal interphalangeal (PIP) joint without involving the joint itself. We present 2 interesting cases of patients with periarticular swelling who were initially diagnosed and treated as juvenile idiopathic arthritis (JIA) with subsequent clinical and pathology confirmation of pachydermodactyly. These cases emphasize the importance of considering pachydermodactyly in young patients with development of periarticular swelling and no joint involvement.
