[Madura's foot (mycetoma)].

Mycetoma is a chronic disorder involving the skin, bones and soft tissues and is characterized by multiple discharge sinuses, nodules and swelling of the affected part. It may be caused by actinomycetes or eumycetes. It most commonly involves the foot but occasionally involve the hands, back or shoulders. We describe a 66-year-old male agricultural worker, hospitalized in Military Hospital of Craiova, for leg amputation caused by the disease evolution without response at treatment that was initiated 20 years ago.

The diagnosis of onychomycosis in a geriatric population: a study of 450 cases in South Florida.

An investigative study was performed to determine the diagnosis of onychomycosis in a South Florida geriatric population. In this study, 450 cases of suspected onychomycosis involving men and women 65 years of age and older from a private practice office and two nursing home settings were used. Samples were taken from the hallux toenail and sent to a mycology laboratory for fluorescent potassium hydroxide (KOH) preparation and microscopic examination of a fungal culture. Of the 450 cases studied, 46.4% of the patients had a single fungal organism cultured, 30.4% had a mixed fungal infection cu

Isolation of both Sporothrix schenckii and Nocardia asteroides from a mycetoma of the forefoot.

Mycetoma is a localized primary subcutaneous infection due to fungi (eumycetoma) or aerobic actinomycetes (actinomycetoma). We report a patient who acquired an implantation infection on the forefoot following a motorcycle accident in Crete. Both Sporothrix schenckii and Nocardia asteroides were isolated simultaneously from the lesion. Under combined therapy with itraconazole and trimethoprim-sulphamethoxazole for 7 months the lesion healed completely. A combination of causative organisms in mycetomas is rare, and the combination of S. schenckii and N. asteroides together has not been reported

[Deep granuloma annulare in children].

BACKGROUND: The granuloma annulare is a common benign disorder in childhood which may be difficult to recognize in its atypical forms, like deep granuloma annulare. CASES REPORT: Case 1: A 5-year-old girl suffered from subcutaneous nodules occurring on her soles. Open biopsy at the age of 6 years showed typical features of granuloma annulare. These nodules were still present one year later. Case 2: A 2-year-old girl suffered from nodular lesions on the anterior aspect of the lower legs similar to erythema nodosum with spontaneous outbreaks over 6 months. Histological examination showed typica

A case of peculiar plantar warts. Human papillomavirus type 60 infection.

BACKGROUND: Various kinds of viral warts have their own typical clinical appearances and histologic properties. From each of them, a corresponding unique type of human papillomavirus (HPV) has been detected. More than 60 types of HPV have been identified from the warts and other squamous cell epithelial tumors. Human papillomavirus type 60 has been identified in an epidermal cyst on the sole. We present a case of a peculiar plantar wart infected with HPV type 60. OBSERVATIONS: A 21-year-old male student complained of yellowish hyperkeratotic slightly elevated nodules on the right plantar arch

Localized crusted scabies in the acquired immunodeficiency syndrome.

Crusted scabies (CrS) is an uncommon occurrence among patients with AIDS. Indeed to date only five cases have been described, all with widespread lesions. A case of localized CrS appearing as a yellowish and crusted plaque on the second right toe is reported in a woman with AIDS. Scraping off the verrucous surface, as well as punch-biopsy revealed many mites within the horny layer. The infestation is related to the cutaneous immune response and thus CrS should be considered an opportunistic infestation in AIDS. The importance of the early diagnosis of CrS in order to prevent disseminated lesio

[Idiopathic white atrophy causing cutaneous thrombotic microangiopathy. Apropos of a case].

Authors report clinical data on a 59-year-old woman, bearing, since 15 years, painful purpuric and necrotizing macules and papules on the legs, leading to white atrophic scars. On examination one could see lesions in various stages of development. Clinical examination and all laboratory investigations were otherwise normal. First cutaneous biopsy led to an erroneous diagnosis of vasculitis with subsequent corticotherapy. Second biopsy displayed numerous fibrinous thrombi inside lumens of superficial dermal capillaries without fibrinoid necrosis of their parietal layer nor inflammatory infiltra

[A new type of viral wart? A case of viral wart having intracytoplasmic filamentous substances].

Reported was a 16-year old Japanese girl suffering from peculiar viral warts on the soles. All lesions showed whitish punctate keratotic appearance. Three biopsy specimens taken from one patient were found to have particular intracytoplasmic basophilic eosinophilic filamentous substances in each cell. These intracytoplasmic substances were quite different from those of myrmecia, which was described as inclusion wart by Lyell and Myles. Myrmecia is caused by human papillomavirus type 1 (HPV-1) and its intracytoplasmic substances are characterized by numerous intracytoplasmic eosinophilic granul
