Inert intraocular eyelash foreign body following phacoemulsification cataract surgery.

INTRODUCTION: We describe the first reported case of tolerated eyelash inoculation following phacoemulsification surgery, with 4 years follow-up, that did not result in endophthalmitis or low grade chronic uveitis. CASE REPORT: A 79-year-old man with bilateral cataracts underwent phacoemulsification and posterior chamber intraocular lens implantation in the right eye. Three months later, he was found to have an eyelash within the right anterior chamber. The patient opted to leave his asymptomatic right eye alone. Four years later, he was referred to our hospital for review of his intraocular

Evaluation of alloderm and autologous skin in quadriceps muscles of rats for injection laryngoplasty.

There are three surgical methods utilized in treating glottic insufficiency: laryngeal framework surgery, reinnervation procedures and injection medialization. The trend towards the use of less invasive procedures led us to focus on injection medialization in this study. The advantages of vocal fold injection are that the procedure requires no external incisions, is easily applicable and can be applied in an office setting. Furthermore, injection medialization is applicable in cases of atrophic vocal fold and vocal fold scarring. In developing injection medialization, laryngologists are in sea

Attenuating Chronic Fibrosis: Decreasing Foreign Body Response with Acellular Dermal Matrix.

Surgical implants are increasingly used across multiple medical disciplines, with applications ranging from tissue reconstruction to improving compromised organ and limb function. Despite their significant potential for improving health and quality of life, biomaterial implant function is severely limited by the body's immune response to its presence: this is known as the foreign body response (FBR) and is characterized by chronic inflammation and fibrotic capsule formation. This response can result in life-threatening sequelae such as implant malfunction, superimposed infection, and associate

Syneresis and vitreous hemorrhage in pseudophacia.

The eye of a 60-year-old man with a clinically very successful lens implant became available for pathological study after a very sudden and uncomplicated death about 6 months following cataract surgery. The following changes were found: slight non-granulomatous uveitis associated with foreign-body reaction to the lens implant and its supporting iris suture, absence of the anterior vitreous face, advanced syneresis of the central vitreous, vitreous hemorrhage with diffuse layering of erythrocytes on the inner aspect of the remaining vitreous crust, vitreous traction on peripheral and central re…

[The use of bisphosphonates in arthroplasty].

The use of bisphosphonates in joint arthroplasty is the latest field of application for these agents. The mechanism of action of bisphosphonates suggests that they may optimize long-term survival of the implant. Most important is their potency in suppressing periprosthetic osteolysis due to the inflammatory foreign body reaction of wear debris, to decrease periprosthetic osteopenia caused by stress-shielding and to improve the osseointegration of cementless metal implants. The present review provides the latest information on definite and presumed mechanisms of action of bisphosphonates and th

Aspiration Pneumonia Due to Migration of Fracture Tracheostomy Tube after 14 Years of Use.

Tracheostomy is a common life-saving surgical procedure, which has its own short- and long-term surgical complications. Occasionally, after being in place for several years, the tube may fracture, causing a foreign body reaction in the bronchus followed by life-threatening pneumonia. We report a rare case of a 29-year-old man with a known leukodystrophy disorder whose tracheostomy tube was never changed in 14 years. He presented with signs of sepsis and respiratory distress. The management and intraoperative findings, including recommendations for tracheostomy care, were described.

Pseudolymphoma tattoo-induced.

Tattooing has become more and more popular in today's society. The most common dermatological tattoo complications are represented by hypersensitivity reaction to tattoo pigments like irritant and allergical contact dermatitis, development of lichenoid areas and granulomatous responses, such as sarcoid granulomas or foreign body granulomas. Less frequently patients developing discoid lupus erythematous have been reported. Pseudolymphoma is an uncommon reactive lymphocytic proliferation mimicking the histological and clinical features of a malignant skin lymphoma. We herein report a pseuldoymph

Migration of Cymetra after vocal fold injection for laryngeal paralysis.

Injection laryngoplasty is a common operative technique used in the management of unilateral vocal fold paralysis. Cymetra is a micronized particulate injectable form of acellular human dermis that is commonly used for vocal cord medialization procedures. We report migration of an intracordal bolus of Cymetra into the medial wall of the pyriform sinus. Histopathology from endoscopic resection of the migrated Cymetra demonstrated a localized foreign body reaction with characteristic giant cells. Specific anatomic considerations through cadaveric laryngeal dissection are presented to demonstrate

Lipoid pneumonia: a preventable form of drug-induced lung injury.

We report a case where recurrent "pneumonia" was eventually diagnosed as lipoid pneumonia in an elderly patient with cerebrovascular disease. The discontinuation of paraffin oil laxative led to clinical improvement. Lipoid pneumonia, a foreign body-type reaction to the presence of lipid within lung parenchyma, is probably underdiagnosed and underreported, and paraffin oil laxative is the main causative agent. Paraffin oil is marketed as a food additive, and no information about its hazards is provided to clinicians or patients. We suggest that a change in paraffin oil licensing may decrease th

Granuloma annulare: an immunohistochemical study.

The monocyte/histiocytic response in granuloma annulare has not been extensively studied. We studied the immunohistochemical staining pattern in granuloma annulare by using a panel of markers for "histiocytic" cells including Ham 56, KP1, factor XIIIa, Mac 387, vimentin, and lysozyme. The infiltrate failed to stain with the histiocytic markers, except with antibodies against vimentin and lysozyme. Commonly used histiocytic markers for infectious, immunogenic, and foreign body granulomas fail to stain the infiltrate in granuloma annulare. We hypothesize that a locally derived, immunologically d

Pathology of arteriovenous malformations embolized with isobutyl-2-cyanoacrylate (bucrylate). Report of two cases.

There is controversy as to the possible toxic effects of isobutyl-2-cyanoacrylate (bucrylate) when this substance is used for purposes of therapeutic embolization. Two cases are presented in which cerebral arteriovenous malformations were resected, one 42 days and the other a year after bucrylate embolization. In both, pathological examination revealed a brisk intimal foreign-body giant-cell reaction wherever bucrylate was present in a vessel, along with chronic inflammation in the vessel walls and adjacent brain parenchyma. The findings are discussed in the light of other observations on the

[Disease of metalgrinding workers (author's transl)].

It is a specific clinical picture with extensive soft tissue gas and swelling of the forearm. Usually, the clinical course is without any other symptoms. Therapy is not necessary because gas disappears in few days. The etiology of the disease is not clear. Gas can be generated by the chemical reaction between a metallic foreign body and tissue fluid. Spread of benzine evaporation has been considered as a possible source as well. The disease is very rare and its knowledge is important in differential diagnosis of gas gangrene.

Molecular Biointerface Characterization for an Implanted Medical Device Using Cryogenic Orbitrap Secondary Ion Mass Spectrometry (Cryo-OrbiSIMS).

Implanted medical devices often fail due to foreign body reactions (FBRs), a process that is still not fully understood. This work presents a depth profiling approach to provide insight into the spatial metabolomics of the biointerface of implants, revealing biomolecular strata representative of the host response. This study examines silicone rubber poly(dimethylsiloxane) catheters implanted in mice for 1 and 28 days. Cryo-OrbiSIMS was used in combination with ToF-SIMS to identify metabolite profiles from the biological deposit found on the implants after removal from the tissue, which were pr

First Insights on the Upcoming Role of Next-Generation PLLA-LASYNPRO in Aesthetic and Regenerative Medicine: A Survey of Experts-the PLLA-LASYNPRO Rationale.

INTRODUCTION: Injectable collagen stimulators have traditionally been linked to inflammatory foreign-body reactions (FBR) as a mechanism of action. However, the next-generation PLLA-LASYNPRO microspheres, contained in the CE-marked JULÄINE medical device, may represent a paradigm shift. Preclinical data suggest these microspheres can promote collagen and extracellular matrix (ECM) regeneration with minimal inflammatory response. OBJECTIVES AND HYPOTHESES: This study aimed to evaluate the scientific soundness and clinical relevance of a non-inflammatory mechanism of action for PLLA-LASYNPRO. T…

Unraveling and Harnessing the Immune Response at the Cell-Biomaterial Interface for Tissue Engineering Purposes.

Biomaterials are defined as "engineered materials" and include a range of natural and synthetic products, designed for their introduction into and interaction with living tissues. Biomaterials are considered prominent tools in regenerative medicine that support the restoration of tissue defects and retain physiologic functionality. Although commonly used in the medical field, these constructs are inherently foreign toward the host and induce an immune response at the material-tissue interface, defined as the foreign body response (FBR). A strong connection between the foreign body response and

Poly(Glutamic Acid-Lysine) Hydrogels with Alternating Sequence Resist the Foreign Body Response in Rodents and Non-Human Primates.

The foreign body response (FBR) to implanted biomaterials and biomedical devices can severely impede their functionality and even lead to failure. The discovery of effective anti-FBR materials remains a formidable challenge. Inspire by the enrichment of glutamic acid (E) and lysine (K) residues on human protein surfaces, a class of zwitterionic polypeptide (ZIP) hydrogels with alternating E and K sequences to mitigate the FBR is prepared. When subcutaneously implanted, the ZIP hydrogels caused minimal inflammation after 2 weeks and no obvious collagen capsulation after 6 months in mice. Import

Oral formulation of Wnt inhibitor complex reduces inflammation and fibrosis in intraperitoneal implants in vivo.

The use of implantable biomaterials to replace physiological and anatomical functions has been widely investigated in the clinic. However, the selection of biomaterials is crucial for long-term function, and the implantation of certain biomaterials can cause inflammatory and fibrotic processes, triggering a foreign body reaction that leads to loss of function and consequent need for removal. Specifically, the Wnt signaling pathway controls the healing process of the human body, and its dysregulation can result in inflammation and fibrosis, such as in peritoneal fibrosis. Here, we assessed the

Engineered collagen polymeric materials create noninflammatory regenerative microenvironments that avoid classical foreign body responses.

The efficacy and longevity of medical implants and devices is largely determined by the host immune response, which extends along a continuum from pro-inflammatory/pro-fibrotic to anti-inflammatory/pro-regenerative. Using a rat subcutaneous implantation model, along with histological and transcriptomics analyses, we characterized the tissue response to a collagen polymeric scaffold fabricated from polymerizable type I oligomeric collagen (Oligomer) in comparison to commercial synthetic and collagen-based products. In contrast to commercial biomaterials, no evidence of an immune-mediated foreig
