Cemented humeral surface replacement for a locked posterior fracture-dislocation: a case report.

Posterior fracture-dislocations of the shoulder are uncommon and challenging lesions. We present the case of a 76-year-old male patient who sustained a locked posterior humeral head fracture-dislocation during a convulsion. Because more than 40% of the articular surface was involved, preserving the head was not possible. We treated the fracture with a cemented humeral surface replacement. The patient demonstrated good shoulder function and no instability on short-term follow-up. Surface replacement may be a valuable treatment option for fracture-dislocations of the shoulder with articular surf

Bilateral traumatic anterior dislocation of the hip with an unstable lumbar burst fracture.

Traumatic anterior dislocation of the hip is rare. Bilateral traumatic anterior dislocation is an even rarer injury; indeed, only 5 cases have been reported in the English literature. We describe a case of a bilateral traumatic anterior dislocation of the hip and a concomitant unstable lumbar burst fracture following a mechanism of injury distinctly different from other reports.

Lateral talar process fracture and peroneal tendon dislocation: a previously unrecognized injury complex.

BACKGROUND: Lateral talar process fractures and peroneal tendon dislocations are frequently unrecognized at the time of injury. Lateral process fractures were initially classified by Hawkins as three types. Type II injuries are comminuted fractures involving both the talofibular and talocalcaneal articular surfaces. The purpose of this retrospective chart review was to describe an injury complex of Type II lateral talar process fracture with peroneal tendon dislocation. MATERIALS AND METHODS: Between January of 1995 and December 2006, 13 patients were seen for a lateral talar process fracture

Combined approach for a locked unilateral facet fracture-dislocation of the cervicothoracic junction.

The authors present the case of a 36-year-old patient who sustained a unilateral fracture-dislocation C7-T1 involving all three columns, given the lesion of the C7-T1 disc on MRI. In view of the fractured facet, closed reduction without anaesthesia was not attempted. First, open reduction and instrumentation were performed from posteriorly. In a second operation, anterior fusion C7-T1 was added, using a tricortical bone graft and instrumentation. The authors felt that this three-column lesion at the cervicothoracic junction necessitated combined posterior-anterior stabilisation.

Ipsilateral compound distal radius fracture with missed elbow dislocation. A rare injury pattern.

A rare injury pattern of compound distal radius fracture associated with a missed posterolateral elbow dislocation in an adult is described. To the best of our knowledge, this has not been previously described in the literature. The probable mechanisms of injury are speculated to include longitudinal compressive force along with valgus and external rotatory stress to the semiflexed elbow leading to elbow dislocation. A continuation of the impact when her hand hit the ground led to her hand deviating radially resulting in compounding on the ulnar side. Early reduction of the dislocation and rig

Incarcerated patellar tendon in Hoffa fracture: an unusual cause of irreducible knee dislocation.

Irreducible knee dislocation due to incarcerated soft tissue structures in associated fractures is extremely uncommon. We report a case of irreducible, closed dislocation of the knee secondary to incarcerated patellar tendon in a Hoffa fracture (unicondylar fracture of femur in the coronal plane) with associated communited fracture of patella and disruption of both cruciates. We treated this case initially with open reduction and internal fixation of fracture and repair of the patellar tendon. During the total followup period of eighteen months, the patient had restricted joint range of motion

Dislocation of the shoulder joint with ipsilateral humeral shaft fracture: two case reports.

INTRODUCTION: A combination of the shaft fracture of the humerus with shoulder dislocation is a rarely documented injury. Moreover, few reports describe a long-term outcome after a follow-up period over 10 years. The present article is the first report of long-term clinical results obtained with this combined injury. MATERIALS AND METHODS: We present two cases of the dislocation of the shoulder joint with ipsilateral humeral shaft fracture. One is an anterior dislocation and the other is posterior. The anterior dislocation was closely reduced on the date of injury, but the posterior dislocati

Lisfranc fracture-dislocation in patients with multiple trauma: diagnosis with multidetector computed tomography.

BACKGROUND: We assessed acute phase multidetector computed tomography (MDCT) findings of Lisfranc fracture-dislocations in patients with multiple trauma referred to a Level I trauma center over a 29-month period. METHODS: Two hundred and eighty two patients (208 male and 74 female) between the ages of 13 and 89 (mean 42) years had, at the request of emergency room physicians, MDCT of the foot and ankle after acute injury. RESULTS: A total of 21 Lisfranc fracture-dislocations were found in 19 (7%) patients. Two main injury mechanisms were established: falling from high places in 10 injuries (

[Surgical treatment of fracture-dislocation of connect of cervical vertebrae and thoracic vertebrae].

OBJECTIVE: To discuss the value of anterior operation in fracture-dislocation of connect of cervical vertebrae and thoracic vertebrae. METHODS: From 1997, 21 patients with fracture-dislocation of connect of cervical vertebra and thoracic vertebrae were operated on. The interval between injury and operation was within 24 hours in 4 cases, 2-7 days in 9 cases, 8-14 days in 3 cases and within 4 weeks in 5 cases. The locations were C6,7 in 7 cases, C7 in 9 cases and T1 in 5 cases. The nerves function of spinal of all the cases have different degrees of injury. Classification of Frankel were the f

[Dislocation of the Ulnar Head following Malunion of Distal Radius Fracture in Childhood].

Four children between the age of 10 and 13 years with distal radius fractures healed with dorsal angulation presented to us in our out-patient department. All had been treated conservatively with an upper-arm cast, all fractures had healed with an average dorsal angulation of 10 degrees. Despite this dorsal angulation, the patients were pain free. Subsequent minor injury resulting in pain to the wrist was now the reason for their presentation. On examination we found a spontaneous palmar dislocation of the ulnar head during supination. This proved to be resistant to conservative therapy. In al

Arthroscopic treatment for traumatic hip dislocation with avulsion fracture of the ligamentum teres.

Traumatic hip dislocation in children is a rare condition. Immediate closed reduction is a treatment of choice, but when incongruent reduction results, it can be due to interposition and requires open arthrotomy. We present a previously unreported technique of arthroscopic treatment that was used successfully for traumatic hip dislocation in a 10-year-old girl with impingement of the avulsed ligamentum teres. Arthroscopy was effective in confirming the exact nature of the interposition material and excising the fragment with minimal invasion.

Irreducible fracture-dislocation of the hip: a severe injury with a poor prognosis.

OBJECTIVE: To determine the outcome of and prognostic factors associated with irreducible fracture-dislocations of the hip. DESIGN: Retrospective review of a prospectively gathered trauma database. METHODS: Using a prospectively gathered trauma database, we identified twenty-five patients with fracture-dislocations of the hip that were irreducible with closed means under a general anaesthetic. All were victims of high-energy trauma with severe injuries (mean Injury Severity Score = 25, range 9 to 54). Eighteen patients had multiple fractures, fifteen had associated systemic injury, and only

Carpometacarpal dislocation of the thumb associated with ipsilateral fracture of the distal radius.

An acute carpometacarpal (CMC) dislocation of the thumb is itself an extremely rare injury, but one associated with a fracture of the distal radius has never before been reported. In our case, the fracture of the distal radius was treated by closed reduction and immobilization with a plaster cast, whereas the dislocation of the CMC of the thumb was treated with closed reduction, percutaneous fixation with Kirschner wires, and immobilization with a plaster cast. At the patient's two-year follow-up examination, the distal radius fracture displayed successful functional and radiographic results,

Late-onset sternomanubrial dislocation with progressive kyphotic deformity after a thoracic burst fracture.

Fractures and dislocations of the sternum may be associated with flexion-compression injuries of the thoracic spine. Sternal injuries most commonly occur at or near the sternomanubrial joint. We present a patient with a known thoracic spine fracture who developed a subsequent late-onset, symptomatic sternomanubrial dislocation and progression of thoracic kyphosis, ultimately requiring operative fixation of both the sternum and the spine. Internal fixation of these sternal injuries should be considered in the setting of a flexion-compression thoracic spine fracture to possibly prevent a worseni
