Laparoscopic treatment of ureteroduodenal fistula: Case report, technique, and review.

Ureteroduodenal fistula is extremely rare, with fewer than 20 reported cases. We describe a 50-year-old woman with recurrent flank pain and hematuria whose imaging showed proximal ureteral hydronephrosis, a 2-cm stone, and a migrated metallic foreign body. During laparoscopic ureterolithotomy, dense adhesions revealed a fistula caused by transduodenal needle penetration with associated stone formation. Laparoscopic removal of the stone and foreign body, duodenal repair, and ureteral suturing with stent placement were successfully performed. The patient recovered uneventfully and remained asymp

[Abdominal parietal abscess secondary to the absorption of a knife by a mental patient, Treichville University Hospital, Côte d'Ivoire].

CASE REPORT: We present the successful management of the consequences of voluntary knife ingestion in a 24-year-old male with undocumented psychiatric disorders. The patient was admitted to the emergency department with a fistulized epigastric skin abscess, with the tip of a knife protruding from it. This extraordinary case involved the migration of an ingested knife, which perforated the stomach and externalized through an epigastric abscess. Surgical removal of the foreign body was performed. DISCUSSION: Several hypotheses explain the ingestion of the knife in a person with psychiatric diso

Epidemiology and Surgical Management of Foreign Bodies in the Liver in the Pediatric Population: A Systematic Review of the Literature.

Retention of foreign bodies (FB) in the liver parenchyma is a rare event in children but it can bring a heavy burden in terms of immediate and long-term complications. Multiple materials can migrate inside the liver. Clinical manifestations may vary, depending on the nature of the foreign body, its route of penetration and timing after the initial event. Moreover, the location of the FB inside the liver parenchyma may pose specific issues related to the possible complications of a challenging surgical extraction. Different clinical settings and the need for highly specialized surgical skills m

Thread-like Bone Cement in the Right-side Heart and Pulmonary Arteries Causing Diffuse Pulmonary Embolism as a Late Complication.

Pulmonary embolism (PE) is usually associated with deep vein thrombosis (DVT) in the lower extremities. However, foreign bodies in the pulmonary arteries can rarely cause PE without evidence of DVT. In this report, we present a case of diffuse PE associated with a migrated thread-like structure of the right-side heart and pulmonary arteries in a 70 year-old woman. The patient underwent several episodes of percutaneous vertebroplasty (PV) to treat compression fractures. The thread-like foreign bodies were identified as the bone cement injected during the previous PV procedures. We present this

Penile augmentation with resultant foreign material granuloma and sequalae.

Throughout history, a proportion of men appear to correlate penis size and dimensions directly with physical fitness and sexual prowess. Foreign materials, such as paraffin oil, paraffin balm, mineral oils, and silicone, have been used to promise an improvement in penile shaft contour and dimensions. These materials are injected directly into the penis; inducing granuloma formation to achieve increased penis length and girth. However, the result is a severely disfigured and swollen penis, which cannot achieve erection. Local complications of penile lipogranuloma include infection, ulceration,

A broken sewing needle in the knee of a 4-year-old child: is it really inside the knee?

We report on a case of a broken needle that migrated inside the knee joint of a 4-year-old girl. Searching for any small foreign body in the knee joint is not easy in either open or arthroscopic procedures. In this case, the surgery was made more difficult because of technical delays and diagnostic difficulties in defining the surgical plane of the needle. Arthroscopic expertise and some basic precautions can minimize the morbidity to a young patient and prevent migration into the knee joint proper.

A modified surgical technique for ventriculoperitoneal shunt placement using a non-burr hole valve in a pediatric population.

PURPOSE: This study evaluated a modified anchoring technique using a straight connector in non-burr hole valve shunt systems to reduce catheter migration in children under 3 years of age. METHODS: We retrospectively reviewed records of 48 children (< 3 years) who received frontal ventriculoperitoneal (VP) shunt placement using a non-burr hole valve system between January 2017 and July 2022. RESULTS: Catheter migration occurred in 18 of 48 patients. Migration was significantly higher with the right-angle adaptor (61.5%) than with the straight connector (9.1%; p = 0.0002). Younger age at surge

Delayed brain and spine migration of a retained SEEG electrode fragment: An unexpected complication.

BACKGROUND: Stereoelectroencephalography (SEEG) is a well-established technique for localizing epileptogenic zones in patients with drug-resistant epilepsy, including children. While considered safe, rare but serious complications can occur. CASE PRESENTATION: We report the case of a girl with tuberous sclerosis complex and drug-resistant epilepsy who underwent SEEG implantation as part of pre-surgical evaluation. Electrode placement was uneventful. However, during SEEG monitoring, the signal was lost on two electrodes, raising suspicion of fracture. During explantation, these electrodes were

Evaluation and refinement of thresholds for early migration of total knee replacements as an estimator of late aseptic loosening: an updated systematic review of RSA and survival studies.

BACKGROUND AND PURPOSE: This study updates 2 parallel systematic reviews and meta-analyses from 2012, which established the 1-year radiostereometric (RSA) migration thresholds for tibial components of total knee replacements (TKR) based on the risk of late revision for aseptic loosening from survival studies. The primary aim of this study was to determine the (mis)categorization rate of the 2012 thresholds using the updated review as a validation dataset. Secondary aims were evaluation of 6-month migration, mean continuous (1- to 2-year) migration, and fixation-specific thresholds for tibial c

Oral extrusion of a cervical screw: A rare late complication of anterior cervical discectomy and fusion.

Anterior cervical discectomy and fusion (ACDF) is a commonly used surgical technique for treating degenerative and traumatic conditions of the cervical spine. However, late-onset implant extrusions following this procedure are extremely rare but can lead to serious complications. In this case report, we present a 78-year-old male who presented to the emergency department with a sole complaint of hemoptysis, 16 years after undergoing ACDF surgery, and was found to have spontaneously expelled a screw through his mouth. Imaging studies revealed the anterior migration and pharyngeal extrusion of t

Filler found in bone: Surgical removal of polyacrylamide hydrogel (Amazingel) from the mandible after 24 years: A case report and literature review.

Polyacrylamide hydrogel (PAAG) is widely regarded as a safe soft tissue filler and has been extensively utilized for cosmetic enhancements, such as breast and facial augmentation in China from 1997 until its ban in 2006. Common complications associated with its use include inflammation, infection, granulomas, fibrosis, gel migration, and facial and soft tissue deformities. This case report describes a 45-year-old Chinese woman who experienced PAAG migration into her mandible 24 years after facial augmentation, causing irritation of the mandibular alveolar nerve - apparently the first documente

Sinking bullet syndrome: A unique case of transhemispheric migration.

BACKGROUND: Spontaneous migration of retained intracranial bullet fragments is an increasingly recognized phenomenon. However, such migration is usually limited in extent, since it occurs along the bullet tract or cerebrospinal fluid (CSF) spaces. Transhemispheric migration through an intact cerebral hemisphere has not been previously reported. OBSERVATIONS: A 20-year old man sustained a gunshot wound (GSW) to the head with a left parieto-occipital entry point, resulting in retained bullet fragments within the anterior right frontal lobe. The patient developed medically refractory intracrania

Retroperitoneal Migration And Transgluteal Extrusion of a Distal Lumboperitoneal Shunt Catheter: An Unusual Complication of Cerebrospinal Fluid Shunt.

Cerebrospinal fluid shunts provide many and different complications. Only 1-3% of them are rare, related to migration, and perforation of hollow organs. Even more unusual are the complications associated with the distal catheter leaving the peritoneal cavity and the tip migrating through some of the anatomical foramens to the muscle layers on the back. We present a rare complication, migration of the distal lumboperitoneal shunt catheter through the retroperitoneum and the gluteal muscles to the hypoderma. The peritoneal catheter entered the retroperitoneal area, passed through the greater sci

Gastric perforation by a ventriculoperitoneal shunt in an adult.

Ventriculoperitoneal (VP) shunt operation is a common neurosurgical procedure applied for managing intracranial hydrocephalus. Migration of a distal catheter is an uncommon complication, and related gastric perforation is rarely reported. Herein, we report the case of gastric perforation in a patient with VP shunt who presented with fever. The distal catheter within the stomach was confirmed by preoperative computed tomographic scan of abdomen and gastric endoscopy. Surgical intervention and appropriate antibiotics management enabled patient recovery. This case emphasizes the importance of ear

Gelfoam Migration: A Potential Cause of Recurrent Hydrocephalus.

BACKGROUND: Gelfoam is a simple and effective hemostatic agent that is used to seal brain corticotomies or skull burr holes. Owing to its low cost, it is one of the most widely used tools in neurosurgical daily practice. However, migration of Gelfoam fragments can cause occlusion of endoscopic third ventriculostomy (ETV) or shunt, leading to hydrocephalus recurrence. CASE DESCRIPTION: Two cases of Gelfoam migration causing recurrent hydrocephalus are presented: a 12-year-old girl who underwent surgery for posterior fossa tumor removal and ETV for associated hydrocephalus, where a portion of G

Medial migration of the tympanostomy tube: what is the optimal management option?

The tympanostomy tube insertion is the gold standard of treatment for secretory otitis media. Complications are associated with this surgery in 17% of cases. One of the rare but real complications is the medial migration of the tympanostomy tube. To our knowledge, this is the 14th case reported in childhood. Considering this rarity, there is no consensus for the management of this phenomenon. Some authors propose a surgical removal, while others prefer to observe whether the patient is asymptomatic. We reported a case of migration of the tympanostomy tube and described the detailed clinical fe
