Encephalomyelitis with hearing loss: case report on neurobrucellosis.

Neurobrucellosis is a zoonotic infection transmitted by infected animals or by contact with animal products and is an important health problem in resource-limited settings. In this case report, we present the case of a middle-aged man presenting with encephalomyelitis with hearing loss and diagnosed with neurobrucellosis based on imaging and serological tests. Because brucellosis infection is indolent, a high index of suspicion should be maintained in individuals presenting with fever, neurological signs and symptoms so that early management can prevent long-term neurological sequelae such as

Neurocysticercosis: neurologic, pathogenic, diagnostic and therapeutic aspects.

Worldwide neurocysticercosis is the most common parasitic infection of the human brain and meninges. Clinical features of the illness vary with the stage of ova infection, but most problems arise when the mature cyst degenerates. Seizures, increased intracranial pressure, and focal neurologic signs then often develop. Computed tomography and magnetic resonance usually demonstrate Cysticercus cellulosae cysts in the brain. A new immunoblot test for antibodies to the cysticercus seems both sensitive and specific. Treatment with praziquantel or albendazole has hastened the disappearance of the cy

Endodontics or root canal therapy.

This article discusses the diagnosis and treatment of pulp involved teeth. There are still some patients and some older physicians who associate certain systemic diseases with dead teeth. For this reason, the subject of focal infection is considered first. As many patients with facial pain or dental infection present first to their physician, it is important for the medical practitioner to understand the cause and its treatment. Finally, brief reference is made to the treatment of teeth injured as a result of trauma with special emphasis on first aid where permanent teeth have become dislodged…

Infections by entomopathogenic fungi in common green iguanas (Iguana iguana) in captivity in Brazil.

Entomopathogenic fungi, widely available biological agents used to control agricultural pests, are sporadically reported to cause focal or disseminated infection in reptiles and mammals, including humans. This study summarizes the clinical presentation, histopathological and molecular findings by panfungal polymerase chain reaction and sequencing of four cases of hypocrealean fungal infections in captive common green iguanas (Iguana, iguana). One case of granulomatous pneumonia, hepatitis and serositis was related to Metarhizium flavoviride complex infection. Two disseminated fungal infection

Early Pathological JC Virus Lesions in a Patient without Any MRI-based Indications.

A 70-year-old woman with a human T-cell leukemia virus type 1 infection without any focal neurological symptoms showed age-related atherosclerotic changes in the white matter without any suspicious signal changes suggestive of progressive multifocal leukoencephalopathy (PML) based on the findings of MRI. Viral polymerase chain reaction (PCR) revealed 6,700 copies/mL of the JC virus genome in the cerebrospinal fluid (CSF). An immuno-pathological examination of the autopsied brain revealed JC virus capsid proteins, and in situ hybridization confirmed a JC virus infection, indicating that an acti

Central nervous system alterations as sequelae of Venezuelan equine encephalitis virus infection in the rat.

Alterations of the Central Nervous System (CNS) in rats surviving acute infection with a virulent strain of Venezuelan Equine Encephalitis (VEE) virus were studied by light and electron microscopy. Cavitary necrosis of the cerebral cortex, macrophage activity and degenerative axonal changes were considered to be sequelae of the lesions induced during the acute phase of the infection. Mononuclear cell infiltrates of the neuropil, 3 mth after inoculation, were related to the immune response of the host. Focal lesions and mononuclear cell activity in the brain are thought to be the equivalent of

Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis syndrome and collapsing focal segmental glomerulosclerosis.

Collapsing glomerulopathy is a distinct and aggressive clinicopathologic variant of focal segmental glomerulosclerosis (FSGS). Here we present a 46-year-old man with acute renal failure from biopsy-proven collapsing FSGS and nephrotic syndrome in the setting of a diagnosis of hemophagocytic lymphohistiocytosis secondary to acute Epstein-Barr virus infection. This rare association has not been well described previously.

Recurrent Meningitis and Subarachnoid Hemorrhage Due to Salmonella in an HIV+ Patient: Case Report and Mini-Review of the Literature.

Meningitis due to non-typhi salmonella is infrequent in HIV-positive adults. We report a case of a patient with >300 CD4+ cells/mm3 who presented with five episodes of recurrent meningitis, focal subarachnoid hemorrhage and cerebral vasculitis ultimately attributed to Salmonella choleraesuis infection. Even within the cART era invasive salmonellosis can occur in unusual ways in HIV-infected patients.

Listeria monocytogenes osteomyelitis.

Listeria monocytogenes is usually an opportunistic pathogen causing either meningitis or bacteremia in adults. Focal infection outside the central nervous system occurs infrequently. We describe two cases of osteomyelitis caused by L monocytogenes. Certain characteristics of L monocytogenes may make cure difficult, particularly in a deep-seated focus such as bone, and may warrant special consideration when planning therapy.

FAdV-4 can cause more noticeable clinical symptoms compared to FAdV-8b after infecting specific pathogen free chickens.

Chickens infected with FAdV-4 and FAdV-8b both exhibit hepatic lesions characterized by hemorrhagic necrosis and intranuclear inclusion body formation. However, only FAdV-4 induces pericardial effusion and acute mortality in chickens. To investigate the similarities and differences in the pathogenicity of HPS and IBH, this study intends to compare the infectivity and pathogenicity of FAdV-4 and FAdV-8b, 2 serotypes of fowl adenovirus isolated in our laboratory. The 2 viruses were respectively inoculated subcutaneously into SPF chicks at the neck. The clinical manifestations and pathological ch

Polymorphism of the transmembrane region of the MICA gene and human brucellosis.

We investigated the polymorphism of the transmembrane region of the MICA gene (major histocompatibility complex class I chain-related gene A) in relation to susceptibility to human brucellosis. We typed 114 patients with brucellosis and 121 healthy controls for MICA transmembrane polymorphism with polymerase chain reaction methods combined with fluorescent technology. We found a significant decrease in the frequency of the MICA-A4 allele in the patients with brucellosis compared with the controls (4.4% vs 10.3%, Pc = 0.03). The frequency of the MICA-A5 allele was increased in the group of pati

CNS infections with free-living amebas: neuroimaging findings.

OBJECTIVE: The purpose of this report is to describe the clinical history, treatment, pathology, and imaging in two cases of rare CNS infection caused by free-living amebas. The Naegleria fowleri and Acanthamoeba species cause primary amebic meningoencephalitis and granulomatous amebic encephalitis, respectively. We describe the neuroimaging findings of a case involving a nonspecific cerebral edema pattern in primary amebic meningoencephalitis and a case involving focal enhancing lesions in granulomatous amebic encephalitis. CONCLUSION: Primary amebic meningoencephalitis and granulomatous ame

Morphologic alterations in the long Q-T syndrome. Light and electron microscopic observations in the conduction system and in sympathetic trunks.

Post-mortem examinations of 2 patients with long Q-T syndrome revealed marked focal fibrosis and lipomatosis of the conduction system as well as focal round cell ganglionitis of both sympathetic trunks. The patients, young women of different nationality displaying syncopal attacks and a long Q-T interval in the ECG died suddenly of ventricular arrhythmias. A family study of one of them revealed Q-T prolongation in 4 generations. The ultrastructure of the conductive tissue and the ventricular myocardium showed no specific alterations indicative of a primary metabolic defect. Ganglionitis of the

Intrarenal vascular changes in adult patients with recurrent haematuria and loin pain--a clinical, histological and angiographic study.

Clinical features have been correlated with renal function, histology and selective renal angiography in 19 patients with recurrent painless haematuria, recurrent loin pain, or both haematuria and loin pain in whom urinary infection, calculi and anatomical abnormalities of the urinary tract had been excluded. No deterioration in renal function was observed in any patient over periods of up to nine years. Although all patients showed similar glomerular changes histologically, consisting of focal and segmental mesangial thickening and proliferation and periglomerular fibrosis, mild tubular damag

Random spatial distribution of Schistosoma mansoni and hookworm infections among school children within a single village.

Schistosomes and soil-transmitted helminths currently infect a third of the world's human population. An important feature of these parasitic infections is their focal distribution, which has significant implications for control. Only a few studies have been carried out at the microepidemiological scale, comparing infection levels among individuals or households within a single village. In this study, data are presented from a cross-sectional survey, examining all children attending a primary school in rural Côte d'Ivoire over several consecutive days for Schistosoma mansoni, soil-transmitted

Successful recovery of associated interstitial nephritis and focal segmental glomerulosclerosis in patients with HCV and HIV treated with sofosbuvir and daclatasvir and revision of literature.

In this report, we describe the coexistence of two rare and debated complications of hepatitis C virus (HCV) infection: interstitial nephritis, with associated focal glomerulosclerosis, and autoimmune hepatitis, in a 55-year-old HIV/HCV-coinfected woman. The patient was treated for the immune-mediated manifestations with mycophenolate mofetil, which she continued for 9 years, as symptoms relapsed at every attempt to discontinue immunosuppression. The patient finally cleared HCV infection thanks to new direct-acting agents and could discontinue immunosuppressive therapy maintaining stable condi

Discrete papular lichen myxoedematosus: a rare subtype of cutaneous mucinosis.

Primary cutaneous mucinoses are characterized by abnormal mucin deposits in the skin. Discrete papular lichen myxoedematosus (DPLM) is an unusual subtype. Only 11 of the cases described in the literature to date showed no relation to human immunodeficiency virus (HIV) infection. We report a 21-year-old woman with numerous symmetrical asymptomatic papules on her trunk, arms and thighs. Results of investigations were normal. On histological examination, the upper dermis showed a focal mucin deposit. DPLM can be associated with HIV or C hepatitis virus (HCV) infection and therefore an early diagn

Orofacial infections in Iraq.

In Iraq, oral and maxillofacial (OMF) infections are common; these infections may be of odontogenic or nonodontogenic origin. The former, the commoner, gains importance from the catastrophic consequences that may take place in neglected or mismanaged patients and, as a paradox, from a fact that most of these infections are avoidable and preventable if a timely and accurate management took place for the primary dental problem. Nonodontogenic infections are not uncommon and are of life-threatening potential. Most of these infections are associated with an underlying medical condition that, whene
