[PECULIARITIES OF STRUCTURAL CHANGES IN RAT LUNGS IN APPLICATION OF DIFFERENT METHODS OF REHYDRATION AFTER GENERAL DEHYDRATION].

Dehydration of the organism leads to the disturbances in homeostasis and structural changes in the lungs. Inadequate rehydration can also lead to the undesirable consequences. Experiments were carried out on rats, which modeled general dehydration and its correction: with metered potable water consumption, with water consumption without restrictions and correction by using a physiological solution. It was established that total dehydration leads to a decrease in the mass of the organ, the thinning of the interalveolar partitions with simultaneous decrease in the diameter and area of the alveol

How to guide volume expansion in severe sepsis and septic shock patients? Possibilities in the real world.

Early fluid administration is fundamental for the initial treatment of severe sepsis and septic shock patients. A large portion of suspected severe sepsis and septic shock patients can be quickly resuscitated with therapeutic tests until surrogate cardiac output markers, such as heart rate and urinary output, are normalized. Delaying volume expansion until invasive instrumentation is installed is not justifiable. When clinical stabilization is not achieved with initial fluid resuscitation, more careful, complete, and accurate monitoring should be started for both reversing tissue hypoxia and p

Fluid management for the prevention and attenuation of acute kidney injury.

In patients with acute kidney injury (AKI), optimization of systemic haemodynamics is central to the clinical management. However, considerable debate exists regarding the efficacy, nature, extent and duration of fluid resuscitation, particularly when the patient has undergone major surgery or is in septic shock. Crucially, volume resuscitation might be required to maintain or restore cardiac output. However, resultant fluid accumulation and tissue oedema can substantially contribute to ongoing organ dysfunction and, particularly in patients developing AKI, serious clinical consequences. In th

[Discussion on the fluid resuscitation and monitoring during burn shock period].

The index of monitoring burn shock resuscitation includes clinical signs and symptoms, laboratory examinations, hemodynamic variables. In re … t combined with other traditional variables still should be considered primary criteria of adequate fluid therapy. Specification of the variables of monitoring burn shock resuscitation is also the basis to revise and optimize the fluid resuscitation formula.

Diabetic ketoacidosis.

Diabetic ketoacidosis (DKA) is a complex and potentially fatal metabolic disorder in patients with diabetes mellitus. An understanding of the pathophysiology of DKA is essential in order to optimize patient management. A combination of insulin deficiency, increased stress hormone levels, and volume depletion account for the laboratory abnormalities and clinical signs observed in these patients. Successful therapy depends upon correction of hyperglycemia, dehydration, and electrolyte and blood gas abnormalities.

[The medical treatment of primary hyperparathyroidism].

The first-choice treatment of primary hyperparathyroidism is surgical removal of abnormal parathyroid gland(s) and the medical management is usually reserved only to control severe hypercalcemia. However, asymptomatic primary hyperparathyroidism with mild hypercalcemia and renal and bone status close to normal is now the more common picture of the disease and in this situation conservative management can be considered, because many of those patients may have a prolonged benign course. Management guidelines are therefore devised to minimize the risk for deterioration of renal, skeletal or gastr

Twenty-five years of septic shock hemodynamic resuscitation trials: a conceptual perspective.

BACKGROUND: Twenty-five years of septic shock resuscitation trials have progressively transformed the physiological understanding of shock and the way bedside interventions are conceptualized. Early strategies focused on correcting isolated global hemodynamic or metabolic targets through protocolized interventions, whereas more recent approaches increasingly recognize septic shock as a heterogeneous, dynamic, and context-dependent condition requiring individualized physiological assessment. MAIN BODY: This perspective discusses how hemodynamic resuscitation trials during this quarter of a cen

Navigating an Intravenous Fluid Shortage Crisis: A Health System's Response to a Hurricane-Induced Supply Disruption.

INTRODUCTION: Hurricane Helene's disruption of Baxter's North Carolina manufacturing facility in September 2024 created significant supply disruptions and an urgent need to reduce intravenous fluid (IVF) usage across our 12 acute care hospital health system. METHODS: Our approach combined an integrated informatics and operational response that included real-time electronic health record monitoring, clinical decision support tools, and hospital-level operational interventions. RESULTS: We achieved and sustained a peak 44% reduction in IVF usage while maintaining quality metrics. CONCLUSIONS:

Septic shock: Past, present, and perspectives.

Septic shock remains one of the most severe complications of infection, defined by circulatory, cellular, and metabolic dysfunction and associated with persistently high mortality. The concept has evolved markedly, from early descriptions of "blood poisoning" to the recognition of sepsis as a systemic syndrome in the late 20th century. Consensus definitions and large clinical trials, including early goal-directed therapy, led to structured treatment bundles and the launch of the Surviving Sepsis Campaign, which have contributed to gradual improvements in survival. Current management emphasizes

[Myth "Volume Deficiency in Sepsis"].

Septic shock is a life-threatening condition caused by a dysregulated immune response to an infection and can lead to tissue and organ damage and ultimately to death. Early recognition and elimination of tissue perfusion loss caused by sepsis are key elements in the treatment of patients with septic shock. Hemodynamic management is divided into four phases. 1) initial phase, 2) optimization, 3) stabilization, 4) de-escalation. Fluid therapy is widely accepted in the initial phase, but recent evidence challenges its universality, highlighting fluid overload risks and increased mortality, necess…

Revolution in sepsis: a symptoms-based to a systems-based approach?

Severe infection and sepsis are medical emergencies. High morbidity and mortality are linked to CNS dysfunction, excessive inflammation, imm … served and secondary injury will be reduced. We have been developing a systems-based, small-volume, fluid therapy comprising adenosine, lidocaine and magnesium (ALM) to treat sepsis and endotoxemia. Our early studies indicate that ALM therapy shifts the CNS from sympathetic to parasympathetic dominance, maintains cardiovascular-endothelial glycocalyx coupling, …
